DISCHARGE PLANNING
Medication Educated the client and the SO about
the name of the medication, why it is important, what it treats, and the needed doses. Advised the SO and the client to put medication in a container, and label them on how and when to take it to avoid medication error. Discussed to SO and the client the side effects of the drug for the patient to be prepared and thus be able to cope. Advised the patient and SO to take whole capsule or tablets When it comes to medication safety, advise the patient and SO to keep it out from direct sunlight.
Exercise Assist the patient with cardiovascular
exercises such as walking, stationary cycling, recumbent cross training, and other cardio exercises to improve heart endurance, maintain healthy lungs, and improve sensory perception and motor skills. Assist the patient with resistance exercises for strengthening muscles such as wrist curls, wrist and hand stretch, shoulder openers, table towel slide, trunk bends, knee rotation, hip abduction, standing knee raise, sit-to- stands, and hip thrusts to help the patient to recover physical strength, stamina, stability, and improve range of motion. Treatment Advised patient to engage in therapies for treatment such as physical therapy to restore mobility. Educate the patient on the importance of participating in occupational therapy. This type of therapy involves exercise and training. Its goal is to help the patient become independent or semi- independent. Hygiene Instructed patient to always maintain a good personal hygiene Instructed SO to assist the patient when on the shower, this is because a person is less likely to fall while getting in and out of the shower. Out Patient Emphasized and instructed the patient on the importance of follow-up checkups and report immediately signs and symptoms of recurring stroke Encouraged the family to help and assist the patient with therapies as his physician ordered. Instructed to seek medical attention right away when experiencing any issue or difficulties Advised patient and SO for emergency medical service if the patient feels worsening - Confusion - Personality Changes - Dizziness - Headache that will not go away after treatment - Forgetfulness Diet Advised patient to reduce amount of fat and cholesterol to eat Advised patient to reduce amount of salt(sodium) in diet, especially if he had high blood pressure Advised patient to to eat more fresh vegetables and fruits Advised patient to lessen red meat and processed meats Advised patient to use low fat dairy products Advised patient to limit vegetable oil and nut oil. Advised patient to avoid eating trans fats that was often found in processed foods, and avoid foods that has hydrogenated listed in its ingredients. Spiritual and Sexual Teaching Encouraged to keep seeking God's guidance and understanding. Emphasized the significance of prayer in healing Encouraged to seek heavenly assistance in all situations Encouraged to maintain a cheerful attitude in life. Encouraged to pursue interests and hobbies for enjoyment in the future as an alternative to sexual desire
