AFFIDAVIT

I, RYAN DATIG GALVEZ, of legal age, Filipino, single, and a resident of P-4
Buhang , Magallanes, Agusan del Norte, under oath, depose and state:
1. That I am the brother of the late ALDIN DATIG GALVEZ, who died last
September 11, 2019 in Agusan del Norte Provincial Hospital, Butuan City;
2. That he died single and does not have any legitimate or illegitimate children;
3. That our mother is unable to process the said registration of death due to High Blood
Pressure;
4. That I am the one processing his registration of death because I was there on the
moment that he died;
5. That I am executing this affidavit to attest to the veracity of the foregoing for
whatever legal purpose it may serve.
