CHECKLIST OF EMPLOYMENT REQUIREMENTS (NEW AND PROMOTED EMPLOYEES)

¨ PERSONAL DATA SHEET


Fully-accomplished CS Form No. 212, Revised 2017. Fields that are not applicable should be marked “N/A”.
¨ MEDICAL CERTIFICATE
Use the CS Form No. 211, Revised 2018 as certified by any licensed government physician that the employee is fit to work.
Medical results should be attached as evaluated or required by the physician. Drug Test results are mandatory.
¨ OATH OF OFFICE*
Use CS Form No. 32, Revised 2018 with attached photocopy of any valid government ID with signature.
¨ STATEMENT OF ASSETS, LIABILITIES AND NET WORTH (SALN)*
Use the SALN Form, Revised 2015. Must be notarized for Regular, Temporary, Coterminous and Casual employees; no need
for notarizJob Order and Contract of Service personnel are not required to notarize the SALN Form but all are required.
¨ POSITION DESCRIPTION FORM
Use DBM-CSC Form No. 1, Revised Version No. 1, s.2017, signed by the personnel or employee and the office/department
head.
¨ BIR REGISTRATION FOR TAX IDENTIFICATION NUMBER (TIN)
 BIR 1901 for Job Order and Contract of Service Personnel – use the BIR Form 1901 – Application for Registration. BIR
will require a copy of contract, a payment of Php 500.00 within the first month of effective date of contract (penalties apply
thereafter). Submit photocopy to HRMO of the BIR-received BIR Form 1901 with receipt and a notarized Affidavit of
Sworn Declaration (R-11-2018 Annex B-1 for several income payors / Annex B-2 for lone income payor).
 BIR 1902 for Regular, Temporary, Coterminous and Casual Employees – Submit accomplished form to HRMO with
photocopy of two (2) valid government-issued ID and birth certificate.
 Note that employees with existing TIN and not registered to the Revenue District Office (RDO) No. 018 – Olongapo City is
required to transfer to RDO 018 by filing BIR Form No. 1905 at their home RDO.
¨ PAG-IBIG MDF RECORD*
Accomplish Pag-IBIG Member’s Data Form (MDF) and submit to HRMO for registration or MID verification if with existing Pag-
IBIG MID.
¨ PHILHEALTH MDR FORM*
To secure PhilHealth MDR, completely fill up PhilHealth Member Registration Form (PMRF) with remarks of HRMO for Job
Order and Contract of Service personnel. PhilHealth ER2 Form signed by HRMO is an additional requirement for Regular,
Temporary, Coterminous and Casual employees. Accomplished forms should be submitted to PhilHealth Office at 1120 Rizal
Ave., East Tapinac, Olongapo City 2200 to get a copy of PhilHealth Member’s Data Record (MDR) Form. Submit a copy of
your MDR to HRMO.
¨ GSIS FORM A*
Applicable only for Regular, Temporary, Coterminous or Casual employees. A copy of the form is available at HRMO, and our
office shall process your GSIS registration upon availability of appointment record.
¨ DBP ATM ACCOUNT
Employee shall open an ATM Account at Development Bank of the Philippines (DBP), Subic Bay Branch at Blk 3, Heemang
Bldg., Greenwoods Park, CBD Area, Subic Bay Freeport Zone 2222. You will be required to secure an endorsement first from
HRMO to submit to DBP Subic Bay with two (2) 1x1 photo, photocopy of valid government-issued ID and initial deposit of Php
200.00.
¨ EMPLOYEE ID
ID forms must be completely-filled up and submitted to HRMO for signing and data capture at the ID Center Office located
across Police Clearance releasing area beside PESO.
*Required for Payroll Purposes

¨ OLONGAPO CITY GOVERNMENT EMPLOYEES MULTIPURPOSE COOPERATIVE (OCGEMPC)
