1. Preface
1.1 Validity of the service manual
This service manual is valid for all CURIS® units with all hard and software versions covered by num-
bers “0000“ or “0100” on the identification label . The identification label is on the back of the unit.

1.2 Important information


1.2.1 General
The unit CURIS® fulfills the basic requirements according to Annex I of Guideline 93/42/EEC.

The unit has been classified as Class IIb and has the CE mark .
1.2.2 Warranty
There is an 18-month warranty for the unit starting on the day of delivery to the end user. For details
about the warranty see Sutter Medizintechnik’s General Terms and Conditions.

Please note
The manufacturer can only be held legally liable for the safety and functions of the unit if main-
tenance, servicing and changes to the unit have been performed by the manufacturer himself or
a person acting according to the manufacturer’s instructions.
If the servicing is performed by a person authorized by the manufacturer or by the manufacturer, the
user needs to obtain a receipt stating the kind and extent of the repairs. The receipt has to indicate the
date of the repairs as well as the company name and signature of the service person. If servicing is not
done by the manufacturer of the unit, repaired units and parts must also bear the service person’s iden-
tification.

2. Transport and Packaging


2.1 Incoming inspection
2.1.1 Transport damage
Check the unit and accessories for transport damages and faults immediately after receiving the unit.
Each CURIS® unit comes with a power cable and user instructions.
2.1.2 Compensation for damages
Damages may only be claimed if the distributor or forwarder is informed immediately upon detecting a
damage. Records of damages have to be made and be sent to the nearest Sutter representative or to
Sutter, who will pass the claims on to the insurance company.

2.2 Return shipments


To return a unit to Sutter or a Sutter service point use the original box to ship the unit if possible and in-
clude the following information:
Name and address of owner
Type and unit number
Description of error

3. Warning and Safety Signals


3.1 General
Radiofrequency units produce high voltage and currents to perform according to their intended use. To
prevent danger for the patients, users and others, always use the unit properly and strictly adhere to the
user instructions and safety guidelines.
We recommend that the user keep a technical user file (in accordance with the German Medical Device
Operation Regulations (Medizinbetreiberverordnung).
Improper functioning of an RF unit may lead to an undesired increase of power output. As a precaution
the RF generator CURIS® comes with an integrated protective circuit to avoid undesired, excessive
power output.

3.2 Pace makers, implants


Take into consideration before applying an active electrode or patient plate that RF current must
not be channelled via metal implants, e. g. never attach patient plates above endoprotheses or
metal implants.
Patients with active implants, e. g. pacemakers or implanted electrodes are at risk when treated
with an RF unit. The RF unit may impair the function of active implants or cause irreparable
damage to them. Due to this risk factor avoid the use of radiofrequency on such patients.

3.3 Interference with other units


The intended use of RF surgery may lead to interference of the CURIS® with other
electromedical equipment (e. g. ECG monitors) and electronic devices (e. g. telephone,
computer).

To reduce interference, take the following measures:


• Connect the RF generator CURIS® to another power outlet.
• Increase the physical distance between the CURIS® and the malfunctioning device.
• Make sure that the instrument cable is not near other devices or their cables.
• Make sure that the potential equalizer (41) is properly earthed.
• Make sure that connector cables are not near other devices.

4. Technical Description of the CURIS® radiofrequency unit


4.1 General
The radiofrequency unit CURIS® is an electrosurgical unit designed for general clinical use. It has the
following features:
• A maximum of 100 watts RF output for cutting, a maximum of 80 watts for contact coagulation,
60 watts for soft spray coagulation, and 100 watts for bipolar coagulation to ensure substantial
power reserves.
• Separate adjustment of RF output for cutting and coagulation.
• Separate display of RF output for cutting and coagulation.
• Easy to understand design and user-friendliness.
• Control with two micro-controllers. These run parallel to fulfil the safety functions and control
each other. In case of deviation of data, the energy output is cut.
• Adaptive NE control, for one-piece and split patient plates.
• Only one footswitch for monopolar and bipolar operation required. The footswitch adjusts to the
operation mode.
• Interface for irrigation-pump activation signals on back.
• Automatic bipolar forceps control may be activated if desired.
• Timer to limit the maximum activation for coagulation modes may be activated if desired.
• A casing that prevents dust and liquids from penetrating the unit.
• Multi-voltage power supply adjusts automatically to a mains voltage from 100 to 240 V, 50/60
Hz.

Advice
In the following chapers numbers in parenthesis, e. g. (9), indicate the position of features on
the front or back of the unit.

4.4.2 Self-test
After switching the unit on it will perform a self-test.
All display elements on the front panel of the unit light up. Whether these elements work properly can-
not be tested by the unit itself. Verify the correct function of these a follows:
After switching the unit on, the red signal above the patient plate socket will briefly light up.
Then all the push buttons will briefly light up. No dashes or dots may show on the displays for power
output and TIMER. At last all three of the displays will show “888”. The activation lights for monopolar
“cut” and “coag” light up successively and are accompanied by their respective activation signal tone.
If the self-test does not run as described, the unit is probably defective. Do not use it. Contact your ser-
vice partner.
After successful self-testing, the unit is ready for use in the program last used. If an error was found, the
unit will show an Error-number (see chapter 10).

4.4.3 Connecting patient plates


Connect the patient plate to its socket (29) at the front of the unit. Use one-piece
patient plates or split patient plates to facilitate patient monitoring applications.
The signal „Patient plate not connected“ (30) above the socket will come on if the
patient plate is not connected in monopolar working mode. The signal does not
come on in bipolar mode. When trying to activate the RF unit in monopolar mode
while the patient plate is not connected, a loud acoustic alarm signal will be audible
and it will not be possible to activate RF.
The bipolar mode is not affected.
When using a split plate, the signal light ”Patient plate not connected“ will only disappear after plate
has been properly secured. Take into account additional time requirements as running-in periods may
vary.

4.4.4 Connecting monopolar handpieces


For cutting and coagulation in monopolar mode plug in a handpiece with
fingerswitch or a handpiece without fingerswitch and a footswitch. Connect the
monopolar handpiece to the connector socket (27) at the front of the RF unit.
Connect the cable for the footswitch to the socket (36) at the back of the RF
unit.

Insert the active electrode into the handpiece until it is firmly locked.
For the safe use of accessories see also the user instructions of the instruments
employed.

Warning
Do not activate RF power while inserting an electrode or while changing electrodes. This may
cause burns!

4.4.5 Connection of bipolar accessories


For working in bipolar mode plug the instrument connector cable into its socket
(23). Connect the cable to the appropriate bipolar instrument. Use the foot-
switch, connected to socket (36) at the back of the RF unit, to activate the in-
strument. Alternatively, for bipolar coagulation electrodes activation is also
possible via the AUTO START (25) button. Bipolar forceps with handswitch may
be connected with a special cable.
The use of a patient plate is not required for bipolar applications.
Attaching a patient plate for bipolar applications will not enhance the safety and effectiveness of bipolar
applications.
For the safe use of accessories see also the user instructions of the instruments employed.
4.4.6 Bipolar accessories and automatic intstrument recognition
Some accessories connected to the RF generator CURIS are identified automatically via a special cod-
ing feature. Automatic instrument recognition is based on certain generator pre-settings that are
adapted to the accessories and cannot or can only be partially altered by the user. For automatically
identified instruments, access to and saving of programmed applications (P1 to P4) are only possible
within the parameters assigned to each instrument.

4.5 Choosing a mode of operation


Choose the desired mode of operation prior to using the unit.
CURIS® offers the following modes of operation:
MONOPOLAR Cutting CUT 1 (5) Smooth cut without eschar formation.
For monopolar e- Yellow-lined buttons
lectrodes CUT 2 (7) Cutting current with coagulation (hemosta-
sis) around the edges.
Coagulating CONTACT (4) Contact coagulation. Coagulation affecting
Blue-lined buttons the deeper layers of the tissue. Direct con-
tact of electrode and tissue.
SOFTSPRAY (6) Spray coagulation. Coagulation current not
affecting the deeper tissue layers for sur-
face coagulation by means of arcing (fulgu-
ration).

BIPOLAR Cutting BICUT 1 (10) Smooth cut without eschar formation.


For bipolar e- Yellow-lined buttons
lectrodes BICUT 2 (12) Cutting current with coagulation (hemosta-
sis) around the edges.
EXCISE (14) For use with e. g. ring forceps.

4.5.1 Working with the unit

Warning
Do not activate RF power while inserting an electrode into the handpiece or while changing
electrodes. This may cause burns!
To activate the unit, choose the RF power output according to the desired mode of operation, and use
the handswitch or the footswitch. RF current is administered according to pre-adjusted power settings.
The activation of the RF unit is accompanied by a continuous acoustic signal and a visual signal for
MONOPOLAR (8) and BIPOLAR (15) respectively.

4.5.2 Power adjustment


Adjust the power output for cutting and coagulation at the turning knobs on the front panel of the unit.

4.5.3 Special functions


The RF generator CURIS® has the following special functions:

TIMER (28)
This special function limits the RF output to a pre-selected maximum activation period. After this time
the RF unit will switch the probe off automatically. Adjust the duration with the turning knob (21). The
display (20) will show the remaining time. Time is only counted when RF current is activated and stops
when it is not activated. The activation period can be adjusted from 0.2 to 60 seconds. The TIMER func-
tion works in the coagulation modes only, both in the monopolar and bipolar modes, but not in the Ra-
VoR™ and soft spray mode.
Proceed as follows:
1. Push the TIMER button (28).
2. Use the turning knob (21) to adjust the maximum RF activation time.
3. Activate the electrode.
After the adjusted time has expired, the electrode will be deactivated automatically, even when the
footswitch or the button on the handpiece is pressed.

STANDARD (26)
This button serves to turn off all special functions. Activate or deactivate the electrode manually (via
handpiece or footswitch).

AUTO START (25)


This special function is only available in the bipolar MACRO and PRECISE coagulation modes. The RF
output will be automatically turned on at tissue contact. The function is turned on with a delay of 0.0 to
5 seconds, which can be adjusted with the turning knob (21).

Warning
The function AUTO START of the RF generator CURIS® may lead to undesired RF activa-
tion. Always make sure that the bipolar instrument used cannot come into unintended
contact with the tissue, and avoid contact with low-resistance, electrically conductive
parts.
Activation can also be caused by contact with metal pieces (e. g. trocars, retractors) or similar low-
resistance materials (e. g. liquids). Unintended RF activation may also result from tissue contact when
bipolar accessories are positioned on the patient, or during perioperative instrument cleaning.
The bipolar cutting modes are not available when AUTO START has been selected.

AUTO STOP (24)


This special function is only available in the MACRO, PRECISE und RaVoR™ coagulation modes and is
automatically activated in the RaVoR™ mode. Bipolar coagulation will desiccate the tissue during the
main coagulation phase. Increased desiccation leads to increased tissue impedance. The RF generator
CURIS® has automatic tissue impedance control and will turn off automatically before tissue is ne-
crotized.
Activate the electrode with a handpiece or footswitch.

AUDIO FEEDBACK (22)


This special function is only available in the BIPOLAR coagulation modes. During cutting and coagula-
tion the pitch of a simultaneous acoustic signal indicates how high tissue impedance is. The higher the
resistance the higher the degree of tissue desiccation and the pitch of the acoustic signal.

4.5.4 Programming memory


The RF generator CURIS® has four program storage buttons P1 to P4 (3), which may be used to save
current settings.
Proceed as follows:
1. Choose the desired settings of the RF unit (mode, output, special function).
2. Press one of the buttons P1 to P4 (3) until you hear the confirmation signal.
The current settings have now been saved and the green lamp next to the programming button comes
on. The unit is ready to work with these settings. All four of the programming spaces may be used in this
fashion.
To retrieve the settings that have been stored, press the respective programming button. The green
light next to the button shows that the stored settings are active. If the special function AUTO START is
part of a setting, the AUTO START button (25) will flash and cannot be activated separately. This func-
tion can only be used after pressing the AUTO START button (25) again for confirmation. This is a pre-
caution to avoid unintended burns.

Advice
For reasons of safety, settings in the bipolar cutting mode cannot be saved. If attempted, the
programming button will save “0. When choosing the bipolar cutting mode, readjust the settings
with turning knob (17).

The green light will disappear as soon as the settings are changed. By pressing a programming button
until the confirmation signal sounds, the new settings will be saved and overwrite the previous settings.
When using bipolar accessories with automatic instrument recognition, the programming functions of
the unit are limited (see 3.5.4). The settings cannot be overwritten if the codings for the predefined
identification of bipolar accessories are changed (if possible at all).
When switching the RF unit on for use, the most recently used program settings will be retrieved.

4.5.5 Performance check


Before using the RF unit, perform the following function test to make sure that all functions of the unit
work properly:
1. Pull the plug for the patient plate cable out of its socket (29). The red signal light (30) flashes. When
trying to activate monopolar current, an intermittent acoustic warning signal can be heard instead of
the continuous activation sound, and the RF output is blocked. Bipolar current may still be activated
if selected, and the red light will not flash.
2. Push the plug for the patient plate cable back into its socket (29). In monopolar mode the red light
will stop flashing. Split-patient plates have to be attached properly to the patient before the alarm
will stop. The light will not flash in bipolar mode.
3. Connect the cable for the monopolar handpiece to its socket (27). Activate RF power by pressing the
fingerswitch on the handpiece or the footswitch. The lights for MONOPOLAR (8) and BIPOLAR (15)
have to flash when selected, and the RF activation signal must be audible.

Warning
If the activation signal can be heard while the footswitch or handpiece is not connected, the
RF unit is not working properly. Do not use the unit in this case. Technical service is required.
If the RF activation signal can be heard while the footswitch or the handpiece are attached
without being used, one of the accessories is not working properly. Do not reuse defective ac-
cessories. Replace defective accessories.

4.6 Mechanical Composition


The lower part of the casing contains the RF generator module, a low-heat network socket, a connecting
socket for the footswitch and the irrigation pump, a power supply unit for low-voltage supply, the loud-
speaker and front part.
The front part is mounted on the lower part of the casing with two pivots and secured with four screws.
The lid of the casing is pout on the lower part from the back and locked at the bottom with four screws.
For service and demonstration purposes the unit maybe dismantled into the following components.

4.7.2 Voltage supply


A separate power supply unit provides the working voltages needed (+ 5 V/+ 12 V/- 12 V) for the elec-
tronic cards. The voltages are fed to the main board via cables.
More voltages (+ 5 V/8V/10 V/- 5 V/-8V/-10 V) are produced on the main board, including the redun-
dant voltage supply (+ 5 V B) for the watchdog controller, which is located on the right sight at the rear
of the main board.
Voltage supply RF generator
The RF generator is fed by a variable voltage ranging from 0 V to 60 V that is dependent on the initial
power output. The voltage is produced by a primary synchronized switching supply unit in voltage-fed
half-bridge circuits located on the right sight at the rear of the main board.
The switching power supply is addressed by a switching power supply controller on the secondary side.
The switch transistors on the primary side are addressed by impulsed transformers with a working fre-
quency of about 100 kHz.
In case of an incident the mains adaptor is protected from damage by current reduction, e. g. through a
short circuit in the RF generator.
The unit’s safety devices also serve as a primary-side protection for the switching power supply. The
value of these safety features is T3,15 A/250 V for the entire system voltage from 100 V to 240 V.

Warning
Danger of electric shock and electocution!
The main card contains live components. These are marked with the high voltage symbol.
Warning – live components, which are directly connected to the mains supply:
• All components of the power supply unit belonging to the primary side to supply power to the RF gen-
erator.
• The part of the main board on which the components are mounted.
This part is located on the left side at the rear of the board and is specially marked by a separator line
and high voltage arrows.
It is still possible to touch live parts on the surface of the board, even though constructive measures
have been taken to avoid contact with live components.
After switching off the power supply, the storage condensators discharge after the rectifier and only
gradually so that dangerous voltages flow after disconnection from the network for some time.

4.7.3 Main board


RF generator
The actual RF generator is located on the main board on the left at the front. It has been onfigured as a
push-pull amplifier and works at a fixed frequency of 4 MHz. The output stage is activated by a pulsed
sample generator, which produces the different signals for the chosen kind of current in dependence on
the current mode. The 4.8 mm flat-cable plugs and driver transistors serve for cooling purposes and as

measurement points. They may be used to verify different auxiliary supplies as well as the control
pulses for the output stage.
There is an adapter network between the output stage and the output transformer, the branches of
which are operated via relays in accordance with the current mode chosen. RF power is uncoupled
through separate output transmitters for the monopolar and bipolar RF modes and guided to the re-
spective connector socket via the RF output board.
The output transmitters are mounted at the front of the main board and have plugs at the the winding
ends coming out of the transmitter. The plugs are connected with the RF output board.
Controller system
The controller system is on the right side at the front of the main board. Relays K1 and K6 are on the
outer left side of the main board.
The controller system is first-error proof and consists of two independent controllers that communicated
with each other via an SPI bus. Monitoring relevant to the safety of the unit is performed by both con-
trollers independently at the same time. If there are no errors or disruptions, the RF output will be
switched off in the usual fashion.
Deviation during comparison of the recorded measurement data will shut down the RF output. Each of
the two controllers can open safety relays K1 and K6 respectively. In case of error relays K1 or K6 will
additionally cut off the generator’s voltage supply.
During regular use both relays will be closed, and the relays’ red LED displays will be illuminated.
The controller system communicates with the front card via the IIC bus.
For in-house company purposes the JTAG and ISP interfaces have been realized.
An update of the operation systems may be realized through the RS 232 interfaces (see chapter 7 on
software updates). The programs will also contain the performance charts for the respective RF cur-
rents.

4.7.4 RF output card


The output card is divided into an area for monopolar functions on the left side and an area for bipolar
functions on the right side.
Each of these areas is subdivided into a galvancially separated patient circle (at the front of the board)
and an intermediary circle (at the back of the board).
Separation of each of these areas is effected by four transmitters and five fiber-optic light guides. The
rear part of the RF output card has the same mass reference value as the main card.
Patient plate control
Both one-piece and split NE may be connected to the CURIS® and will be recognized automatically by
the unit. On the side of the NE, the current path is split via the two capacitators C31 and C32 with both
lines leading up to the NE. Impedance between the two lines is controlled by a monitor. The monitor
consists of a free running oscillator, the amplitude of which is attenuated by the NE’s parallel resistance
unit.
For split NE impedance monitoring can only be effected when it is applied to the patient’s skin as the
patient is the link that closes the monitoring circle. When falling below a predefined threshold of about
270 ohms the alarm will stop. Usually, however, impedance is much lower and the minimum value is
reached only after a few minutes. The monitor stores the minimum value and uses it as an alarm-
release reference. As soon as impedance climbs by a predefined value above the minimum value, a dy-
namic NE alarm will occur and the generation of monopolar RF current is not possible or will be dis-
rupted.
Measurement voltage may generate an oscillogram through transmitter T4. In case of oscillator failure
the LED display for the NE will flash at half frequency (1 Hz) and monopolar RF will be blocked.
Monopolar handpiece control
RF current may be activated either by the footswitch or a handpiece with activation buttons or by
equally by both.
For activation of high frequency via a fingerswitch on the handpiece an interface switch will channel the
control signals via two light ducts to the controller system. Auxiliary energy is transferred to transmitter
T2 where it may generate an oscilogram.
Bipolar handpiece control
Switching is identical with that for monopolar handpiece control. However, auxiliary energy is channelled
through transmitter T3 and combined with the bipolar instrument encoding function.
Bipolar instrument encoding
The bipolar instrument socket also offers automatic instrument recognition through resistance encod-
ing. When connecting an instrument with encoded resistance the unit will automatically select a pro-
gram that is best suited for the use of the instrument. By this function certain current modes and power
ranges may be blocked.
Auxiliary energy is channelled through transmittor T3. A change in frequency will automatically release
calibration. There are no trimmers for this switch component. Via light duct IC the signal measured is
transmitted as a frequency modulated value to the main board.

Bipolar impendance monitoring


In parallel function to the bipolar outlet, impedance control will deliver an analog output signal IMP. The
signal designates the tissue-resistance value measured between two lines and serves to control auto-
matic activation and deactivation of bipolar coagulation current. When activating an automatic function
or audio-feeback, the monitor will hydrodyne a small voltage of medium frequency via transmitter T1
and thus produce a current, which is a criterion for tissue resistance.
Automatic activation is caused by fixed resistance values related to the individual current modes. Auto-
matic deactivation can be caused by fixed resistance parameters and by dynamic activation, i. e. when
the lowest value is stored and activation is terminated by a certain increase.
When using the unit in AUTO START mode, it is not possible to use bipolar cutting current. The CURIS®
cannot distinguish whether the lowering of resistance is due to tissue contact with a coagulation or cut-
ting instrument. For this reason the bipolar cutting modes and the bipolar AUTO START mode are mutu-
ally exclusive and secured against each other.
4.7.5 Front card
The front card stores the keys for selecting a current mode and the function, activation LED displays
and NE alarm LED as well as the seven-segment displays.
They are selected and red via IIC bus expander components.
When the unit is switched on, the keys are illuminated dimly and may be easily recognized, even when
the OR environment is dark.

5.2 Closing and opening the casing of the unit


The upper and lower parts of the casing are locked with four screws.
Open the casing as follows:
• Disconnect/Pull the power plug.
• Turn unit upside down. Unscrew and remove the four metal screws at the bottom of the unit.
• Pull back the upper part of the casing and remove. Store safely.
Close the casing as follows:
• Push the upper part from the back onto the lower part of the casing.
• Turn the unit upside down.
• Slide the lower part of the casing entirely into the upper part and align the fixing holes.
• Bolt together the upper and lower part with four contact discs and screws.
• Return the unit to its original position.

5.3 Removing and installing the rotary encoder


Open the unit as described in 5.2.
Remove the rotary encoder as follows:
• Pull (5) turning knob.
– Manually in axial direction.

• Remove the corresponding cable connector piece from the main card.
• Unscrew the nut of the rotary know with an 11 mm socket key and remove.
– Pull the rotary know from the front part to the inside.
• Insert the new rotary encoder from the inside into the front part and tighten the nut.
– When tightening the nut lock the rotary knob with other hand.
• Mount turning know and turn. It has to turn easily.

– If it does not move easily, the rotary encoder is probably not centered well. Unscrew and tighten
nut again to center the rotary encoder more precisely.
• Attach cable connector piece to the main card at the corresponding cable connector X2, X3 or X4.
Close the casing as described in section 5.2.
Perform a performance test as described in section 6.3.

5.4 Removing and installing the front part


Open unit as described in section 5.2.
The front part has been secured to the lower part of the casing with four screws.
Remove as follows:

• Unlock and remove the four fixing screws.

5.5.2 Removing and installing connector sockets


The connector sockets are mounted and screwed to the front part from the inside. The individual sock-

Danger of electric shock and electocution!
The main card contains live components. The capacitators of the switching power supply are
electrically charged and will be under high voltage for about 20 seconds after the mains sup-
ply has been switched off.
The main card has been mounted on spacer bolts below the RF output card.
The main board is secured with five hexagon spacer bolts and three hexagon nuts M4.
It may be removed as follows:
• Remove RF output card as described in section 5.6.
• Unplug all cable connectors from the main card.
• Unplug light conducting cables.
– Loosen coupling rings from the opto-elements.
– Pull light conducting cable from its sockets.

• Push mains connector socket to the inside and remove.
Install the mains connector socket as follows:
• Slide the mains connector socket onto studs from the inside and secure with nuts.
• Plug in all cable connectors for the mains connector socket.
• Remove fuse holders:
– Mount 2 fuses T 3,15 A/250 V.
• Install fuse holder.

Open the unit as described in section 5.2.
The loudspeaker is secured with 4 plastic hexagon spacer bolts on the back of the lower part of the cas-

• Unplug connector cable from plug X25 “SPEAKER“ on the main card.
• Remove loudspeaker
– Loosen the 4 hexagon spacer bolts and remove.
– Pull loudspeaker from its stay bolts to the inside.
– Remove the connector cables by soldering.
– Remove the loudspeakers.
Install the loudspeaker as follows:
– Attach the connector cables by soldering.
– Slide the loudspeaker onto its stay studs. Make sure that the 4 plastic insulation sleeves are on
the studs.
– Secure the loudspeaker with 4 hexagon spacer bolts.
• Plug in connector cable to plug X25 “SPEAKER“ on the main board.

casing of the unit.
Remove as follows:

• Unplug connector cable from plug X5 “SPEAKER“ on the main board.


• Remove potentiometer:
– Loosen the 2 nuts fM3 (arrow) and remove.
– Pull the potentiometer from the stay studs to the inside.
– Remove the connector cable by soldering.
– Unscrew the potentiometer with a 10mm socket wrench and remove from its support.

Install the potentiometer as follows:


– Put the potentiometer on its support and secure with a 10 mm socket wrench.
– Attach the connector cables by soldering.
– Slide the potentiometer onto its stay studs.
– Secure with two nuts M3.
• Plug in connector cable to plug X25 “SPEAKER“ on the main board.

5.10 Removing and installing the connector sockets “Footswitch“ and “Pump“
Open the unit as desribed in section 5.2.
The connector sockes “Footswitch“ and “Pump“ are mounted in the back of the lower part of the unit’s
casing from the outside and secured with screws on the inside. Remove as follows:

• Unplug connector cable from the main card:


– Plug X8 “FOOT“ for the footswitch.
– Plug X22 ”PUMP“ for the pump.
• Remove connector socket:
– Loosen nut with 20 mm spanner wrench and remove.
– Pull the connector socket off to the outside.
– Pull the connector cable through the opening in the casing.
Install the connector socket as follows:
– Guide the connector cable inside through the opening in the casing.
– Insert the connector socket from the outside into the opening of the casing.
– Align the connector socket with the slot downwards to prevent wiring the wrong way around.
– Secure with nut.
• Attach connector cable on the main card:
– Plug X8 “FOOT“ for the footswitch.
– Plug X22 ”PUMP“ for the pump.

Remove the small voltage supply unit as follows:

• Unplug connector cables from small voltage unit:

– Small voltage connector J2
• Remove small volltage supply unit:
– Loosen 3 plastic hexagon spacer bolts and remove.
– Loosen 1 nut and remove.
– Take small voltage supply unit from the spacer bolts by moving to the inside. Store securely.

6. Adjustments and Checks After Change of Modules


The test steps have been mentioned in the test protocol. They have to be ticked off after they have been
performed, and the protocol needs to be signed by the tester.

6.1 Checks and tests


6.1.1 Visual control
Front:
• Verify correct installment of sockets (see photo)
• Verify correct fit of front foil and pressure points of keys.
• Ensure that rotary knob moves easily and does not get stuck.
• Verify that paintwork of the front part is not damaged.
• Make sure that ratched of NE engages properly.
• When using a Bovie connector, make sure it locks properly.

Inside the unit:


• Verify that plastic bolts are secured firmly.
• Verify that all cable connections are attached firmly.
• Verify that nuts for securing main board are secured firmly.

6.3.2 Self-testing
Provided that the unit is switched off, the self-test works as follows:
Switch the unit on and watch the process as the displays will be illuminated.
• Switch unit on

The displays are illuminated and obey the following procedure:
1. The red display above the NE display will be briefly illuminated.
2. The keys are briefly illuminated:
– Watch and check if all LED are illuminated.
– No segment (line, dot) may be illuminated on the power and timer displays.
3. All segments of the seven-segment displays are illuminated and show “888“ simultaneously.
4. While the seven-segment displays are illuminated, the following happens at the same time:
• The activation signals are illuminated successively for:
– Monopolar cut and coag as well as

• The corresponding activation signal (four-tone sequence) and confirmation signal can be heard

5. After successful performance of the self-test the unit has the following status:

– If no NE connector cable is attached, the red LED light will flash, but only in monopolar mode.
6.3.3 Performance testing of user elements
1. Turn all knobs in both directions.
• The numbers indicated have to:
– Increase when turning clockwise.
– Decrease when turning counter-clockwise.
2. Connect monopolar handpiece and verify the proper functioning of the fingerswitch.
– Make sure Cut and Coag are assigned correctly.
3. Test bipolar fingerswitch signals with the fingerswitch for a bipolar instrument or with the CURIS®
testing adaptor.
4. Connect two-pedalled footswitch with mushroom button in the middle or corresponding testing
adaptor to the footswitch socket at the back of the unit. Verify the proper functioning of the activa-
tion paths by testing the buttons.
– Make sure that the alignment is correct: Yellow pedal = cutting; blue pedal = coagulation.
6.3.4 Performance testing of special function keys
Push the “Timer“ key and adjust 3.0 seconds. Activate “Bipolar Macro-Coag”. After 3 seconds the timer
will stop activation.
Push the “Audio Feedback” key and activated “Bipolar Macro-Coag”. The activation signal will be on a
higher frequency.
Push the “Auto Start” button. To check the performance use a test lamp or a measuring kit (adjust at 50
ohms).

6.4 Basic settings


6.4.1 Deleting user programs

Advice
Only delete user programs after the user of the RF unit agrees.
Delete user programs as follows:
• Push the keys for program 1 and program 3.
• Switch unit on

– The erasing procedure is indicated by “Pro DEL“.

6.5 Adjustments after card change


The following adjustments have to be made after the main card or the RF output card has been ex-
changed:
6.5.1 RF output card
The measuring points and balance points for the adjustments described below can be found on RF out-
put card C 40-2171.
Basic setting for NE monitoring
Adjust NE monitoring as follows:
• Connect the voltometer on the RF output card to:
– Measuring point X14 (NEI) and
– Measuring point X10 (GND).
• Insert 270 test resistor in NE socket.
• Use the trimmer R75 to adjust a value of 1.70 V on the RF output card
– The red NE warning LED has to flash (threshhold: 1.67 V).
• Insert 220 test resistor in NE socket.
– The red NE warning LED does light up.

7. Updating the Operating Program


It is possible to update the operating program (software) for the CURIS® without having to exchange
storage components. The contents of the storage components will be overwritten from the outside. For
this purpose the main card has serial interfaces, which are accessible after removing the lid of the cas-
ing. The connector sockets are at the front on the right side of the main card.

The main card contains live components. These are marked with the high voltage symbol.

7.1 Requirements for program updates


• A serial connector cable RS232 with 9-pin SUB-D connector plug connected 1:1.
– Connect the cable to the serial interface socket of your PC. The connectors are marked with Se-
rial, RS232 or a symbol „I0I0“.
• A PC/notebook computer and operating system Microsoft Windows with an unused serial interface
which is available for the operating system, e.g. COM1.
• The program „Megaload“,installed on the PC/notebook computer.
• The current CURIS® operating program as hex-files for the CURIS®’ main controller (HC) and the
watchdog controller (WDC)

7.1.1 Possible problems


The serial port is blocked.
The serial port may be blocked by another program.
This is particularly apt to happen with synchronization programs for mobile phones and PDAs, which are
uploaded when the processor boots and will block the interface. Most of these programs can be deacti-
vated via the task bar so that the interface port will be available again.
Availability of a serial port may be tested with the Windows accessories terminal program Hyperterm.

7.1.2 Alternative use of an USB adaptor for computers without serial port availability
If there is no serial interface, a USB-RS232 adaptor may be substituted. It has a serial port and will be
recognized by Windows and used as COMx interface.
We recommend the use of USB adaptors that have been tested by the manufacturer:
UC-232A by ATEN (Manhattan REF. no 205 146)

Detecting a proper serial port


Try out the options available to detect the proper COM port. The process may be complicated by other
programs using the serial interface such as mouse drivers and other programs that have not been
closed down. Often synchronization programs for mobile phones, PDAs, etc will interfere. They will be
launched while the processor boots, and occupy the ports available. Most of the time they can be deac-
tivated via the task bar, and the ports will be available for use again. The same applies to internal mo-
dems or IrDA (switch often hidden in the PC’s setup). Usually the program will run with any serial port
that can be opened with the terminal program Hyperterm (Windows accessories). Please note: Unless
you close down Hyperterm after trying to find a port and prior to launching the flash operating software,
Hypoterm itself may block the serial port.
Other information regarding the use of an USB adaptor
Communication with the converter must be guaranteed during the entire programming process, and
must not be interrupted. This is best achieved when all user programs are closed down, including those
which use the processor intermittently such as Messenger and firewall programs, and the network is
disconnected.
If problems persist, the RS232/USB adaptor may be exchanged. The adaptor recommended by the
manufacturer has been found to work reliably.

7.2 Software update CURIS® BM1700 with PC/Notebook computer


To install a software update remove the lid of the unit. Looking at the main board from the front there
are two 9-pin SUBD connectors on the right edge. Connector “x9 RS232 HC” is for a software update of
the main controller. Connector “x10 RS232 WDC” is for a software update of the watchdog controller.
Always include both controllers when installing an update. Program the watchdog controller (WDC) first,
and the main controller (HC) next.
7.2.1 Install a software update as follows:
• Step 1: Remove the lid of the unit. Plug in cable RS232 to connector x10 (WDC) and connect with the
serial port (Com1, 2 etc) of the PC/notebook computer.
• Step 2: Launch the program Megaload on the PC/notebook computer.
• Step 3: List the file path of the current software version under “File to be program in the Flash“

8. Recurring Safety Checks


The RF unit CURIS® has to be serviced at least every 24 months by independent inspectors who are
entitled to perform such safety checks properly on account of their training, knowledge, and practical
experience.
1. Make sure that the identification label and inscriptions are legible. The information on the identifi-
cation label and the inscriptions on the back of the unit, in particular the nominal value for the
safety plugs of the mains cable connector have to be clear and easy to read.
2. The user instructions have to be on hand and have to be valid for the specific unit. Contact SUTTER
for revisions of user instructions.
Read software version on the displays and make sure that it is correct and up-to-date. Press the keys
monopolar Cut 1 and bipolar Cut 3 (Excise) at the same time while switching the unit on. The software
versions of the main controller (upper display) and of the watchdog controller (lower display) have to be
identical. Write down the version number in the test protocol and switch unit off.

8.1 Self test


8.1.1 Watching the self-test
Provided that the unit is switched off, the self-test works as follows:
Switch the unit on and watch the process as the displays will be illuminated.
• Switch the unit on
– Mains switch on “ON“
The displays are illuminated and obey the following procedures:
1. The red display above the NE willl be briefly illuminated.
2. The keys are briefly illuminated:
– Watch and check if all LED are illuminated.
– No segment (line, dot) may be illuminated on the power and timer displays.
3. All segements in the seven-segment displays are illuminated and show “888” simultaneously.
4. While the seven-segment displays are illuminated, the following happens at the same time:
• The activation signals are illuminated successively for
– Monopolar cut and coag as well as
– Bipolar cut and coag
• The corresponding activation signal (four-tone sequence) and confirmation signal can be heard
for:
– Monopolar cut and coag as well as
– Bipolar cut and coag
5. After successful performance of the self-test the unit has the following status:
– The last program used is active.
– If no NE connector cable is attached, the red LED light will flash, but only in monopolar mode.

8.2 Visual and performance checks


8.2.1 Performing checks
1. Check the performance of the user elements on the front: Test pressure points and switch functions
of all keys on the front. Make sure that the rotary knob moves easily.
2. Check all of the unit’s connectors visually. Verify that the RF sockets have no visible damages. Verify
that the sockets for footswitch, potential equalizer and pump connector on the back are not dam-
aged or loose. Plug out the mains cable from the connector socket and check. Verify that socket is
not damaged or loose.
3. Verify the cables of all accessories used with the unit and, if possible, make sure that the insulation
and connector plug are damaged. Check if original accessories have been used. If not, write down
the brand and manufacturer of accessories used.
4. Check the performance of the rotary encoder. The power and time display has to increase when
turning in clockwise direction and decrease when turning counter-clockwise.
5. Check the performance during activation, of the activation lamps and signals. Deactivate NE alert
with the short-circuit connector. Connect handpiece for electrodes to the monopolar instrument
socket and press fingerswitch. If the unit functions properly, RF current will flow, the monopolar light
displays will be illuminated in yellow or blue, depending on the active mode, and the activation sig-
nal will be audible.
6. Check the performance of the two-pedal footswitch. Connect the two-pedal footswitch with its cable
to the footswitch socket on the back of the unit. Verify the activation paths by activating the pedals
and bipolar RF. The bipolar light displays have to be illuminated in yellow or blue, depending on the
active mode, and the activation signal has to be audible.
7. Check the performance for NE control. When the NE connector socket is open and monopolar mode
has been chosen, the red NE light will flash. If the unit functions properly, no monopolar RF current
may be activated, and a loud, intermittent alarm signal will sound.
8. Check the performance of special functions.
Press the “Timer” button and set on 3.0 seconds. Activate the macro-coag mode. The timer mode
must stop activation after 3 seconds.
Press the “Audio feedback” button and activate the bipolar macro-coag mode. The activation signal
must be on a higher pitch.
Press the “AutoStart” button. Check the performance for automatic activation with a test lamp or
measuring kit (at 50 )

8.2.2 Performance check for automatic instrument encoding


Random-check two of the instrument encodings stored in the unit’s software.
Warning: Do not activate RF current as leakage currents may cause burns.
Plug in customary resistor with 270 into the two 2 mm auxiliary contacts of the bipolar instrument
socket. Instrument code “IC 03” must be shown on the display and a confirmation signal must sound. If
the CURIS® has information stored for the incoding, the display will show “en” (for “enabled). If no in-
formation has been stored, the display will show “dis” (for “disabled”). When the resistor is plugged out,
the display will briefly give the message “IC off”.
Repeat checks with 4.700 . The instrument code “IC 29” must be shown on the display. Otherwise
see above.
There is also a special measuring gauge available to test instrument encodings.

B. Measuring the output data


Checking the monopolar RF output performance
Connect the RF measuring device with a handpiece to socket M1 and to the NE socket. Adjust the
measuring device to 400 and select current mode Cut 1 (pure) of the unit to be tested.
Adjust 100 W and activate the current. Read off the value from the measuring device and write it down
in the test protocol. Observe the applicable tolerances.
Select current mode Cut 2 (blend) of the unit to be tested, adjust 100 W and activate the current. Read
off the value from the measuring device and write it down in the test protocol. Observe the applicable
tolerances.
Select current mode KK (contact coagulation) of the unit to be tested and adjust 80 W. Set the measur-
ing device at 200 and activate the current. Read off the value from the measuring device and write it
down in the test protocol. Observe the applicable tolerances.
Select current mode SK (soft spray coag) of the unit to be tested and adjust 60 W. Set the measuring
device at 500 and activate the current. Read off the value from the measuring device and write it
down in the test protocol. Observe the applicable tolerances.

Checking the bipolar RF output performance


Connect the RF measuring device to the CURIS® instrument connector cable and the bipolar socket
and. Adjust the measuring device to 200 and select current mode BiCut 1 (pure) of the unit to be
tested.
Adjust 80 W and activate the current. Read off the value from the measuring device and write it down in
the test protocol. Observe the applicable tolerances.
Select current mode BiCut 2 (blend) of the unit to be tested, adjust 80 W and activate the current. Read
off the value from the measuring device and write it down in the test protocol. Observe the applicable
tolerances.

device at 100 and activate the current. Read off the value from the measuring device and write it
down in the test protocol. Observe the applicable tolerances. Follow the same procedure for 5 W.

C. Safety measuring checks according to EN60601-1


These measurements are often performed in an automated safety window. The mains voltage will be
turned off and on several times during the automatic testing procedure.
For recurrent technical safety checks the following measurements have to be performed according to
EN60601-1:
1. Measure the resistance of the grounding conductor between the ground contact of the mains plug
and the PE bolt at maximum. According to EN60601-1 section 18 this value may not exceed 0.2 Ω
with a measuring current at a value of 25 A.
2. Grounded leakage current – normal case. Measure the current in the grounding conductor of the
mains cable without error simulation. The value may not exceed by 1.5 times the first value meas-
urement indicated on the technical information sheet that comes with the unit. It also must match
the absolute higher threshold value of 500 µA according to EN60601-1 section 19.3.
3. Grounded leakage current – first error. Measure the current in the grounding conductor of the
mains cable while simulating a disruption of the near-ground phase (which leads to a switch-off of
the unit). The value may not exceed by 1.5 times the first-value measurement indicated on the
technical information sheet that comes with the unit. It also must match the absolute higher
threshold value of 500 µA according to EN60601-1 section 19.3. Usually the value is double that of
the grounded leakage current.
4. Leakage current of casing – normal case. Measure the current that flows from the CURIS®’ connec-
tor to the ground. As the current path forms a parallel path to the grounding conductor, its value will
usually be low and undefined as it is dependent on the undefined ratio of the partial grounding re-
sistors. Its value may not exceed the absolute upper threshold value of 100 µA.
5. Leakage current of casing – first error, grounding conductor. Will usually correspond to the value of
the grounding current as the measuring path serves as grounding conductor. Maximum 1.5 times
the first-value measurement, and absolute maximum of 500 µA according to EN60601-1 section
19.3.

6. Leakage current of casing – first error, network. Measure the current that flows from the CURIS®’
connector from maximum to the ground. As the current path forms a parallel path to the grounding
conductor, its value will usually be low and undefined as it is dependent on the undefined ratio of
the partial grounding resistors. Its value may not exceed the absolute upper threshold value of 500
µA.
7. Patient leakage current – normal case. Measure the current of the instrument connectors monopo-
lar, bipolar and NE when not activated after grounding. The value may not exceed the absolute up-
per threshold of 10µA.
8. Patient leakage current – first error, grounding conductor. Same as described previous paragraph 7,
but with interrupted grounding conductor. According to EN60601-1 Abs. 19.3 the absolute upper
threshhold is 50µA.
9. Patient leakage current – first error, voltage on application part. Measure the current of the instru-
ment connectors monopolar, bipolar and NE when not activated after grounding. Grounded voltage
is provided with grounded voltage from the outside to the CURIS® connectors wobei durch das

9.2 Error Messages and their Meanings


The RF generator CURIS® performs a number of function tests during its self-test when the unit is
switched on, and constantly checks its own safety-relevant functions while it is switched on. If any dis-
turbances are detected, the RF output is stopped and an error message displayed.

When an error message occurs, the unit can only be reset by switching it off and on again.

10. Revisions
10.1 Procedure
Technical products are subject to continuous technical changes during their life due to continuous
product improvement. As there may exist several versions of one type of unit, service personnel has to
identify of the version of the unit before repairing it.
Prior to each service it is essential to determine the version of the unit. The version and status for the
CURIS® are part of the device number.

10.2 Hardware status


The hardware status is determined by:
• Revision status of circuit boards
• Revision status of mechanical composition
• Revision status of callibration
10.2.1 Identification of hardware status
The hardware status is shown on the labels which are attached to the circuit boards.
• Sticker 1 has the manufacturing ID number C 40-XXXX as well as a continuous testing number with
the tester’s identification or code number.
• Sticker 2 indicates the revision status. If a revision is available on the market, two sections with addi-
tional information will mention it.
A third repair sticker will be attached if a circuit board is repaired by the manufacturer. Such circuit
boards are updated to the latest revision status and used as exchange boards for repairs after thorough
testing.
A circuit board with a higher revision status may be used for a circuit board with a lower revision status
if it has the same Sutter reference number as the older original (see 5.1) (downward compatibility). This
does not apply vice versa!

11. Maintenance
11.1 Cleaning and disinfection
For cleaning and disinfection, disconnect the RF generator from its power source. Make sure that no
cleaning agents and disinfectants or sprays penetrate the interior of the unit.
Clean the surfaces on the outside and the front of the RF generator with commonly used non-alcoholic
cleaning agents.
Disinfect the surface of the unit and non-sterilisable accessories with disinfectants commonly used in
doctors’ surgeries and OR departments.
Prior to use ensure that there is no residue of any disinfectant on the unit.

Advice
Only accept accessories for RF generators if they are in perfect condition and work properly. Ac-
cessories that do not work well and are damaged or defective can pose a danger for the patient
and user, and interfere with the intended use of the RF generator. Discard unsuitable accesso-
ries!

12. Environmental Aspects


12.1 Packing
The vendor will accept the return of empty packages and possibly reuse them.
Alternatively dispose of packages with waste paper or regular garbage.

12.2 Disposal
The RF generator has largely been made from materials that do not use composites in order to ensure a
high degree of recyclability after its life cycle. Follow the regulations for electronic waste when disposing
of a unit or return it to the manufacturer/distributor for proper disposal.
