1. Determine patient’s health condition and


ability to participate in the procedure.

2. Check when the client last voided or was last


catheterized.

3. Percuss the bladder to check for fullness or


distention.

B. PLANNING

1. Determine the most appropriate method of


catheterization and need for assistance.

2. Gather and prepare equipment and supplies.

C. IMPLEMENTATION

1. Identify the patient.

2. Provide privacy.

3. Explain procedure and why it is necessary.

4. Adjust height of the bed to a comfortable


working level.

5. Raise side rails opposite the working side.

6. Assist patient to a dorsal recumbent position


and drape.

7. Ensure for adequate lighting.

8. Place a waterproof pad under the patient’s


buttocks.

9. Wash hands and don gloves.

10. Perform perineal care.

11. Remove gloves and wash hands.

12. At the bedside, open the Catheterization Kit.


Use the wrapper of the Kit to establish a
sterile field. Prepare equipment needed
observing sterile technique.

13. Wash hands, done sterile gloves.

14. Provide sterile drape under patient’s


buttocks, and the perineal area exposing the
urinary meatus.

15. Stand on the patient’s right if you are right


handed or on the patient’s left if you are left
handed

16. If inserting a retention catheter, test the


catheter balloon by injecting 10 cc of sterile
water to the indwelling catheter inflation
hub, to inflate. Then aspirate and deflate the
balloon. Detach the water filled syringe and
set aside.

17. Attach the catheter to the urine bag (if


necessary)

18. Lubricate the insertion tip of the catheter


liberally. Place aside in a sterile container
ready for use.

19. FOR FEMALE

20. a. Use your non-dominant hand to separate


the labia majora with the thumb and the
index finger. With your dominant hand clean
each side of the labia majora with cotton
balls soaked in antiseptic solution. Use a new
swab for each stroke in a downward motion
from the pubic area to the rectum. Repeat
for the labia minora.

21. b. Expose urinary meatus clean each side


using a new swab for each downward stroke.

22. FOR MALE

23. a. Use your non-dominant hand to grasp the


penis just below the glans. If necessary
retract the foreskin. Hold the penis firmly
upright with slight tension With your
dominant hand clean the meatus with cotton
balls soaked in antiseptic solution wiping
from the center in a circular motion going
around the glans.
