Overview:
Casey is a withdrawn teenager, having been molested as a child by her uncle John,
her legal guardian. While hanging out with her classmates, Claire and Marcia, the three are
kidnapped by Kevin Wendell Crumb, a man suffering from dissociate identity disorder
(DID). Kevin is in therapy with Dr. Karen Fletcher, who has identified 23 distinct
personalities within Kevin; in his mind, these personalities sit in chairs in a room, waiting
for "Dennis", the dominant personality, to grant them their turn "in the light" (in control).
She has also found that Kevin's physiology changes with each personality. Recently,
"Barry" has refused to allow "Dennis" or "Patricia" their turns, in part due to Dennis'
tendencies towards bothering underage girls and Patricia's undesirable traits, and also
because both appear to worship "The Beast", a yet-unseen 24th personality. Fletcher has
found that she can bring back Kevin's own personality by speaking his full name.
Kevin, shows many of the diagnostic features, such as the presence of two or more
identities, dissociative amnesia, and different perceptions of his own physicality when
changing identities.

Dissociative identity disorder also known as multiple personality disorder is a type of


personality disorder in which a person is suffering from mental disturbance and adopts
more than one personality.

There are several causes of DID:


Childhood Abuse
Problems in Brain Functioning
Sexual, Physical and emotional abuse.

Biological Causes:
Imbalance of Dopamine and Serotonin.
Psychological Causes:
Dissociation with an event

Social Causes:
Physical and sexual abuse and trauma.

Kevin: Dennis is the most disturbing personality and he has OCD because of his
mother. He is very intelligent and tough.
Petricia: A polite British speaking women who at times can be psychotic. Dennis and

Hedwig listens to her and all three are called “The Horde” .
HedWig: He is a 9 years old boy. He loves to draw ad dance to music. He listens well to
Dennis and Patricia.
Barry: An extrovert is interested in Fashion. He was the most prominent personality but
got taken over by The Horde.
Jade: A foul mouth but a sweet girl, who has diabetes and has to take insulin shots.
Orwell: A highly intelligent man in the field of History.
Heinrich: Motioned by Dr Fletcher.
Norma: Is an apologetic personality.

Behavioural Observation:
23 personalities are present in Kevin.
+ Four Personalities are the most dominant.
+ For more than 2 years Kevin did not get his real identity.
+ Barry used to be the most prominent and he used to meet DR Fletcher. But Dennis took
over him.
+ Changes in his personality from man to woman, an adult to a child according to the
situation. Changes in his dressing and way of talking changes with his personality.
+ A 24th personality emerges at the end named as “beast”.
Personal history

 Kevin was born to parents Clarence and Penelope Crumb.

 Kevin adopt 23 personalities because he suffers from DID.

Light”

 This disorder itself is characterized by disruption of consciousness in the subject.

 Throughout the movie he is visiting his psychiatrist Dr. Fletcher

Past medical history

Proposed Diagnosis

 Disruption of identity characterized by two or more distinct personality states, which may
be described in some cultures as an experience of possession. The disruption in identity
involves marked discontinuity in sense of self and sense of agency, accompanied by related
alterations in affect, behavior, consciousness, memory, perception, cognition, and/or
sensory-motor functioning. These signs and symptoms may be observed by others or
reported by the individual.
 Recurrent gaps in the recall of everyday events, important personal information, and/or
traumatic events that are inconsistent with ordinary forgetting.
 The symptoms cause clinically significant distress or impairment in social, occupational,
or other important areas of functioning.
 The disturbance is not a normal part of a broadly accepted cultural or religious practice.

Note: In children, the symptoms are not better explained by imaginary playmates or other
fantasy play.

 The symptoms are not attributable to the physiological effects of a substance (e.g.,
black<outs or chaotic behavior during alcohol intoxication) or another medical condition
(e.g., complex partial seizures).

Risk & Prognostic Factors

Environmental: Interpersonal physical & sexual abuse is associated with an increased risk of
dissociative identity disorder. Prevalence of childhood abuse and neglect in the United States,
Canada and Europe among those with the disorder is about 90%. Other forms of traumatizing
experiences, including childhood prostitution and terrorism have been reported.

Course modifiers: Ongoing abuse, later-life retraumatization, comorbidity with mental disorders,
severe medical illness & delay in appropriate treatment are associated with poorer prognosis.
