SECRETARY CERTIFICATE

I, ______________________, of legal age, Filipino, with residence address at


________________________________________________, after having been sworn in accordance
with law, hereby depose and say as follows:

1) That I am the duly elected Secretary of BLUE BAY HOMES HOMEOWNERS


ASSOCIATION, INCORPORATED, a homeowners association, with principal office
and place of business at Blue Bay Homes, Pulanglupa, las Piñas City;

2) That I am the custodian of the books and records of the association, including the
Minutes and Resolutions of its Board of Directors;

3) That our homeowners association is a non-stock, non-profit association primary purpose


is to facilitate the ownership of lots and houses or both, by its members and to enhance the
delivery of adequate social services to improve the quality of life and well-being.

3) That at the Regular/Special Meeting of the Board of Directors of the Bank held on April
26, 2022, during which a quorum was present and acted throughout, the following
resolution was unanimously approved:

RESOLVED, that our homeowners association is a non-stock, non-profit association and


therefore request for an exemption of filing the Quarterly Percentage Tax Returns (2551Q)
from the Bureau of Internal Revenue.

RESOLVED, that the undersigned is hereby authorized to process the above request:

“RESOLVED, finally, to confirm and ratify the acts and things done by the majority of the
Board of Trustees pursuant to and in accordance with the foregoing authorities.”
