STEPS FOR VALIDATION OF TEST QUESTIONNAIRE, SURVEY

QUESTIONNAIRE,
& LESSON EXEMPLARS

1. Polish all your TQ, SQ, and Lesson Exemplars based on the suggestions of the
Thesis Advisory Committee/Panel Members.
2. Submit your TQ, SQ, and Lesson Exemplars to your Thesis Adviser for
checking prior to validation.
3. Once approved by your adviser, write a letter to your prospective validators.
At least 3 validators are required. Validators should not be your adviser or
your panel member. As much as possible, your validators should have
extensive experience (at least 5 years) in the teaching of your chosen
competency and grade level preferably from the Department of Education.
They may not be Master’s degree holder.
4. Give or send your instruments to your validator in hard or soft copy
depending upon the convenience of your validators. Place all instruments and
tools on a folder. Arrange your tools and instruments as stated below:
a. Cover letter
b. Title of Study and Statement of the Problems
c. Validation Form for Test Questionnaire
d. Clean copy of Test Questionnaire with Table of Specifications
e. Validation Form for Survey Questionnaire on Level of Interest
f. Clean Copy of Survey Questionnaire for Level of Interest
g. Validation Form for Survey Questionnaire on Perceptions on the Proposed
Innovation
h. Clean Copy of Survey Questionnaire on Perceptions on the Proposed
Innovation
i. Validation Form for Lesson Exemplars (with the SIM or Material if
applicable)
j. Lesson Exemplars (with the SIM or Material if applicable)
5. Once the validators are done with the validation, polish your instruments and
lesson exemplars.
6. Return the polished instruments and lesson exemplars to the validators for
final checking.
7. Once approved, proceed to pilot testing.
STEPS FOR PILOT TESTING AND RELIABILITY TESTING OF TEST
QUESTIONNAIRE, SURVEY QUESTIONNAIRE, & LESSON EXEMPLARS

1. Once your TQ, SQ, and Lesson Exemplars/Innovation have been validated,
proceed to reproducing the questionnaires and prepare for pilot testing.
2. Write a letter of permission to the head of your chosen school asking
permission to allow pilot testing of your instruments for your study. This
should not be the same school where you will conduct your study. Specify the
grade level to whom you will administer the pilot testing.
3. Once approved, proceed to the grade level for pilot testing. There should be a
minimum of 30 participants for your pilot testing.
4. Start with an orientation with the students. Ensure that there will be no
cheating and that students answer the questions as honestly and religiously
as they can.
5. Administer your survey questionnaire to the students on the level of interest.
Ensure that they understand each of the items by guiding them. Do
translations if need be.
6. After the survey questionnaire on interest, administer the test questionnaire
to your students.
7. After the administration of the test questionnaire, do a pilot testing of your
proposed innovation. Teach the students using the proposed innovation
following ONE of your lesson exemplars. Only one lesson exemplar (or one
episode of your SIM/CAIM) will be pilot tested.
8. After pilot testing of the lesson exemplars, administer the survey
questionnaire on their perceptions of your proposed innovation. Guide them
in answering the questionnaire. Translations maybe done or one-on-one
survey for Elementary pupils may be done.
9. After pilot testing, tally all their responses to the different questionnaires on
MS Excel. X-axis will indicate the number of items, and y-axis will be the
participants (coded). For test questionnaire, record whether students’ answer
for each question is right (code it as 1) or wrong (code it as zero).
10.Have a separate sheet for the survey on level of interest. X-axis will indicate
the number of items, and y-axis will be the participants (coded). Record all
their responses whether 5, 4, 3, 2, 1 for each question/statement.
11.Have a separate sheet for the survey on their perceptions on the proposed
innovation. X-axis will indicate the number of items, and y-axis will be the
participants (coded). Record all their responses whether 5, 4, 3, 2, 1 for each
question/statement.
12.Prepare these data for reliability testing on January 6, 2024.
