PARTICIPANT CONSENT FORM

Title of Research Project:

The Influence of User Generated Content in Buyer’s Decision Making Process: Evidence from the
Philippines

Brief description of research study, and what participation involves:

User-generated content (UGC) refers to any contents in a digital format such as video, photo or
mere characters that are produced, posted or shared by internet users (excluding company
sponsored contents). Examples of UGC includes postings and comments found in the social
networking site, blogs, online forums, product review sites and the like.

For the purpose of this study, UGC is categorized only into three platforms i.e. Social Media Websites
(SMW), Consumer Review & Rating platforms (CRR) and Blogs. This research uses the cognitive
theory of Information Adoption Model to investigate the perceived source credibility and
information quality of the user-generated contents from the viewpoint of the respondents. The data
will be used to determine how it affects a person's behaviour towards his or her buying decision-
making process i.e. need recognition, information search, evaluation of alternatives, the stage of the
actual purchase and the post-purchase stage.

At this point, I would like to ask you to answer all the questions in this survey and answer it based on
your actual experience, perception or judgment.

Thank you very much.
