At North Fork Valley Community Health Center, we offer patients from Perry County and throughout Eastern Kentucky access to the primary care services they need to enhance their health and well-being. North Fork Valley Community Health Center is a joint project of the UK College of Medicine and the North Fork Valley Community Health Center board. It is located at the UK Center of Excellence in Rural Health in Hazard. Our clinic was the first university-affiliated community health center in Kentucky. Through that relationship, we are training the primary care providers of tomorrow through a family medicine residency training program. We’re doing our part to improve access to primary care now and in the future.

North Fork Valley Community Health Center offers general wellness services and primary care for the whole family, including:

• Annual wellness visits and physicals
• Family planning
• Medication management
• Pre-employment screenings
• Preventive screenings, such as Pap tests for cervical cancer and PSA blood tests for prostate cancer
• Testing and monitoring of blood pressure, blood sugar and cholesterol
• Vaccinations

Our healthcare providers specialize in the diagnosis, treatment and management of common chronic and short-term illnesses, such as cold and flu. They can also give you advice on diet, exercise and taking care of yourself to help avoid health issues in the future.

Your primary care provider can also refer you to other providers for conditions that need specialized care.

North Fork Valley Community Health Center is part of the Health Resources & Services Administration’s national Health Center Program. We offer a variety of healthcare services and offerings, including:

The health of your teeth and gums is important to your overall well-being. You can find dental care for everyone in your family at North Fork Valley Community Health Center. We offer dental services by appointment and on a walk-in basis.

We provide dental care Monday through Thursday from 8 a.m. to 5 p.m. Call 606-439-1559 to schedule an appointment. Medicaid and most dental insurances are accepted.

Many Kentucky children ages 2-11 experience tooth decay, which can lead to other health problems and affect learning. To help solve this problem, our center operates the Ronald McDonald Care Mobile, a dental office on wheels. Featuring two dental chairs, the Care Mobile brings education and dental preventive services to children in Perry and surrounding counties.
