1. Purpose
The purpose of this document is to establish the requirements for the performance of
preventive maintenance to ensure compliance with industry best practices, manufacturers’
recommendations and specifications, and regulatory requirements to maximize life
expectancy and operational safety of hydrovac power units and operating equipment.

2. Scope
This document applies to the employees of MBI Energy Services, subsidiaries, and affiliated
companies.

3. Preventive Maintenance Policy


Preventive maintenance activities shall be performed in compliance with industry best
practices, manufacturers’ recommendations and specifications, and regulatory requirements
to maximize life expectancy and operational safety of hydrovac power units and operating
equipment. Equipment with defective or unsafe conditions shall not be used.

4. Requirements
4.1. Inspecting
Drivers shall complete a pre-trip inspection prior to use on a highway to ensure
safe operation. Deficient findings shall be documented on a driver vehicle
inspection report (DVIR) (paper or electronic) and submitted to the Service
Center Department in accordance with the Commercial Motor Vehicle Program.
Equipment with deficient findings shall be placed out of service until verified by
service center personnel.
Drivers shall drain the cyclone filter and dispose the sludge properly.
Drivers shall clean the cyclone filter daily.
Drivers shall grease operating equipment daily.
Drivers shall complete a post-trip inspection following the completion of any trip
and before the truck is used again. Deficient findings shall be documented on a
DVIR (paper or electronic) and submitted to the Service Center Department in
accordance with the Commercial Motor Vehicle Program. Equipment with
deficient findings shall be placed out of service until verified by service center
personnel.
Authorized, qualified mechanics shall perform repairs noted on the pre-/post-trip
inspection and deficiencies noted from other inspections. Repairs shall be
completed in conformance with manufacturers’ specifications (i.e., parts,
procedures).
Defects shall be verified by authorized, qualified mechanic and a driver
• that they were corrected; or
• need not be corrected for safe operation.
Authorized, qualified mechanics shall complete a Routine Inspection Checklist on
a unit that has documented, identified deficiency(ies) or is brought into a service
center for any reason (e.g., flat tire, return from vendor, return to active service).
The mechanic shall document repairs made in the fleet information management
system (FIMS).
Authorized, qualified mechanics shall complete scheduled preventive
maintenance in accordance with manufacturers’ specifications and company
authorized schedules.

Preventive maintenance schedules are managed through the FIMS.


Preventive maintenance on the power unit shall be completed no later
than 36 engine hours from the schedule as found in the FIMS, unless
approved by the director of fleet maintenance upon request from the
operations manager during a critical event (e.g., customer request for
24-hour operations, weather-related travel restriction, excessive
distance from repair services). If approved, the director of fleet
maintenance shall communicate the terms of approval to the
operations manager and the dispatch office using the PM Bypass Form.
The PM Bypass Form does not circumvent regulatory requirements.
The operations manager shall ensure the conditions of approval are
followed. Upon completion of the critical event, the unit shall be driven
directly to a service center for performance of scheduled preventive
maintenance.
Preventive maintenance on operating equipment shall be completed on
an annual basis as found in the FIMS.
Authorized, qualified mechanics shall complete a periodic (i.e., annual) DOT
inspection in compliance with 396.17.
4.2. Testing and Diagnosing
Authorized, qualified mechanics or designees shall collect and submit lubricant
samples while performing scheduled preventive maintenance. Samples shall be
analyzed by an authorized vendor.
Service center managers shall be responsible for managing the results
of the analyses and discovered corrective actions.
• Normal Alert – No further action.
• Caution – Resample and submit.
• Action – Truck is brought in to service center for further investigation.
4.3. Calibrating
This equipment does not require calibration.
4.4. Certifying
Authorized, qualified mechanics or designees shall certify the accuracy and
completeness of annual DOT inspections by completing the inspection form;
affixing the inspection sticker on the driver’s side of the cab in front of
the door; and placing a hard copy of the inspection form in the
document holder.
Service center managers or designees shall submit a copy of the annual
inspection report to the MBI DOT Department.

Annual inspection reports shall be retained in the MBI shared


(\\mbwsfs03) (S:) drive while owned by MBI plus one year and NAV for
a period of no less than 14 months.

5. Training
5.1. There is no training required for this program document.

6. Key Responsibilities
6.1. Drivers
Complete and document pre-/post-trip inspections.
Perform daily preventive maintenance on operating equipment.
6.2. Mechanics
Perform routine and preventative maintenance, and annual inspections.
Perform repairs, inspections, and certification.
6.3. Service Center Managers/Designees
Manage service center documentation for completeness and accuracy and fulfill
recordkeeping requirements.
Ensure annual vehicle inspections are performed.
Ensure repairs are completed in conformance with manufacturers’ specifications
(i.e., parts, procedures).
6.4. Director of Fleet Maintenance
Manage the Preventive Maintenance Program.

7. Glossary
Term Description/Definition
critical event An activity involving a commercial motor vehicle in use other than on a
highway.
highway Any road, street or way, whether on public or private property open to
public travel.
open to public travel Is available, (except during scheduled periods, extreme weather, or
emergency conditions), passable by four-wheel standard passenger
cars, and open to the general public for use without restrictive gates,
prohibitive signs, or regulation other than restrictions based on size,
weight, class, or registration. Toll plazas of public toll roads are not
considered restrictive gates.
qualified mechanic A mechanic who holds a certified Annual Inspector Qualification Form.
