NARRATIVE/ACCOMPLISHMENT REPORT
FIRST SCHOOL LEARNING ACTION CELL (SLAC) SESSION)
September 17, 2020
I. INTRODUCTION
As stipulated in the DepEd Order No. 35, s. 2016 section II No. 6, it is mandatory
to all teachers to undergo trainings and seminars to ensure teachers’ continuing
professional development (CPD) within the framework of School-Based
Management. So, Reina Mercedes National High School is allotting periods or
sessions to conduct School Learning Cells to discuss school concerns, issues, and
problems that need urgent solutions, to talk about updates about teaching-learning
process, new trends and methodologies in teaching, and to disseminate information
which need to be known by the teachers for the benefit of learners or the school as a
whole in this new normal of education.
II. DISCUSSIONS OF THE AGENDA/TOPICS
The First SLAC session was started with a prayer led by Ms. Diana Rose Carpio.
It was followed by an opening remarks of Mr. Noel G. Malsi, principal of this school.
He urged everyone to perform his/her duties and responsibilities religiously and
heartily so to meet the thrusts and objectives of the department set in this pandemic
time. He shared thoughts and words to ponder as one performs his/her tasks as a
teacher amidst the treat of COVID-19. Mr. Malsi asked the teachers what issues or
problems met during the distribution of modules in the different barangays. Some of
the problems raised by the teachers were some students were not listed in the class list
so they did not receive their learning package and observance of social distancing was
difficult to control.
The second part of the session was the discussion on how to construct and make
the Weekly Home Learning Plan and the Individual Learning Monitoring Plan. It was
clearly discussed and demonstrated on how to properly and correctly accomplish
these templates by Mr. Reymar D. Barrientos, HT1. The steps on how to craft the
WHLP and ILMP were well-explained.
Lastly, Mr. Julius Rey C. Agno and Mr. Edison L. Ramil presented and finalized
the crafted classroom program for the school year 2020-2021. The classroom program
was patterned based on the given guidelines by the SDO.
The session was smoothly done because of the active participation and
cooperation of the teaching personnel of the school. Some of the issues were
addressed properly through the suggestions and feedbacks given by the faculty
members.
III. SIGNIFICANCE OF LEARNING
The conduct of the SLAC session allowed each teacher to know the things to be
accomplished, to recognize how to attack issue which needs immediate solution, and
to address problems that may occur in the school. Each member of the faculty was
able to grasp the essence of attending and participating this kind of session for their
improvement and for the betterment of the learners.

SECOND SCHOOL LEARNING ACTIONS CELL (SLAC) SESSION)
October 1-2, 2020
I. INTRODUCTION
The Department of Education initiated the Learners’ Information System (LIS),
an online facility that provides for the registration of learners enrolled in public
schools. Since its installation, the LIS has allowed the Department to generate the
total public school enrolment based on the actual registration and enrolment of
learners.
The LIS is an innovative tool that the Department uses to manage information
with the aim of promoting transparency, informed decision making, and
empowerment at different levels of the organization. While being a tech-based
solution, the LIS involves an inclusive, community-driven process that thrives
because of the active engagement and participation of all teachers, principals,
planning officers and other DepEd personnel all throughout the Philippines.
RMNHS upholds its commitment to ensure the accuracy, completeness, and
veracity of all the learners’ data to be encoded in the system that is why it is a must to
hold this SLAC session.
II. DISCUSSIONS OF THE AGENDA
The focused of the session was merely about the Division Memo dated September
18, 2020 which focuses on the BOSY 2020-2021 encoding in the LIS. The following
salient points were tackled:
1. Deployment of the BOSY prior to the actual opening of classes
2. Date of Official Enrollment
3. Transfer of learner to another school
4. Generation of SF 1, SF 2, and SF 5
5. Additional data/information to be collected ( modality, country of citizenship,
etc.)
6. Setting of the class calendar
7. Confirming enrolment of the learners through phone calls or messaging
8. Discussion on Quick Guide and Frequently Asked Questions
9. Provide assistance to teachers without DepEd email
III. SIGNIFICANCE OF LEARNING

The conduct of the second SLAC session helped each adviser to be familiarized
with the how’s and what’s of LIS in the BOSY encoding for this school year.

MINUTES OF THE THIRD SCHOOL LEARNING ACTION CELL (SLAC) SESSION)


June 29, 2018
- The third SLAC session was conducted last June 29, 2018. Topics discussed and discussants
were as follows:
- Mr. Malsi shared a story about as he opened the 3rd SLAC Session. He gave some reminders
like, refrain students from sending out driving a motor; observe cleanliness; do not allow vendors
inside the school, check electrical circuits; deadline of service credits (Thursday); objectives and
accomplishments for the month of June shall be submitted the soonest.
- Sir Mariano discussed/ talked about the SHS Tracking System. He talked about how the system
works. He briefly gave a glimpse about how to trace the students? If the students are employed;
check all the grade 10 students who enroll in Grade 11; after SHS what happened to the students;
how many of them finished college; how to use the system; what info shall be input in the
system? CONTENT OF SF 1
- Sir Ramil, PTA treasurer, updated and reminded the advisers about the authorized fees to be
collected.
- Mrs. Sinfrosa, guidance counselor designate, talked about the attendance of the students’ and
others.
- Sir Ronnel, property custodian designate, reminded the teachers on how to safe keep of the
school property.
- The session was successfully conducted and ended with positive thoughts.

asked urged all the advisers to be diligent in LIS matters so not to get late in the
encoding as well as to enter the correct data of the students in the system.
After which Mr. Ramil, the ICT coordinator of the school, together with Diana
Rose Carpio,started the orientation and discussion about LIS enrolment. He presented
videos on how to properly enroll the learner.
August 24, 2018, the session was opened up through a prayer. Then, the main
agenda were immediately discussed. Mr. Ramil and Ms. Carpio spearheeadedd the
send session of ICT SLAC.
The session was smoothly done because of the active participation and attention
of the teaching personnel of the school. Some of the issues were addressed properly
through the suggestions and feedbacks given by the advisers. The two sessions of
SLAC were successfully conducted.

III. SIGNIFICANCE OF LEARNING


The conduct of the SLAC session allowed each adviser to properly and correctly
enroll the learners in the LIS.
Each participant was able to grasp the essence of attending and participating this
kind of session for their improvement and for the betterment of the learner and the
school in general.
