Form Template & User Guidance

1 Introduction

“Toolbox talk” (TBT) is another term for a safety meeting/ conversation that workers participate in prior to any task being
carried out. It is widely accepted as a common way of ensuring that consultation occurs between workers, and is a
practical way to raise workers’ awareness of specific problems, and safety risks present at the location of work. It helps
workers/ teams to take a proactive approach towards safety by anticipating and taking actions to correct potential unsafe
act and conditions at the beginning of work itself in order to prevent accidents. Toolbox Meetings are helpful in
effectively communicating (and verifying) the following with the participants:

• Scope, and objective of the job at hand


• Schedule/ sequences of tasks
• The tools and material needed to complete the job and their condition
• Risks involved and corresponding control measures to be taken
• Site-specific hazards and safety rules
• Make everyone to think safety before work commences
Toolbox Talks should be scheduled as needed, depending on the level of risks faced on the job, or the levels of experience
of the workers. The Person in charge should assess how often and for how long Toolbox Talks should be held in your
respected sections.

2 Guidance for Completing the TBT Form


Part A • Provide general information such as Date, Location, Shift, Job, Department and Section.
Part B • It is a check list and it is mandatory to check all fields. In case, any of the fields cannot be checked then it
shows that some actions are pending and hence work shall be started only after achieving the same.
• HIRA/JSA, PTW SOP, WMS/ others: Provide reference number/ name/ details, as applicable.
Part C • Discuss (HIRA/JSA) with team and search the work area for any additional hazards using SNAP
technique.
• In case any additional hazard which is not listed in the HIRA/JSA, then those hazards with its control
measure to be mentioned in the space allocated in the form.
• The check box on the hazards falling under Line of Fire (LoF) is mandatorily required to be checked,
even if it is mentioned in your HIRA/ JSA document or the same it is identified at the work area.
• Similarly to the previous point, if the LoF hazards identified on site are adequately captured in your
HIRA / JSA document for the activity, then nothing else is required to be done. However, if this is not the
case, then the details of same should be documented in the TBT form, as discussed in second bullet point.
• Identify and remind the whole team regarding Life Saving Rules (LSR), applicable to the job and
highlight them in the form by marking the appropriate checkboxes.

Part D • This is a plan for the TBT Leader / Executor to take a pause and check the site safety conditions and to
identify if there is any change in the work area or its surroundings.
• Utilize the space provided in first row to define the schedule for this Pause and Check, as agreed with
your team. The second row is meant to document the actual timings when you actually performed the
Pause and Check.
• It is not mandatory but advisable for jobs which are being carried out for a long duration of time or are
carried out in areas with a lot of parallel activities. As an example, the Executor / Team Leader can
decide with his crew to have a Pause and Check at the area after any work breaks such as tea-time, lunch,
etc. prior resuming the activity. It is particularly useful to assess if there are any changes in the scope of
work or the surroundings that may create additional hazards (such as crew changes, weather conditions,
simultaneous activities in the area etc.)

Part E • Space for acknowledgement by all team members (Name, Emp. No and Sign.)
• Write down all the topics that were discussed in TBT, including questions raised and answers provided.
• Space for additional remarks may be utilized in case there is shortage of space for user inputs in other
part of the TBT form, by providing suitable references.
• TBT Leader: The person who is leading the TBT (say, Permit executor) to provide Name and Sign.
• Supervisor: Supervisor or Permit executor to provide Name and Sign, as the case may be.
