PERFROMANCE TASK FOR 1ST QUARTER

Task 1: Sentence Transformation Activity

Task 2: Fill Out a Student Information Form Accurately


Objective:
Students will practice filling out a basic student information form with attention to detail, ensuring all
fields are completed accurately.

Instructions:
1. Read the instructions carefully before filling out the form.
2. Fill in all required fields using accurate information.
3. Review your form to make sure there are no mistakes in spelling, capitalization, or formatting.

Guidelines for Accuracy:


 Ensure proper spelling, including capitalization of names and cities.
 Double-check dates (especially birth dates).
 Ensure contact numbers are entered correctly and have the right number of digits.
 Make sure to write full addresses with correct postal codes.
 All required fields must be filled in; do not leave blanks unless the form allows.

Task 3: Create a Short Story Using Compound, Blended, and Clipped Words

Instructions:
1. Objective: Write a short story that includes at least:
o 5 compound words
o 5 blended words
o 5 clipped words
2. Guidelines:
o The story should be at least 100-150 words long.
o The meaning of the compound, blended, and clipped words must be clear from the
context of the story.
o Highlight or underline each compound, blended, and clipped word in your story.

Example Story:
"Last Sunday, we decided to go camping at the campground near the lake. We packed our backpacks
with snacks and some essentials like a toothbrush and sunglasses. On the way, we listened to the brunch
plans on the radio. After driving for hours, we were happy to finally reach the motel by the lake.
The next morning, we hiked to the waterfall, and I brought my cam to take pictures. The view was
amazing! At night, we watched an infomercial on TV about the best hiking gear, which we needed since
our boots got wet. After the trip, we took the bus back home.
It was an unforgettable adventure, and I can’t wait for the next one!"
