I, Robet Duke C. Oca, of legal age, Filipino citizen, and a resident

of Magsaysay St., Miputak, Dipolog City, under oath, hereby depose
and state that:
1. I am a holder of Andres Bonifacio College’s Identification Card;

2. On August 19, 2024, Monday, while I was walking on school
grounds, I failed to notice that the lock on my ID cord was loose
and I only noticed that my ID was gone when I got home;
3. Despite my efforts in searching for my lost ID, I failed to locate
the missing item up to the present. I now consider the item
irrecoverable.
4. I am executing this affidavit to attest the truth of all the
foregoing and in order to support my application to replace the
lost one.

2. In connection with the foregoing criminal case, the Parties were
able to communicate with each other and have come to an
amicable settlement regarding the subject matter of the same.
3. In view thereof, I am no longer interested in pursuing the
criminal case against the Defendant;
