1. Check the order from the prescribing doctor/ chart of the patient.
2. Hand hygiene/hand washing.
3. Gather all equipment.

4. Prepare the medication from the vial or ampule for drug withdrawal.
Verify the expiration date of the sterile water, syringe and the medicine.

5. Before carrying out the procedure, it is important to introduce oneself


and confirm the client's identity in accordance with the agency's
established policy.

6. Provide privacy by closing the curtains / doors / windows.


7. Position the client in a manner that allows for relaxation of the arm, leg,
or abdomen, depending on the specific spot to be utilized.
8. Provide a clear and comprehensible explanation of the medication’s
intended purpose and its potential benefits to the client. Include pertinent
details regarding the impacts of the medicine.
9. Choose a location that is free of discomfort, firmness, swelling, scarring,
itching, burning, or localized inflammation. Choose a location that has not
been utilized regularly.
10. Thoroughly cleanse the skin at the location by applying pressure in a
circular motion, beginning from the center and gradually expanding the
circle outwards. Ensure that the area is completely dry.
11. Position the swab between the third and fourth fingers of the non-
dominant hand or place the swab on the client's skin above the target spot.
12. To prevent contamination of the needle, remove the cap directly by
pulling it off from the outer edge.
13. Hold the syringe firmly in your dominant hand by gripping it between
your thumb and fingers. Position your hand with the palm facing either to
the side or upward at a 45° angle for insertion, or with the palm facing
downward at a 90° angle for insertion, in preparation for the injection.
14. To perform the task, utilize the non-dominant hand to grip and stretch
the skin at the desired location. Then, with the dominant hand, enter the
needle by applying a firm and consistent force.
15. After inserting the needle, shift your non-dominant hand towards the
plunger's end. Certain nurses may find it more convenient to relocate their
non-dominant hand to the barrel of the syringe, while placing their
dominant hand on the end of the plunger.
16. Administer the medication by maintaining a stable position of the syringe
and applying gradual and consistent force to the plunger.
17. For optimal results, it is advised to keep the needle inserted into the
skin for 5 seconds following the full depression of the plunger, particularly
when administering subcutaneous injections such as insulin.
18. Withdraw the needle gently, following the direction of insertion while
applying pressure on the skin with your non-dominant hand.
19. With a passive safety syringe, the needle remains inside the syringe
barrel once the medication has been administered. In the event of bleeding,
use dry sterile gauze to apply pressure to the spot until the bleeding ceases.
Hemorrhaging is infrequently observed following subcutaneous injection.
20. After using, immediately put the syringe and needle into the sharp
container. Avoid recap.
21. Remove and discard gloves and do hand hygiene.
22. Document the procedure in the patient's medical record.
