A. Content Standards Learners demonstrate foundational competencies across reading and numeracy domains, along with awareness of personal health
status, as assessed through diagnostic tools. This week serves to identify baseline proficiency levels and health indicators to inform
targeted instruction and interventions.
B. Performance Standards Learners will participate in assessments on phonemic awareness, word recognition, and reading
Reading comprehension. They are expected to demonstrate appropriate decoding and understanding of texts
aligned with their grade level.
Learners will complete tasks involving basic number sense, operations, and problem-solving. They are

expected to show functional understanding of grade-appropriate mathematical concepts.
Learners will undergo height and weight measurements to assess BMI-for-age. The objective is to

establish nutritional profiles that guide school-based feeding programs or referrals.
Learners will be assessed using formative tools designed to gauge prior knowledge and skill readiness.
