The undersigned Cielo T. Nadal , INSTITUTION HEAD of the Nabas Aklan

Institute of Technology Inc.,a technical-vocational institution located at Brgy Gibon ,
Nabas, Aklan hereby apply for registration of the following qualification/s /program/s
and offer the same under the laws of the Republic of the Philippines:
TVET Course/s / Program/s / Qualification/s Duration (in hours)
ORGANIC AGRICULTURE PRODUCTION NC II 232 Hours
To support this application are the following documents:
A. CORPORATE AND ADMINISTRATIVE DOCUMENTS
1. Board Resolution to offer program (signed by the Board Secretary and

attested by the Chairperson: SUCs, LCUs, and Private Institutions);
2. Special Law creating the Institution (for public institution, e.g., Republic Act,
Executive Order, Sanggunian Resolutions);
3. Securities and Exchange Commission (SEC) Registration must specifically
cover the training delivery site (for private institutions only);
4. Articles of Incorporation (for private institutions only). For institution that will
branch out, the Articles of Incorporation & Bylaws must state reasons for
opening of the branch. The Board Resolution signed by majority of the
Incorporators must be notarized, received and noted by SEC;
5. Current Certificate of Ownership of building/Contract of Lease (covering at
least two years);
6. Current Fire Safety Certificate.
B. CURRICULUM AND PROGRAM DELIVERY

1. Competency-based Curriculum (Curriculum Design (TESDA-SOP-TSDO-01-

F02) & Course Design/Module of Instruction);
2. List of equipment (TESDA-SOP-TSDO-01-F03), tools (TESDA-SOP-TSDO-
01-F04) and consumables (TESDA-SOP-TSDO-01-F05) necessary to deliver
the program;
3. List of instructional materials (such as reference materials, slides, videotapes,
internet access and library resources) necessary to deliver the program
(TESDA-SOP-TSDO01-F06);
4. List of physical facilities (TESDA-SOP-TSDO-01-F07) and off-campus
physical facilities (TESDA-SOP-TSDO-01-F08) indicating floor area
(Memorandum of Agreement to be attached if rented or leased (Firing Range,
Driving Range, etc.);
5. Shop layout of training facilities indicating the floor area.
C. FACULTY AND PERSONNEL
1. List of officials with their qualifications supporting evidences available, such as

copies of certificates, etc. (TESDA-SOP-TSDO-01-F09);
2. List of faculty teaching on the program, with their qualifications, areas of
expertise, and courses/seminars attended. Supporting evidences available,
such as TQ/AQ Certificates, copies of contracts of employment, etc. (TESDA-
SOP-TSDO-01-F10).
D. ACADEMIC RULES
1. Schedule and breakdown of tuition and other fees (duly signed by the school
head indicating the effectivity of the school year);
2. Documented Grading System, details of which are provided to
students/trainees at the start of their program;
3. Entry requirements for the program comply with the relevant training
regulations;
4. Rules on Attendance.
E. SUPPORT SERVICES
1. Health services are available to the students/trainees (if these services are
contracted out or out-sourced, the contract or MOA or similar documents
must be submitted);
2. Career guidance services are available to the students/trainees;
3. Community Outreach Program (documented evidences available) – optional;
4. Research that supports the operation of the school is carried-out (e.g.
surveys, consultations, meetings with local industry and community
representatives; technical research) – optional.
I, CIELO T. NADAL as INSTITUTION HEAD promise to maintain the

standards required for the qualification/s / program/s and to follow faithfully all laws,
the rules and regulations, and the requirements of the Technical Education and Skills

Development Authority governing the operations of authorized technical-vocational

institutions and to inform the Technical Education and Skills Development Authority
of any action regarding closure or phasing out of the qualification/s /program/s or any
changes in the prescribed requirements.
The institution will not conduct classes in the qualification/s / program/s

applied for above until and unless this application is approved and the Technical
Education and Skills Development Authority issue the Certificate of Program
Registration (CoPR).
