AFFIDAVIT

I, MERNA M. MILLO, of legal age, Filipino Citizen, married, and a resident of


Barangay Quinavite, Bauang, La Union do hereby depose and state that:

1. James M. Perez died intestate without any issue;

2. The late James M. Perez’s wife and siblings were also all dead;

3. As such, I am one of his immediate family and/or nearest kin by consanguinity


being a daughter of his late sister, Carla Perez Corpuz;

4. As a rule of succession, I am one of the legal heir of James M. Perez, entitled


to request documents and/or papers in connection with the decease;

5. I am executing this affidavit to attest to the foregoing and for all legal intents
and purposes.

IN WITNESS WHEREOF, I have hereunto set my hand this 10th day of


December, 2020 at _________, La Union.
