The Rivera Family

Mr. and Mrs. Rivera aged 79 and 71 respectively, are of Mexican heritage and have been raising
their three grandchildren for the past three years. Marianna (age 4), Miguel (age 9), and Rosa
(age 13). The children were placed in kinship care with their grandparents after their mother and
father were incarcerated on drug related charges. Neither of the Rivera’s two other children were
in a position to take on parenting responsibilities. Mr. Rivera is retired from a position with the
local school district, but she supplements their income by cleaning houses.

Approximately two months ago, Mr. Rivera underwent a triple bypass to address underlying
heart disease. After the surgery, he was admitted to a rehab facility, where he was progressing
well, with the expectation that he will be returning home. While at the rehab, Mr. Rivera
developed a staph infection which led to Mr. Rivera developing Sepsis. The rehab center was
able to stabilize Mr. Rivera and eliminate the infection. However, because of the infection, Mr.
Rivera was left with long-term, disabling muscle and joint pain, along with some mild cognitive
impairments. This has left Mrs. Rivera to act as Mr. Rivera’s full-time caregiver. Mr. Rivera
has limited mobility now as a result of his muscle and joint pain and can no longer assist Mrs.
Rivera in handling the family finances, childcare, parenting, and family affairs. It is not clear
how he will be able to manage as the home has two stories, and the only bathroom is on the
second floor.

Prior to Mr. Rivera’s heart surgery, Miguel was beginning to get into trouble at school. At the
beginning of the school year, Miguel had mostly A’s and B’s in all of his classes. However,
since Mr. Rivera’s health has declined, Miguel’s grades have dropped to C’s and D’s. Mrs.
Rivera is getting regular phone calls from the school about Miguel misbehaving in class. Last
week, the school called to say that Miguel had gotten into a fight with another peer during lunch,
over a handball game. He pushed the peer to the ground and then was found crying in the
bathroom but wouldn’t say why. As a result of the fight, Mrs. Rivera had to pick up Miguel early
from school and learned that he would be suspended for one day. Rosa, always a quiet child, has
presented no problems at school and always did well socially. Yet, the last few months, her
teachers have noticed that she is a bit more withdrawn and mentioned that they often see Rosa
eating lunch by herself, away from the other kids.

Mrs. Rivera initially was able to handle things with the same determination and task-oriented
focus that she used to deal with the incarceration of her daughter and son-in-law and transition to
the “parent” role for her grandchildren. She is a practicing Catholic and the priest at her local
parish has been a tremendous support for her during this time of transition and adjustment.
However, Miguel’s continued difficulties and behavioral issues at school has taxed her beyond
her ability to cope. There is a family history of high blood pressure and heart disease on Mrs.
Rivera’s side of the family and at a recent medical appt with her primary physician, the doctor
expressed concern about Mrs. Rivera’s stress level and how it is impacting her health. Mrs.
Rivera has become weepy, had difficulty concentrating and completing daily tasks, and has been
unable to make decisions on her own. She has had difficulty explaining to Marianna and Miguel
what is happening to their grandfather. She has not told Mr. Rivera about Miguel’s school issues
and about Rosa’s withdrawal and mental health concerns.

Mrs. Rivera is called to attend a meeting with the school principal and social worker. When the
social worker asks Mrs. Rivera if she could give them any insight into why Miguel has become
so aggressive, she begins to sob.
