PREFACE

The project has been prepared in fulfillment of the requirement of the DIPLOMA
COURSE IN Advance Diploma Industrial Safety conducted by Devi Mahalaxmi
College, Vikhroli, Mumbai.
For any field in this modern and progressive world book knowledge alone does not
satisfy the need of a person. In addition to this knowledge we should also undergo
practical training and see the real problem, which may face during our job. It well
said that an example is better than the advice. Kipping this thing in mind our college
has also attached one practical subject in our syllabus of final year.
From this project we learnt to tackle different types of problem in high rise building
in this project, we have covered all the information, which we collected from jio
world center project.
The objective of this project is to study, evaluate and present the existing safety
system and the process and associated utilities, the hazards association with them
and their meditative measures.

OBJECTIVE and METHODOLOGY

OBJECTIVE:
• To identify the potential hazards in construction industry.

• To evaluate the risk of construction hazards associated with various erection


processes in the industry.

• Conditions and work practices that lead to job accidents and industrial ill-
ness.

• Documentation verification for compliance and completeness and


comprehensive safety programs, audits or reviews.

• Legal compliance status

• Systematic evaluation of the safety and health situation/practices.

• Stated more positively, it was to be checking and to see that things are
in good shape through SWOT analysis of safety preparedness.

• Interacting with management, staff and interviewing working personnel.

• Looking at the safety elements of the Organization as defined in the audit cri-

ABOUT THE PROJECT
The project on Safety in Construction with specific reference at large and detail to
construction of residential Towers at BKC Jio World Center .

Hazards are associated with construction since long. Efforts have been made make
these processes inherently.

Safe and process routes are also selected accordingly but exposure to hazards like
height work (fall) cannot be eliminated completely it requires careful study
planning organizing, education and training of employees and people.

Accidental / Occupational injuries and property damage in construction requires


effective control based on sound knowledge of civil and mechanical engineering.
Safety in construction requires study of man in his individual environment, con-
side ring the complexity of the activities carried out at the same time construction
industry is considered to be safe as compared to many industries profession and
activities. But this is a relative aspect depending on criteria.

SCOPE
• To examine and evaluate the safety performance.

• To integrate Safety health and environment in day to day construction


work and to improve safety performance of the industry.

• To study safety organization and safety procedures of the industry.

• To study the underlying causes of the accidents and near miss incidents.

• To give suggestions and recommendations for conserving environment and


preventing occupational disease.

Role and Responsibility

Role and responsibility are defined to clearly individual role in safety system
implements. To implement Safety Manual at site, Site Head shall be fully responsible.

Chairman/Vice Chairman/Managing Director:


• Management shall be responsible to ensure all projects under able guidance of Project
Head run according to set SAFETY procedure, guidelines and commitment of
SAFETY Policy statement.
• Management shall be responsible for annual review of SAFETY performance, policy
objectives and update plans.
• Management shall be responsible to provide required resources for implementation of
SAFETY Plan, procedure at workplace.
• Management shall be responsible to deploy one independent person to ensure the
entire Project site on behalf of him. The person appointed on behalf of Management
shall be responsible to report all performance to Management annually.
Vice President (V.P.)
• V.P. shall be responsible for Quarterly review of SAFETY performance, policy
objectives and update plans.
• He shall be responsible to provide required resources for implementation of SAFETY
Plan, procedure at workplace.
• He shall be responsible to take strict action on issues found not cleared at Site Head
level.

Site Head - G.M./D.G.M./A.G.M. and Managers:


• Site Head shall be accountable for implementation of safety system and accident
prevention at sites under his control. He shall be responsible for Monthly review of
SAFETY performance of sites under his control.
• He shall be fully responsible to carry out all site activities by his down the line staffs
and contractor as per standard procedure, Legal requirements and by take safety
precautions to minimize workplace accidents and harm to health and environment.
He shall be responsible to stop activity, if found not as per applicable standard and
SAFETY Manual.
He shall be responsible to give his feedback on Monthly Safety report and to clear issues not
resolved at site by contractor. He reports safety performance of sites to V.P. Projects monthly.

Execution Staffs:

• Execution person shall be responsible for safety of all man, material and to carry out
work as per applicable standard and SAFETY Manual.
• He shall be responsible to carry out work from start to end by safety and quality.
• He shall be responsible to control unsafe acts, unsafe conditions and negligence in
work at work site to prevent accident, incident at site.
• He shall be responsible to stop activity, if found not as per applicable standard and
SAFETY Manual.
• He shall be responsible to carry out work only after receive work permit.

• He shall be responsible to ensure and check all workers and staffs PPE's as per work
requirement at workplace and his area of working.
• He shall be authorized to stop work in case of any unsafe act or unsafe condition
observed and not controlled prior to start of work at site.
• He shall be responsible to work safely and to wear and use all PPE's at site every time
for safe working.
• He shall be responsible to give his ideas, inputs and suggestions to implement safety
system and accident prevention at site.

Safety Executive:

• He shall be responsible to identify workplace hazards, housekeeping issue and to


inspect all area of working and labour camp for safe working.
• He shall be responsible to conduct all workers and staffs safety induction, trainings
and to keep records of all safety work.
• He shall be responsible to observe labour camp housekeeping and cleaning points and
for report the same to concern contractor for corrective action.
• He shall be responsible to maintain fire extinguishers and first aid kit at workplace
and up to date the same.
• He shall be responsible to conduct frequent trainings for the same.
He shall be responsible to maintain all records related to PPE compliance, Hazards
compliance, Work permit, Safety material records, Health records, Medical expenses, Injury,
Accident cases records.

Contractor and his Site In charge:

• Contractor is the main person who is carry out work at site directly by deploy
workers and supervisors.
• He shall be responsible for all near miss accident, accident, incident took place at site
due to unsafe act, unsafe condition and due to negligence of workers or due to
failure/fault of machine, tools.
• He shall be responsible to provide trained, experienced person to carry out work at
site. He shall be responsible to provide mandatory PPE to all; Safety shoes and safety
helmets.
• He shall be responsible to provide other PPE as per work requirement such as safety
harness for height work, safety goggle, ear plug, nose mask for concreting, chipping,
grinding work and face shield for welding hot work, fall arrestor for bamboo
scaffolding and other scaffolding of more than 10 meter height.
• He shall be responsible to provide good condition tested IS approved PPE (Such as
Brand of KARAM, Anchor, ACME) and machines for use at site.
• He shall be responsible to submit test certificated of the PPE's and machines.
• He shall be responsible to provide first aid and fire extinguishers for give first aid to
workers and control fire incident as per workplace men power and area of working.
• He shall be responsible to control workplace hazards before start of work and to
provide total PPE's to all workers and staffs working directly under his contract or in
his sub contract.

• He shall be responsible to provide and deploy safety officers for training, monitoring,
and hazards control at site as per site requirement.
• He shall be responsible to submit all Safety training records, inspection records and
other reports as per job requirement and instructions of safety executive of Marathon
Group.
• He shall be responsible to provide all medical help to workers in case of injury and
accident at site and in case of hospitalization.
• He shall be responsible to change damaged PPE and machines, tools provided at site
whenever observed and reported by Safety executive.
• He shall be responsible to provide mosquito nets to all workers in monsoon season in
July.
• He shall be responsible to carry out all workers blood test for MPSC/Malaria and to
submit report of testing at site office to Safety executive.
• All workers blood test required to conduct in monsoon season 4 times/ every month
from July to Oct. month.
• He shall be responsible to provide all electrical connections with proper method and
with good condition IS material.
• He shall be responsible to provide/deploy trained and experienced electrician and
operators from working at site. He shall be responsible to maintain workplace, site
and labour camp neat and clean all the time.
• He shall be responsible to dispose all waste and scrap from site to avoid unhealthy
situation and fire cases at site.
• He shall be responsible to eliminate workplace hazards before start of work such as
placing/fixing safety nets vertically at working floor and horizontally at immediate
below working floor and at podium level.
• He shall be responsible to fix safety nets wherever required to protect persons and
arrest fall of material at site as per instruction of Site Head.
• He shall be responsible to cover all ducts, lift shafts and cut outs at working level,
immediate below working level and at every 3rd floor from mid side by safe and
secure method.
• He shall be responsible to barricade all edge of excavated area, openings, lift shafts,
ducts, cut outs etc. from front side to prevent fall.
• He shall be responsible to clear all issues reported by safety executive for safe
working and accident prevention at site.
• He shall be responsible to deploy his representative at site for accept and
acknowledge all observations, issues and documents, receive permit to for height,
hot and cold work (While erect any type of scaffold, Cutting, Grinding, Welding and
External painting, Crane, Lift maintenance, Erection, dismantling work).

Other Departments working at site and Head Office Departments:

• All staffs of other departments of sites and working at head office are responsible for
safety of individual and for his area of working.

• Department head shall be responsible to prepare his department and work under his
control running, H.I.R.A. preparation and to communicate related precautions and
safety guidelines.
• Fire, Safety, Housekeeping and Use of PPE's is responsibility of Department head
and of person who carrying work at site or at head office.
• If anything found unsafe at any site or any floor of head office, immediate stop and
report to concern person for implement right process.

Observation:
All role and responsibility written in manual and all staff aware about safety.

Recommendation:

Required to all staff’s role and responsibility display on board.

Definition: SAFETY committee can be a good option if you have a significant number of
workers who may be reluctant to take on the role of a health and safety representative (HSR).
SAFETY committee is constituted comprising project manager as the chairman, heads of
concerned department/section and contractor/worker representatives as the members and
safety officer as the secretary. The formation is duly announced through a circular signed by
Site In charge.

Main functions of SAFETY Committee:

• facilitate co-operation in improving, developing and carrying out measures to


improve the health and safety of workers assist in developing health and safety
standards, rules and procedures

HAZARDS ASSOCIATED:

• Legal issue, if not followed as per BOCW Act


• Issues increased and not properly addressed,
• Moral decreased
• Improper recordkeeping,

The committee also found continuously does the safety round as a part of work and
gives onsite training to other employees.

While determining the risk controls, consideration is given for reducing the risk
in the following priority / hierarchy of control.

i. Elimination of risk
ii. Substitution of materials / processes
iii. Engineering controls
iv. Signage/warning/visual/administrative control
v. Using of Personal protective equipment’s

The management directive is communicated to concern and also dis-


played on the communication boards for information to all employees.

Observation

It is observed that member of each department is contributed in a safety commit- tee as


a committee member.
Committee members are found well trained in all fields such as first aid, firefighting.
Safety committee member are found meets every month for comprehensive meeting and
discussions are held on EHS observations; target date is given, minutes recorded in the
MOM, Minutes of safety committee are displayed on notice boards and sent to
concerned members through E-mail and report is sent to the corporate office.

Recommendation:

Motivational Scheme

1. PURPOSE

The purpose of this procedure is to serve as formal means to motivate employees in order
to implement and continually improve the SAFETY management system through
employee participation as management program.

2. PROCEDURE

Definition: A motivational program is a formal scheme used to promote or encourage


specific actions or behavior or persons by a specific group of people during a defined
period of time. Motivational events is a tool to increase thoughts in to action, people
work more efficiently and give his 100% inputs for organizations growth.

HAZARDS ASSOCIATED:

• Less engagement towards company,


• Higher absenteeism,
• Unwillingly work style,
• Demoralize those giving high inputs,

There are many motivational events conducted by organization for workers and staffs at
site, as mention below:

• Near Miss reporting: Prize on quarterly basis to person (staffs/workers) reported


maximum nos. of near miss cases with details.

• Best Practice: Prize on Quarterly basis to Site persons of company and contractors
who has found carry out work safely with all safety measures on place.

• Best Contractor of Site: Prize on annual basis to contractor who has found carry out
work safely as per rules and provides safe and healthy workplace and labour camp to
workers at site. To complete work of year without any accident and incident.

• Award Function: Prize and certificates to all workers and staffs, those found
throughout the year carry out work safely and with all safety measures yearly.

Festival celebration: Celebration of Festivals such as Depawali at every site. Distribution of


sweets and snacks to all attendees.

• National Event Celebration: National Safety day/week celebration at every site. On


that day quiz competition, slogan competition and other motivational programs
conducted.

• Welfare Events such as Medical and health awareness camps: Welfare events
such as Cancer awareness, HIV awareness, Health awareness and checkups.

• Crèche room for children's of workers: Crèche room facilities provided for 30 or
more children's living at site with workers family. Not applicable if less than 30
children at a site.

APPRECIATION PROCESS:

• Appreciation or award is given to workers, employees, contractor, who is work


safely, reporting maximum number of near miss incidents at construction, followed
safety guidelines.

• The selection is based on the performance, criticality and the quantity of


improvements, safe practice, inputs and incidents reported.

• SAFETY executive in consultation with Site Head and SAFETY Head selects the
workers, employee and contractors for these awards.

SAFETY PROMOTION

• SAFETY Induction: All employees shall attend Project SAFETY induction briefing
conducted by SAFETY Executive or SAFETY Manager. The duration of this
orientation will be 30 minutes or more than it. The purpose of SAFETY Induction
training is as follows.
Introduction, SAFETY Policy, SAFETY Responsibilities for each individual, Site
Hazards, PPE, SAFETY Signage & Posters.


Visitors Induction: All visitors to site shall be given; An outline briefing of their
responsibilities as contained in the formal orientation which includes the site-specific
Emergency Response Plan. Information stating the site rules. Safety equipment safety
helmet, safety shoes, reflective jacket and Introduction, SAFETY Policy, SAFETY
Responsibilities for each individual, Site Hazards, PPE, SAFETY Signage & Posters

• SAFETY Toolbox talks


In order to inform potential hazards of the job site, all employees shall participate in
the toolbox meeting and the attendance shall be documented and retained. This
meeting shall be conducted by the SAFETY executive and should address the
application of SAFETY rules and procedures to the hazards involving at workplace.
SAFETY personnel will monitor and support supervisors to make it more effective
and meaningful.
Twice in a month toolbox talks conduct at site.

• Specialized Training
The specialized training shall be conducted by SAFETY Executive alternate month for
workers and equipment operators working at areas in which major hazards are
identified from job safety analysis. This training shall be completed prior to the
commencement of the work. Riggers, welders and helpers, cutters, operators of lift,
hoists, crane, earth moving equipment, and forklift, dump truck, vehicles with wheels.
First Aiders, Confined Space Entrees and Signalman.

• Motivational program
Motivational programs will be organized periodically to appreciate, motivate workers
and staffs
Performed best practices in SAFETY to encourage the moral of workforce.

• Training records:
All SAFETY induction, toolbox talks and trainings records shall be maintained in
format or in register. It shall be kept in good condition. Monthly summary sheet shall
be prepared for the same. The SAFETY Executive shall prepare and maintain the
training register of employees/workers who have been inducted, trained and when

FIRST-AID MANAGEMENT

Definition: People at work can suffer injuries or be taken ill. It doesn’t matter whether the
injury or illness is caused by the work they do or not, it is important to give them immediate
attention, Action taken to minimize severity is called first aid.

HAZARDS ASSOCIATED:
• Misuse of first aid kit,
• Improperly maintained first aid,
• Use of wrong contents, Expiry medicines,
• Non availability of first aid kit and items,
• Late response on handling of injury case,
• Severity of injury increased,

PROCEDURE:
• Make a list of first – Aid trained personnel available at workplace and circulate the
same among all concerned for easy identification of those whose services are
required in this regard.
• First-aid kits inside a first- Aid box are provided at convenient locations under the
charge of a trained person with a list of approved medicine / aids made available. If a
first Aid center is established inside the premises of the workplace, the same is
maintained under a part time trained attendant to provide first-aid to the casualties.
• Safety Executive checks the entire first-aid kits monthly and facilitates availability of
the required quantity of all the specified contents.
• In case of an injury, or ill health case, the affected person requiring first-aid
treatment is sent for the same expeditiously
• First-aid record register is maintained by first aider for keeping on record the names
of the casualty or sick person with brief, which is checked weekly by safety officer.
• Safety officer analyses the first-aid cases during the month and send analysis of first-
aid and illness cases report to Project Manager and SAFETY Head.
• The exact contents of your first aid kit should be based on workplace injury such as
nail puncture, hit of person and wounds etc.
• All workers must have access to a first aid kit. This will require at least one first aid
kit to be provided at workplace, Security cabin and at civil contractor’s office. First
aid kits can be any size or shape, provided they are large enough to contain all the
necessary items.
• Contractor shall be responsible to provide first aid kit and first aider for his workers
as per strength and site requirements.
• Monitor accidents and ill health, and review your first-aid provision as appropriate.

SAFETY Inspection:
A safety inspection looks at the physical conditions and work practices in a workplace.
Equipment is examined to determine whether all safeguards are in place and whether
its operation presents any hazards. The overall goal of a safety inspection is to identify
hazards so they can be eliminated, guarded, or protected against.

SAFETY Audit
A SAFETY audit involves measuring and collecting information about the reliability
and effectiveness of the safety inspections, programs, training, plans and systems
within a workplace. A safety audit is verification that the safety programs are working.

Responsibility:
SAFETY inspection and audit shall be done by internally and externally for other sites
by Site Head/Manager, SAFETY Manager and SAFETY Executive periodically.
All inspection and audit finding shall be cleared by concern person of company and
contractor.

SAFETY Inspection:

• SAFETY inspection carried out by SAFETY Executive, Execution executive and


duty of every person who is working at site or carrying work at site.
• SAFETY inspection record shall be kept in SAFETY Observation record format as
per instructions given in format. All activities working at site, Labour camp and in
Infra. Work shall be recorded in the same. Observed hazards, housekeeping issues,
PPE issues.
• The purpose of SAFETY inspection and record keeping is to secure compliance,
enforcing concern to clear issues and to promote health and safety at workplace.

Steps in an Investigation Process:

The investigation process should begin after arranging for first aid or medical treatment

include:
1. Call or gather the necessary person(s) to conduct the investigation.

equipment or interface problems may need to be considered.

• Erection, dismantling and modification of scaffolding shall be done only by persons


qualified to install scaffold under experienced person supervision.

• Scaffolding material which is inspected by EHS Executive and Concern Person, If


found corroded, damaged and band should be rejected and shifted from site same
time.

• Safety tag for Safe and Unsafe Scaffolding shall be fixed by Execution person who is
inspecting scaffolding for Formwork or any other use.

• Bamboo scaffolding shall be inspected once in a week and daily in rainy season by
concern person. Bamboo scaffolding shall be erected as per guidelines given in IS
3696:1:1987.

• Standard scaffold should be placed vertically or slightly inclined towards the


structure. Spaced closely enough to provide an adequate support. On a base plate and
sole boards to prevent displacement.

• Ledgers-Ledgers should be Horizontal, fixed securely to inside of scaffolds with right


angled load bearing couplers. Securely fixed to ledgers or scaffolds with right angled
or putlog couplers. The putlog should usually be placed horizontal and be
approximately 75 mm in length.

• Lengths of putlogs and transoms-the length of putlogs and transoms will vary
according to their intends use but must relate to the minimum width of the platform.
The ends of putlog or transforms should not project an in necessary distance.

• Ledger Bracing: Ledger bracing should be fitted on alternate pairs of standards,


except where the width of bays are 1.5meter or less then should be fitted on every
third pair.

• Ledger bracing should be fitted to start at base of plate level, to the full height of the
scaffold.
• Longitudinal (façade) Bracing –Façade bracing must be provided for all scaffolds
unless the longitudinal stability of the scaffold is obtained.

• Ties- ties link the scaffold to the building and are provided to resist the inward and
outward movement of the scaffold and also to give some additional stability. Scaffold
fitted with sheet in, tarpaulins etc., will be subjected to extra stress due to wind
pressure and will required more ties.

• Guardrails-where personnel are liable to fall from 2m or more. at a level between


910mm but not more than 1150mm above the level of the platform.

• Ladder-ladder used for access to the workplace shall be extending to a height of


900mm (3 rungs) above the working platform and be positioned approximately at
angle of 75 degrees

• Inspection – No scaffold shall be used unless it displays the approved SAFE TO USE
tag has been inspected every day. Rejected scaffold will be red tagged “DO NOT
USE” displayed prominently and personnel are not to use such scaffold. Tag shall be

Excavation: Excavation includes any earthwork, trench, well, shaft, tunnel or underground
working. It is the act or process of digging, especially when something specific is being
removed from the ground. The removal of earth, usually to allow the construction of a
foundation or basement.

HAZARDS ASSOCIATED:

SAFETY MEASURES:

• Excavation is one of the important phases of the construction activity. Due to


insufficient attention to the safety aspects, it has become the cause of many
accidents. It is therefore necessary to plan and execute all excavations in safe
manner. Soil analysis is important in order to determine appropriate sloping,
benching, and shoring.

• It is to be ensured that personnel with thorough knowledge and experience of


excavation work supervise all excavations.

• The integrity of the possibility and support system shall be inspected prior to the
commencement of any work on a daily basis.

• Where there is the possibility of any ingress of water, then pumping shall be
established with pumps being daily available for use and additional ladders placed
for use in the event of the emergency evacuation.

• No excavation shall commence till the disposal area has been earmarked and is
ready to receive the excavated material. Excavated spoil, plant or materials should
not be stored close to the sides of excavations as loose material can fall in. The
extra loading can make the sides of the excavation more likely to collapse.

• Before starting any excavation of any description in the existing site, permission in
prevents slides. If sloping is impractical, shoring shall be used whenever the
vertical dimension exceeds 1 meter. After completion of excavation activity,
shoring shall be removed systematically to ensure prevention of sliding of earth
endangering crew working inside.

• Barricades/handrails (1.2-meter-high), signals, or other appropriate warning


devices to protect personnel from any hazardous operation during excavation shall
be provided at every accessible part of an excavation such as pit etc. It shall be
properly covered when handrails or barricades do not provide adequate protection.

• Excavations shall be avoided during night and if necessary, illuminated all areas
properly.

• No excavation or earthwork below the foundation level of any adjoining building


in an existing site shall be taken up unless adequate steps are taken to prevent
damage to the existing structure.

• Mechanical excavation should not be started where underground cables and pipelines
are less than 1.2 meter below ground. Initial digging shall be by hand, in a cautious
manner. Tiles covering electric cables shall not be removed without the consent of
authorized person.

• The excavation permit shall be used.

• If cables or pipelines are accidentally damaged, concern contractor shall notify


concerned authorities immediately.

• Trench collapse should be avoided by battering the sides to a safe angle or by


supporting them with sheeting or proprietary support systems. Support should be
installed without delay as the work progresses. Ensure the workers are competent and
experienced as far as possible and that they have clear instructions.

• Provide safe access in and out of the excavation.

• If cables or pipelines are exposed during excavation work, they shall be sufficiently
supported and protected. No material or load shall be placed or stacked near the edge
of the excavation or opening in the ground. The excavated material shall not be
placed within 2 meters of the excavation area, trench and If depth is more than 5-
meter, material shall be store at separate place.

• No under cutting on the side of excavation shall be allowed.

• Provide safe access and egress for all excavations, for excavation 4 feet (1.2 meters)
deep or more, ladders, steps or ramps for safe access and egress must be provided
within 25 feet of lateral travel. Ladders shall extend above the top of the excavation
by 1 meter for step off.

Definition: Hot work is any process that can be a source of ignition when flammable
material is present or can be a fire hazard regardless of the presence of flammable
material in the workplace. Common hot work processes are welding, soldering, cutting
and brazing.

GENERAL SAFETY PRECAUTIONS

• Equipment operators should be competent and familiar with the equipment to be used
and
should inspect it before use.

• Operators should be given safety instructions if special precautions need to be taken.

• Compressed gas cylinders fitted with safety valve and must not be use in confined
space.

• Before any operation begins, inspections and tests should be carried out to ensure that

• Adequate ventilation should be provided to extract fumes generated by welding and

• operations. If adequate ventilation cannot be accomplished, suitable respiratory
protection should be worn.
• Personal Protective Equipment’s:
The equipment operator and other persons involved in the hot work process should
wear clean and approved personal protective equipment. PPE’s should be free from
oil, grease or other flammable substances.
• The operator and persons involved should wear:

Boiler suit shall be provided to welder as mandatory clothing
• Electric welding equipment:
i. The power supply should be alternate current (AC), which minimizes the risk
from electric shock and later converted by machine in to constant current
(CC).
ii. Cables should be inspected before use, and connectors should be fully
insulated. Damaged cables and connectors should not be used.
iii. Circuit breakers/ELCB/Suitable means should be provided for rapidly cutting
off current from the electrode, should the operator encounter difficulties.
Welding equipment should be kept switched off when not in use.
iv. All welding torches should be properly insulated. Torches with damaged
insulation should be disposed off.
GAS CUTTING AND BRAZING:
• Equipment should have backpressure valves fitted adjacent to the torch in the oxygen
and acetylene lines and flash back arresters should be used at cylinder and torch
end. Oxygen may explode under excessive pressure. It should not be used at a
pressure exceeding one (1) atmosphere gauge
• If a backfire occurs, the valves on the oxygen and acetylene cylinders should be
closed immediately. An acetylene cylinder that becomes overheated is very
dangerous as an impact could cause internal ignition and subsequent explosion.
• Only hoses which have been specially designated for gas cutting operations should be
used. Hoses in which a flashback has occurred, or which are in any other way
damaged, should be discarded.
• Torch should be lit by a special friction igniter/match box. Gases should be shut off at
the pressure reducing regulatory before a torch is changed.
• Gas torches shall e dismantled and cleaned at regular intervals to ensure that there are
no blockages. Red hoses shall be used for acetylene and flammable gases whilst
oxygen hoses shall be blue.
• Hoses end connection shall be properly secured. Regular inspection is required before
use. Leak test shall be performed to all connection parts to ensure no leakage of gas
before every use by concern agency operator. Empty and Full gas cylinder shall be
tagged and properly stored at store area.

Working at Height

Work at height:

The Work at Height Regulations applies to all work at height where there is a risk of a
fall liable to cause personal injury.
Definition: Work at height means working in a place (except a staircase in a
permanent workplace) where a person could be injured by falling from it, even if it is at
or below ground level. Working at height remains one of the biggest causes of
fatalities and major injuries. Common cases include falls from ladders and
through fragile surfaces. ‘Work at height’ means work in any place where, if there
were no precautions in place, a person could fall a distance liable to cause
personal injury (for example a fall through a fragile roof).

The following points to be noted at the time of working at Height:


• Properly planned for any emergencies and rescue Appropriately supervised;
and Carried out in a manner, which is reasonably practicable safe. Height
work 1.8 meter or above shall be consider as height work.

• Where it is required to work at height, appropriate scaffold shall be erected,


or ladder shall be provided. Scaffolding other than metal tube shall be
permitted with written approval for special case. Working platform shall be
provided with Guardrails at 1.1m, mid rail at 0.55m.

• Ladders and scaffolding shall be regularly inspected, and record shall be


maintained.
Inspection: A scaffold used for construction should be inspected before it
is used for the first time and then every 7 days, until it is removed. It should
also be inspected each time it is exposed to conditions likely to cause
deterioration.

• Use of PPE- All fall protection safety PPEs like safety helmet, safety shoes,
safety harness required for working at height are provided.

Continuous supervision: While working at height, continuous supervision shall be done


by contractor.

Safety net installation:

• Safety nets shall be installed by contractor always at minimum three levels.


It shall be shifted to upper levels as per slab work and in high rise tower it
shall be fixed horizontally at every 5th floor independently from podium
level to one level below of top slab.

• First level of safety net shall be installed at the top working floor which will
be vertically fixed with Square tube and Mivan brackets. Safety net
specification (Border rope 12 mm, Inner rope 6 mm with blue fisher net,
Mesh size 100 mm X 100 mm).

• Second level of safety net shall be installed at immediate below working
level. This net shall be similar to the net on first level & horizontally fixed
with MS Props and swivel coupler.

• Third level of safety net shall be installed at podium level at periphery of


building. Podium net fixed with support of I Section (300 mm) and
Anchor fasteners, nylon ropes from fixed structure or securely grout
structure. Safety net specification (Border rope 12 mm, Inner rope 6 mm,
Mesh size 100 mm X 100 mm).

• At every 4th floor horizontal nets need to fix in Cut outs, Ducts, Lift shafts
and Peripheral area of building for fall arrest from middle floors.

• Safety net fixing, removing, cleaning and material proper stacking work
shall be done by civil contractor at site as per site and work requirements
whenever required and instruction by Engineer in charge.

• Testing of safety nets:


Safety nets shall be received at site from supplier only with test certificates
by store person. Safety nets fixed at workplace it shall be drop-tested of 50
kg Gunny bag at anyone point, frequency of testing:

i. After initial installation and before being used.

ii. Whenever relocated.

iii. After major repair.

iv. At 6-month intervals if left in one place

• Cleaning of safety nets:


i. Safety nets shall be cleaned by contractor after every concrete work and

whenever found placed any material/debris.

iii. Safety nets shall be cleaned and checked in frequency of 15 days


period

Safety Harnesses/Fall Arresters:


• Use of safety helmet, harnesses and safety shoes shall be mandatory for
working at height and training need to give for proper use of the same.

• If safety harnesses are used, they should be of the full body type and Anchorage
point/lifeline shall be provided and used. If Column Vertical Steel is use as
anchorage, then two level lateral ties must be there. Lifeline shall be provided
by 16 mm nylon rope.

• While working on bamboo or any other scaffolding fall arrestor shall be
mandatory to use by workers. Not allowed in absent of fall arrestor.

Use of scaffolds
• Scaffolding must only be erected, altered or dismantled by competent riggers of
contractor. Contractor concern person shall be responsible to daily check and
ensure safe working on scaffolding with proper safety measures.

• Use only complete and safe to use scaffold and when Green Tag found on
scaffold.

• All scaffolds must be inspected minimum twice in a month as per checklist and
on alterations are made by concern persons.

• Never use any incomplete scaffold and when Red Tag found on scaffold.

• All scaffolds must be constructed of sound materials free from rust, band and
defects.

• The following measures shall also be taken during scaffold use:

i. The scaffold shall be constructed for the correct use (light or heavy duty).
ii. Securely fixed to existing structures or adequately buttressed.
iii. Items such as barrels, boxes, loose tiles or other unsuitable material must
not be used as supports for working platforms.
iv. All working platforms shall have guardrails at one-meter height and shall
also have an intermediate rail at half height.
v. All working platforms shall be kept free of unnecessary obstruction or
rubbish.
vi. Secured ladder access shall be provided.
vii. All working platform shall be fixed and tied properly, and double MS
planks provided for working on height and stand safely.

Scaffold Inspection Points:


• Base: The lowest part or edge of scaffold, especially the part on which it
rests or is supported. Base shall be level, sound, rigid, and capable of
supporting the loaded scaffold without settling or displacement.

• Line of standards and ledgers: On a tube and fitting scaffold, standards


(MS Tube) must be staggered if the top working platform is higher than
the longest length of tube.
• Diagonal bracing (in both directions): A member installed at an angle to
make a rectangular frame more rigid. It provides lateral stability
preventing collapse of a scaffold.

• Security of board/Platform: Platform shall be provided with guardrails,


toe guards and mid rails.

• Ties: Tie used to tie in fixed structure for stability of scaffold.

• Correct use of tubes, couplers and fittings: Use of defect free, band free,
rust free couplers, tubes and scaffold fitting shall be used.

• Even spread of load on platform: Load shall be defined for use of


scaffold

• Means of access: It shall be marked properly, and relevant signage's shall


be displayed. Access shall be kept clean and no material placed in the
access.

• Maximum intended load" means the total load of all persons, equipment,
tools, materials, transmitted loads, and other loads reasonably anticipated
to be applied to a scaffold or scaffold component at any one time.

• Security of stacked materials: All scaffolding material placed shall be tied


properly and Toolbox mandatory for placing small tools, parts or
material.

Use of Ladders:
• All ladders shall be of sound construction and shall be free from patent
defects.

• Ladders shall be checked weekly and defective ladders shall be promptly

with the purchase of the same. For new appointments allowance will deposited in his
1st salary.
• Contractor shall be responsible to provide all required and mandatory PPE’s to his
staffs, workers. Company shall not be held responsible to provide.
• Penalty for Contractor staffs and workers is Rs. 200 per person, if found without PPE
and removing from work/stop of work same time.
• If PPE like safety helmet lost, damage, it should be replaced only in 6 months period
and other PPE’s as per approval and condition of PPE. Each employee shall be wear
required PPE’s at site as per job requirements. Mandatory PPE’s are Safety helmet,
reflective jackets and safety shoes. When working in such areas where there is a
danger of injuries due to work activities.
• Cap, Ordinary goggles, Slipper, Sandals, Canvas and sports shoes or other shoes are
not permitted to be worn.
• Employees who want exemption from wearing any PPE at site on temporary medical
or physical conditions shall take necessary approval on application from the Safety
Executive and Site Head. Employee not allowed to visit or work on site in the areas
where there is chance of injury in case of health issue, temporary medical or physical
conditions.
• Company will not be responsible for any injuries caused due to non-wearing of PPE
at site. Company, Management and Safety Department will not be responsible for any
legal action, compensation etc.

WORK PERMIT SYSTEMS
Definition: The commitment to follow safety instructions, in writing is obtained through
“Permit System”, which has to commence from the day, the first construction activity
starts at site until the last activity. “Permit System” is followed in case of those activities
where there are higher potential risks and hazards. These generally are:
i. Hot work
ii. Entry into confined space, shaft, UG tank.
iii. Working on electrical lines/equipment
iv. Height work.
v. Demolition Work.

4.1.3 This chapter lays down steps to be taken by project manager before and during the
above
mentioned activities at construction site. There are three different “permit” forms, each deal
with various activities. Each form has certain standard instructions and space for writing
special instructions by project manager, related to the activity and site situation. These
instructions have
to be clearly understood by the workers, engineer and ensured that they are followed by the
persons carrying out the concerned activity. The forms, therefore, require signatures of
concerned responsible persons, implying their commitment to the instruction. The forms also
require communication regarding completion of the permitted activities. After the form is
returned stating completion of an activity, the same activity cannot be carried out again
unless the
permit is renewed.

3. ADMINISTRATION

Safety officer shall ensure that the procedure as outlined below is followed:

• Contracts department shall inform all contractors about the permit system and advise
them in writing (by letters and notices) that permits are required to carry out hot work
and height work activities as mention in work permit.
• Contractors shall be advised to apply for permits for the concerned activity at least
one day before, if activity starts early in morning, or on same day prior to starting of
activity.
• Safety officer shall issue the requisite permit form, noting down special instructions,
if any, to the contractor’s supervisors, who will sign on the form.

• After the site and persons are ready as per the instruction on the form, site
engineer/supervisor
shall inspect the work site, persons and mark the check points as per the requisite form and if
instructions/check points are fully complied, the contractor’s supervisor, project engineer
shall sign the form to permit the activity for the period and date mentioned on the form.

• During the progress of the activity/work permitted \ the contractor’s and project
manager designated engineer/supervisor shall ensure that there are no deviations from
the instruction.
• Project manager or any supervisor and engineer can stop work and cancel the permit
immediately by stating the reason and signing the permit, if they find any deviations
during the progress of work.
• When the activity / work is completed or the period / date stipulated on the form has
lapsed, safety officer, shall ensure that the contractor’s supervisor returns the form
(duly signed) to safety officer who shall file it for future reference.
• Fresh permit shall be issued if the activity has to be continued after completion of the
period, if the work site is different, or if the work site conditions change.
• Contractor’s supervisors shall be advised by safety officer to keep one copy of the
permit
with them as long as activity/work is in progress and it may be demanded by any person as a
proof for authorization of the work in progress.

MANUAL MATERIAL HANDLING

Manual material handling operations are carried out in most construction sites. Each
handling task poses unique demands on the worker. However, workplaces can help workers
to perform these tasks safely and easily by implementing and upholding
proper policies and procedures.

Hazards

To assess the hazards of manual material handling operations, consider the load, the task, the
environment in which the task is performed, and the operator. When these fac- tors interact
with each other, they can create hazards that result in injuries.

A load may be hazardous because of:

● Weight
● Size
● Shape (making it awkward to handle)
● coupling (type of grip on the load)
● Slippery or damaged surfaces
● absent or inappropriate handles, and
● Imbalance surface (i.e., changing center of gravity)

health center and also at the all site. The necessary instruction for the use of medicines is

First aid boxes are available in all sites for providing immediate medical help to ill or / an
injured person. A system is in place ensuring that first aid box is always in good condition.
Also about 18 first aiders trained by St. john Ambulance. Refreshers programs are conducted
every year for these first aiders and also to train some more to take care.

emergency are fire, accidents, near miss cases etc.
• Natural emergency: The conditions which arise due to act of nature and effect people into
great suffering and helplessness. Natural disasters are wind related floods, cyclone, floods,
cloud burst, earthquake, landslide etc.

Motto behind Emergency Preparedness Response Plan:

• Emergency is a situation in which the normal patterns of life within a community gets
suddenly disrupted due to some act of nature, and this puts the affected people into great
