HOW TO CREATE AN ACCOUNT IN PHILHEALTH FOR EPCB

1. Open your browser and type www.philhealth.gov.ph

2. Click Online Services


3. Click Register/Login
4. Click REGISTER

5. Fill the information needed


6. Click SUBMIT REGISTRATION (Please ensure that your personal email address is working before
clicking SUBMIT REGISTRATION)
7. Wait for atleast 1 minute (CHECK YOUR EMAIL DO NOT CLOSE YOUR BROWSER UNTIL YOUR
ACCOUNT (NOREPLY) IS NOT YET ARRIVED)

8. Open your email address (NOTE: Your email address that you put in the registration must active
and accessible. Here’s what it looks like when the email is arrived.

CLICK the link below your PIN AND PASSWORD to activate your account
10. Here’s what it looks like

11. Go back to your email inbox; copy your PIN and PASSWORD and LOGIN your account in
Philhealth
12. Answer the security question and click SUBMIT
13. In this part, there should be a preferred EPCB provider (QUALIMED HEALTH NETWORK SJDM)

14. If QUALIMED HEALTH NETWORK SJDM, You can come to ROOM 212 for Schedule of Utilization.
(SCHEDULE ONLY!) If UNASSIGNED, YOU ARE NOT QUALIFIED TO UTILIZE THE FREE BENEFITS
PROVIDED BY PHILHEALTH.
