I4061172039
ANATOMY

• Diameter: 9-10 mm
• Anterior surface (radius: 10 mm) & Posterior surface (radius: 6 mm)
• Refractive Indeks: 1.39; Refractive Power: 15-16 D
GROWTH
• Youngest cells are always on the
surface and the oldest cells in the
center of the lens.
METABOLISM
EXAMINATION

Retroillumination
(Bruckner Test)
Opacities appear black in
the red pupil
CATARACT
• Loss of transparency • Symptoms
(opaque) of the lens. • Blurred vision
• Difficulty in night driving
• Objects appear browner or yellower
• Image distortion
• Monocular or multiple images
• Glare
• Loss ability to discern colors
CATARACT
• Based on:
• Etiology (acquired of congenital cataracts)
• Morphology
• Maturity
CATARACT -
ETIOLOGY
SENILE CATARACT
• Age-related cataract, usually above 50 years. By age of 70
years, over 90% individual will develop senile cataract.
• Occurs in 2 forms:
• Cortical (soft cataract)
• Cuneiform
• Cupuliform
• Nuclear (hard cataract)
CATARACT - MORPHOLOGICAL
NUCLEAR SENILE CATARACT
• In 40th, lens become hardening because of the over
production  especially the nucleus.
• Yellowish-brown colour
CORTICAL SENILE CATARACT
METABOLIC & SYSTEMIC-RELATED
CATARACT

• Diabetic Cataract (snowflake cataract)

• Galactosemic Cataract
• Hypocalcaemic Cataract
• Wilson’s Disease
• Myotonic dystrophy
• Shield like anterior subcapsular in atopic dermatitis
TRAUMATIC CATARACT
• Most common cause of unilateral cataract in young individuals
• Penetrating trauma
• Blunt trauma may cause a characteristic flower-shaped opacity
• Electric shock is a rare cause of cataract, patterns including
diffuse milky-white opacification and multiple snowflake- like
opacities, sometimes in a stellate subcapsular distribution
• Infrared radiation, if intense as in glassblowers, may rarely
cause true exfoliation of the anterior lens capsule
• Ionizing radiation exposure such as for ocular tumour treatment
may cause posterior subcapsular opacities. These may not
manifest for months or years.

MANAGEMENT OF CATARACT
IN ADULT
• Non-surgical measures • Surgical management
• Treatment of cause of • Indication:
cataract • Visual improvement
• DM • Medical indication
• Cataractogenic drugs • Cosmetic indication
• Irradiation
• Ocular disease
• Improve vision in incipient
and immature cataract
