GENERAL INFORMATION
1. Submit an application for licensure by examination (score transfer) (Doc.43) and $300.00 fee
(effective June 8th, 2022) . Accepted mode of payments are: U.S. cashier’s check, U.S. money order or
U.S. bank draft. OFF ISLAND personal checks are NOT ACCEPTED. The fee is non-refundable.
2. Submit a copy of Birth Certificate.
3. Submit two (2) original U.S. passport type (2”x2” size) photos with white background taken within
the past 6 months (must be signed on the bottom front portion of each photo). Any colored background
will NOT be accepted.
4. Copy of your College transcript of record.
5. A copy of your High School transcript of record.
6. Send notification to the Board’s office as soon as possible if any change occurs in your name,
mailing address, or other application information after you submit your application. Submit
legal proof of change in name such as a certified and true copy of your marriage certificate or court
order.
7. You must submit three (3) authorization letters stating the following:
i. State Jurisdiction of NCLEX Score – Request to transfer NCLEX Score to NMI BON
ii. NCSBN/Pearsonvue – Request to transfer NCLEX Score from your state of NCLEX
score to NMI BON
iii. NMI BON – Request to accept NCLEX Score transfer from your state of NCLEX score
All above authorization letters must be notarized. Original letters shall then be forwarded to NMI
BON along with the application.
8. A copy of passing letter from the state board which the NCLEX was taken.
The NMI BON will not accept an applicant’s NCLEX score transfer unless all
required documents have been received in our office and payment has been made.
The original member board provides the Examination (NCLEX) department with a formal request for the transfer from
the candidate (letter), which is addressed and sent to the Board to which he/she desires to transfer
The letter must include the candidate identification information such as full name, birth date and NCLEX Candidate ID
number. This letter must be signed by the candidate (a sample template is available from NCSBN upon request).
Once NSCBN has received the request, the Operations team will be in contact with both the candidate’s desired board of
nursing for approval to proceed with the transfer. Should the transfer be approved, the candidate’s result would be sent to the
candidate’s desired board and updated in the NCLEX Administration website.
If an applicant still chooses to apply for the NCLEX score transfer with the NMI BON, he/she will still need to comply with the
requirements provided on our website.
