THIS SERVICE AGREEMENT IS MADE THIS DAY MONDAY 11th OF DECEMBER 2023
BETWEEN TERRITT ENGINEERING SERVICES USA AND ROHIM DUARSYAH
MIHARDI
1.0 JOB RESPONSIBILITY: Your job profile or responsibility has been written,
arranged and packaged; it shall be dispatched to your provided address through
courier service.
1.1 JOB LOCATION: Job location is in Territt Engineering Services Texas, USA.
2.0 JOB SITE/JOINING DATE: (Texas, USA) 10th of February 2024.
2.1 BASIC MONTHLY SALARY: $10,400.00 (Ten Thousand and Four Hundred USA
Dollars Only) $2,600.00 paid per week after tax deductions.
3.0 CONTRACT DURATION: Three years (36 Months) and renewable after
contract duration.
Employee shall also be entitled to over-time allowance payment of $70 per hour
if their work time exceeds the official stipulated hours. Salary shall be liable to
Increments with time and employees' official promotions and position in service.
This is with effect from 10th of March 2024.
It is totally the right of the company to procure all working and relocation related
documents/papers for their respective employees but due to company’s past
experiences with previous employees who obviously failed to relocate due to
minus reasons after spending money for their visa and work-permit.
You are required to furnish us with your valid USA Employment Visa Approval
(H-1B Visa)on or before
4.0 LEAVE PERIOD: Employees are entitled to one time leave in four months; the
duration of the leave is one month each. Employee will receive $5,200.00 take
home for each leave period.
5.0. Medical/Health Service Facilities and Insurance Plan for Family: All
expatriates shall be entitled to a comprehensive health care service, which are to
be administered by medical experts on job locations.
After three (3) months of service all expatriates will be entitled to a Health
Service Insurance Plan (HSIP) for family. The Health Service Insurance Plan is
designed to cover only three (3) members of your family. The Health Service
Insurance Plan (HSIP) allowance can also be paid to the expatriates at the rate of
$1,000.00 per family member per month which can be transferred on monthly
basis without being subjected to tax.
5.1 SAFETY & SECURITY: Safety and security of personnel (local/foreign) facilities
on job locations and communities are serious issues, and have been seriously
considered to ensure and assure hitch free operations.
All operations on job location are designed to adhere strictly to the standards of
USA Health and Safety Department (USHSD) and USA Environmental Health
Authority (USEHA) as applicable in the civil engineering, construction &
environmental health sector.
6.0 ADDITIONAL SERVICES: Recreational/Sports facilities shall be made available
at the disposal of all employees with no expenses to be incurred. Laundry and
dry cleaning service will be made available to employee by the quarter’s laundry
and dry cleaning unit on request.
The company may terminate this agreement by giving one month’s written
notice in any of the following circumstances:
(iv)The contractor commits or permits any breach of the terms of this agreement
and fails to remedy the breach within 14 days of receiving notice in writing from
the company requiring the breach to be remedied;
(v)The Contractor dies or becomes unable to perform their duties due to illness
of incapacity;
This agreement supersedes and replaces all previous contracts, written and
verbal, between the parties and constitutes the entire agreement between the
parties.
Neither party may assign their obligations nor rights under this agreement to any
other person in any circumstances whatsoever except with the prior written
consent of Territt Engineering Services USA.
