DESCRIPTION OF INCIDENT At about 7.30am on Tuesday 2nd of October, 2018, the Driver Mr ---------- while inspecting and cleaning up Toyota Tundra APP --- FE parked by the Expatriate lodge discovered that the windscreen of the left door of the vehicle has been shattered. He immediately reported the incident to the Deputy Project Manager, Mr xxxxxxx for further action. LIST OF FINDINGS 1. The vehicle was used last on Friday 28 th of September, 2018 when the Admin Officer, Mr xxxxxxxxx went to recharge Airtel Modem at Ikot Ekpene Road, Uyo and was returned and packed safely by the driver 2. The vehicle doesn’t have tarpaulin as such it was not covered to protect the windscreen. 3. The premises is well fenced and secured with security guards which provide control of access into the premises to prevent theft and damage to properties by mischievous persons. 4. There was an environmental clean-up and grass cutting operation between the hour of 8am and 10am within the premises on Saturday 29 th September, 2018 (during state wide sanitation) using grass cutting machine by --------- of Obot Akpan and Co. This suggests likelihood of flying stone from the grass cutting operation on the windscreen. 5. The Estate Manager, ObotAkpan & Co is responsible for the environmental cleaning and grass cutting in the premises. There used to be a communication between the personnel on grass cutting and the company for the company’s vehicles to be repacked each time they want to cut grasses in the premises except on this day. 6. No staff reported to work at the time of the clean-up due to the state wide sanitation and restriction of movement in the state.
IMMEDIATE CAUSE OF THE INCIDENT
Flying stone from the grass cutting operation is much likely to be responsible for the shattered windscreen of the vehicle. UNDERSLYING CAUSE 1. Absence of communication from the estate management to the company about the grass cutting operation so that all company’s vehicles could be parked where they won’t be exposed to flying particles RECOMMENDATIONS
