    AGREEMENT FOR CCTV INSTALLATION AND CONFIGURATION SERVICE

    (TITLE OF CONTRACT)

    KNOW ALL MEN BY THESE PRESENTS (SALUTATION)


    (THE BODY)

    (INTRODUCTION OF THE PARTIES AND THE AGREEMENT)

    THIS AGREEMENT make this 21 st day of May 2022 between QUEZON STATE UNIVERSITY, a government
    institution, with principal business address at 26 Gumamela Road, Lucena City, Philippines and herein
    represented by President Richard Gomez, and hereinafter called the “Entity” and SUAREZ ENGINEERING
    SERVICES, herein called “the Supplier” represented by Joel Suarez, AVP, Head of Engineering
    Department.

    WHEREAS, the Entity invited Bids for CCTV Installation and configuration Services and has accepted the
    Bid by the Supplier for the supply of goods in the sum of “One Million Two Hundred Thousand Pesos
    Only (PhP 1,200,000.00) hereinafter called “ the Contract Price”.
    (TERMS AND CONDITIONS)

    NOW THIS AGREEMENT WITNESSETH AS FOLLOWS:

    1. In this Agreement words and expressions shall have the same meaning as are
    respectively assigned to them in the Conditions of Contract referred to
    2. The following documents shall be deemed to form and be read and construed as part of
    this Agreement, viz:
    (a) The Supplier’s Bid, including Technical and Financial Proposals, and all other
    documents/ statements submitted
    (b) The Schedule of Requirements;
    (c) The Technical Specifications;
    (d) The General Conditions of Contract;
    (e) The Special Conditions of Contract;
    (f) The performance Security and
    (g) The Entity’s Notice of Award.
    3. In consideration of the payments to be made by the Entity to the Supplier as hereinafter
    mentioned, the supplier hereby covenants with the Entity to provide the goods and to
    remedy defects therein in conformity in all respects with the provisions of the Contract.

    (DISPOSITIVE CLAUSE)

    The Entity hereby covenants to pay the Supplier is consideration of the goods and the remedying defects
    therein, the Contract Price or such other sum as may become payable under the provisions of the
    contract at the time and in the manner prescribed by the contract.

    IN WITNESS whereof the parties here to have caused this Agreement to be executed in Lucena City at 21 st day of
    May 2022.

    Known to me to be the same persons who executed the foregoing instrument and they acknowledged
    to me that the same is their own free and voluntary act and deed.
