INTRODUCTION
The Dr. Jose Fabella Memorial Hospital is a recognized institution with its programs and
services accessible to even the most marginalized Filipino family. Historically, it was the first
maternity hospital and Midwifery School in the country, and the first hospital to cater to
prisoners (being housed in the Old Bilibid Prison Complex).

More than the hospital structure however, the trainings, services, methods, and activities
introduced by Dr. Jose Fabella and his team of physicians, formed the building blocks of
practices still carried out today. Minute details such as taking blood pressure, weighing, and
measuring of infants, pelvimetry, dental care, urinalysis, frequent prenatal visits, nutrition,
breastfeeding, well-baby clinics and home visits were all known back in the day as modern
trends in Maternity and Child Hygiene Work (Diaz, 1952).

It is no wonder that its founder, the first Secretary of Health, Dr. Jose Fabella came to be
known as the Father of Social Work (Diaz, 1952) in the country. From his time when
quackery, superstitions, and unhealthy practices due to ignorance were widespread, the
hospital is now a key player in progressively shaping women and children’s healthcare
delivery system. From his vision to reduce the high rate of infant and maternal mortality of
his time, the institution now looks at a new millennium with a bolder vision to firmly
establish itself as the nation’s Center of Excellence in health for women and newborn. The
social responsiveness of DJFMH, then and now, is innate in the institution, inscribed to its
human resources and reflected in the footprints of its course in history.

TRAINING PROGRAM
PREFACE

The Board of Trustees and the Management of Dr. Jose Fabella Memorial Hospital (the
“DJFMH”) are proud to introduce the Medical Technology Internship Training Program
which is open to all Medical Technology students (“Med Tech Interns”) from any colleges or
universities.

OBJECTIVES

General Objectives

1. DJFMH shall provide quality medical technology internship training program to all
Medical Technology Interns.

2. The Interns, at the end of each rotation, shall develop awareness, familiarity and
insight on laboratory procedures and principles including techniques and
methodologies.

Specific Objectives

1. DJFMH shall provide an alternative academic environment through online and limited
face to face learning following the four-day clinical laboratory duty (12 hour-duty;
day shift between 6 AM to 8 PM) and ten-day quarantine schedule during which the
interns shall engage on synchronous/asynchronous virtual internship activities that
will encourage and promote learning, reasoning, and sound judgment in the
profession.

2. DJFMH shall instill a desire for purposeful direction, respect for others, love for
learning and pride in the chosen profession.

THE TRAINING PROGRAM

Each Intern will undergo and experience a work-like laboratory environment in the different
units of the Department of Pathology and Laboratories such as:

1. Clinical Chemistry

2. Clinical Microscopy

3. Microbiology

4. Hematology

5. Blood Banking/Blood Donation

6. Histopathologic Technique & Cytology

7. Immunology & Serology

8. Phlebotomy

NATURE OF ROTATION

Each Intern will get ample time to have a first-hand experience with various laboratory
sections through robust rotation system.

1. Each Intern shall observe the prescribed number of days he/she has to stay in one unit
to enable him/her to learn, understand, and interpret laboratory procedures and
techniques.

2. Each Intern shall receive a thorough orientation by the Section Supervisors


throughout his/her rotation.

3. Visual presentations will be shown to all Interns to strengthen their appreciation and
understanding of the Department of Pathology and Laboratories.

4. Interns will receive lectures/demonstrations in each rotation.

5. Interns will be immersed in learning activities that will encourage them to complete as
part of the grading system.

6. Interns will receive a shifting examination at the end of each rotation as part of their
internship program and grading system.

Interns are graded based on the following:

The internship training is recommended to consist of face-to-face learning following the four-
day clinical laboratory duty (12 hour-duty; day shift between 6 AM to 8 PM) and ten-day
quarantine schedule during which the interns shall engage on synchronous/ asynchronous
virtual internship activities. This is to ensure that the interns acquire the essential skills in the
different laboratory procedures in the attainment of the intended learning outcomes. The
Clinical Instructor shall also monitor the interns during the ten-day quarantine period, on a
daily basis to see that the intern does not exhibit any sign or symptom of COVID-19. In case
the intern will exhibit signs and symptoms of COVID-19, clearance for returning to school
shall use a symptoms-based screening method and assessment of an attending/school

The Clinical Instructor shall submit the accomplished monitoring form to the accredited
clinical laboratory training coordinator prior to the intern’s return to duty. (JMC No.2021-
001)

The accredited clinical laboratories shall be required to implement the following:

● Conduct training on their institutional biohazard, risk and safety policies including
donning and doffing of Personal Protective Equipment (PPE) for the interns and
clinical instructors

● Conduct orientation on Infection Prevention and Control

● Require health declaration and medical check-up at least three (3) days prior to the
start of the internship training

● Implement proper IATF COVID-19 protocols

● Restructure the set-up of the different sections in the clinical laboratory such that only
a limited number of interns/monitors are assigned per room at a given time with strict
observance of the 1.5 meters physical distance

AFFILIATION FEE

An affiliation fee of not more than six hundred pesos (Php 600) per intern per month and no
other fees shall be collected unless a new policy/guideline shall be issued by the Commission.
This fee is applicable to both public and private training institutions.

The affiliation fee shall be disbursed by the HEI to the accredited affiliate training laboratory.
The affiliation fee should be distributed as follows:
a. Hospital 50%
b. Laboratory staff involved in the training program 50%

1. Clinical Chemistry

● Perform Clinical Chemistry procedures using manual and automated


techniques: routine chemistry, lipid, protein, cardiac, liver, kidney profiles
2. Clinical Microscopy and Parasitology

3.1. Orient all Medical Technology interns regarding their role in the Department
of Pathology and Laboratories in coordination with the clinical instructors of
the university or college.

application of knowledge gained from theoretical knowledge.
3.4. Guide interns in reading and application of theoretical knowledge gained from
school to actual practice, i.e., training in microscopic structures (differential
count, clinical microscopy, and microbiology).
3.5. Give quizzes and compute hospital grades.
4. Functions of University/College Clinical Instructors
4.1. Submit a certified list of Medical Technology interns for the internship
training in accordance with the Official QMS-controlled workflow.
4.2. Determine the unit assignment and the rotation of interns specifying the
inclusive dates, locations, and duration of trainings.
4.3. Ensure that all interns shall sign the internship waiver form prior to their
deployment to DJFMH.
4.4. Give periodical quizzes and computes grades.

Section of Clinical Microscopy

● Receptionist

Assists the medical technology staff in the following:


1. Receiving of specimen from the laboratory clerk
2. Matching of specimen and corresponding laboratory request
3. Checking for congruency of data on the laboratory request with the patient’s specimen
4. Assigning of lab number of specimens on the request and logs in the master list.
5. Performing physical examination of specimens and notes the result at the back of
request

● Performer

Assists the medical technology staff in the following:


1. Receiving specimen from the receptionist
2. Transferring of specimen to centrifuge tube and labels accordingly
3. Performing of chemical examination and notes at the back of request
4. Centrifuging of processed specimen

Section of Microbiology

Assists the medical technology staff in the following:


1. Checking and encoding of request and specimen into the master list
2. Doing gram’s stain, AFB stain, and KOH stain under the strict supervision of medical
technology staff
3. Performing biochemical test, coagulase and catalase test and reading results after the
specified time
4. Loading the cultured bottles into VITEK 2 machine; pulling out both negative and
positive bottles and, if positive, subculturing of specimen
5. Doing the susceptibility testing (streaking, dispensing)
6. Logging and filing of laboratory results
7. Preparing materials for autoclaving

Medical Technology Staff on duty.

Assists the medical technology staff in the following:

TRAINING RULES AND REGULATIONS FOR MEDICAL TECHNOLOGY


INTERNS
1. Medical Technology Interns must report on time for duty in the prescribed complete
uniform.
2. The laboratory shall always be kept clean and tidy. All personal and valuable
belongings of the Medical Technology Interns shall be properly kept in the area
provided for them.
3. Broken, lost equipment and supplies belonging to the DJFMH due to the misuse or
negligence by a Medical Technology Intern shall be replaced.
4. No Medical Technology Intern shall leave the laboratory without the permission of
the supervisor.
5. Medical Technology Interns are not allowed to eat outside DJFMH’s premises during
lunch break.
6. Insubordination, vandalism, forgery, negligence, gambling, and drinking alcohol are
not allowed at any time in the DJFMH or its premises.
7. Use of electrical gadgets, such as mobile phones and tablets, is prohibited during the
tour of duty, except in case of emergency and other unavoidable circumstances.
8. Taking of photos is prohibited as per DJFMH Memorandum No. ______________.
9. Telephones and computers are strictly for laboratory personnel use only.
10. Medical Technology Interns are not allowed to entertain visitors inside the laboratory
or along the corridors of the DJFMH.
11. Loitering in sections other than the place of assignment of the Medical Technology
Intern is not allowed.
12. Order and discipline must be observed at all times in the laboratory.

GENERAL RULES (ONLINE ETIQUETTE)

General Rules of Conduct


1. Interns are expected to always behave as professionals.
2. Interns are expected to attend all synchronous meetings (through online platform) as
these are part of their attendance.
3. Interns are expected to be on time during synchronous meetings/sessions.
4. Interns are expected to participate during synchronous activities or recitations and are
expected to respond to Intern Monitor’s questions.
5. Interns are expected to take extra care of their cameras and microphones during the
synchronous meetings/sessions to avoid unnecessary distractions.
6. While DJFMH respects the privacy of each Intern, we nevertheless encourage each
Intern to be ready (by tidying himself/herself and the surroundings) to open his/her
camera as maybe required during synchronous meetings/sessions.
7. Interns are bound by the Code of Ethics of Medical Technologists.

DUTY, TARDINESS AND ABSENCES


1. Duty
a) Punctuality is expected from Medical Technology Interns at all times.
b) Medical Technology Interns shall report for duty at least fifteen (15)
minutes before the start of the duty.
c) “Out-of-post” Interns but still within the DJFMH premises shall be
given an additional two-hour make-up duty; “Out-of-post” Interns and
are found to be outside DJFMH premises shall be given three (3) days
make-up duty.
d) Each Medical Technology Intern shall render thirty-two (32) hours of
duty per week or one thousand six hundred sixty-four (1,664) hours in
a year.

2. Tardiness
a) Tardiness is the failure of the Medical Technology Intern to report for
duty at the scheduled start time or the official time required by
DJFMH.
b) A Medical Technology Intern coming late to duty shall receive the
following sanctions:
1.) Three (3) consecutive tardiness – one (1) day unexcused
absence.
2.) Accumulated sixty (60) minutes – one (1) day make-up duty.
3.) Habitual tardiness – conference with the Clinical Instructor,
parents, or guardian and/or suspension of the Medical
Technology Intern.
3. Absences
a) Absences incurred during internship shall be given the following
sanctions:
1.) For every excused absence – one (1) day make-up duty.
2.) For every unexcused absence – three (3) days make-up duty.
3.) For accumulated absences exceeding twenty percent (20%) of
the total number of internship hours:

● Per section: Repeat section rotation

● Per hospital rotation: Repeat internship

b) Excused absences

The following are considered EXCUSED absences:


1.) Death of immediate member of the family.
2.) Absences due to ill health or other emergency reasons which
are subject to evaluation and approval by the Training Officer.

Absence due to sickness with medical certificate as certified by


the university, college or school physician may be considered
upon approval by the Training Officer.
3.) Inclement weather, transportation strikes, pandemic, and
incidents involving national security and natural calamities.
4. No make-up duty shall be required if the university, college or school is
pulling out an intern officially. A written letter of pull-out addressed to the
Chief of the Laboratory shall be issued by the university, college or school,
and shall be given to DJFMH at least three (3) days prior to pull-out.
5. Medical Technology Interns shall not be excused when they attend to / follow
up requirements that were incurred in the previous academic years or
semesters.
6. No project, gift or cash shall be accepted from a Medical Technology Intern as
completion of the make-up duty. DJFMH discourages such practices.
7. DJFMH shall allow the Medical Technology Interns to have their make-up
duties due to tardiness/absences/demerits at least two (2) months prior to the
end of their rotation.
8. Medical Technology Intern may file a “Leave of Absence” due to unavoidable
circumstances or emergencies at least three (3) days prior to the start of
internship.

BREAKAGES

After appropriate and thorough investigation with due process done, any Medical
Technology Intern proven to be negligent shall be responsible in settling the full
amount of the replacement or repair of the damaged equipment/instrument only after
having eliminated the possibility that the damage has been caused by wear and tear.

POLICY ON ATTENDANCE, ABSENCES, MAKE-UPS AND GRADES

1. Daily prompt attendance is expected from all Medical Technology Interns.


1.1. Exchange of duties shall not be allowed.
2. Interns who are thirty (30) minutes late are allowed to continue their duty, but such
shall be considered in their accumulated tardiness.
3. A medical certificate from the attending physician is required for absences due to
illness.
3.1. Interns who will be absent due to illness one day before or during shifting
examination day must present a medical certificate.
3.2. No other reason aside from illness will be considered valid.
4. Interns with absences incurred for more than a week may continue with the internship
training.
4.1. Necessary make-ups will be required and are computed as one (1) absent is
equivalent to three (3) days make-up.
5. Interns with absences incurred for more than a week/section, which are not due to
illness, will automatically be dropped from the internship training.
6. Interns are not allowed to take up summer classes during their shift at DJFMH.
7. Make-up duties for all absences shall be allowed only at the end of the academic year.
8. Absences incurred due to illness (with medical certificate) will have an equivalent
make-up duty to the number of absences acquired.
9. Absences incurred for any other reason aside from illness will receive a mark of
incomplete (“INC”) at the end of the shift.
10. Average grades between 70-75% shall receive a passing mark (“Passed”) with a final
grade of 75%.
11. Final grades between 65-69% shall require make-up duties for one month at DJFMH.
12. Grades below 65% shall repeat the internship at the DJFMH laboratory or at the
laboratory designated by the university/college.

DESCRIPTION OF ROTATION AND

HEMATOLOGY

Period: 2 weeks

Objective:

To impart thorough knowledge and rationale of the technical procedures performed in the
section.

Brief description of the program:

Medical technology interns are given different assignments regarding Hematology


procedures on a rotation basis. They are taught the precautions and proper handling of
specimens and are guided on how to operate instruments and equipment such as microscopes,
hematology analyzers, and others.

Methods of training:
1. Orientation and familiarization of the technical procedures performed, including
instrumentation.
2. Direct supervision on staining procedures, cell counting, and others.

Skills to be learned:
1. Proper collection of blood and necessary precautions.
2. Hematocrit and hemoglobin determination (manual method).
3. Clotting time (Lee & White Method), bleeding time (Duke’s and Ivy method), and
clot retraction time.
4. White blood cell count & Red blood cell count (manual and automated)

Brief description of the program:

The interns are taught the principles of the procedures and instruments used. They are to
familiarize themselves with the different chemistry analyzers and the preparation of reagents,
working solutions, and control solutions.

Methods of training:
1. Orientation on how to operate instruments and equipment based on the manual
instructions.
2. Actual performance of laboratory procedures under the direct supervision of staff.
3. Skills to be learned: Standard operating procedures, guidelines and working
instructions of the section.
4. Proper collection of specimens with the corresponding precautionary measures.
5. Solving technical problems that may arise in the procedures by pinpointing each cause
among various factors.

To be able to put into practice the proper knowledge on urinalysis and fecalysis by
performing routine and special techniques including microscopic study and concentration
tests.

Brief description of the program:

Familiarization of the proper use of the microscope and procedures done in the examination
of urine and stool, including special tests for the determination of occult blood, semen
analysis, pregnancy test, cell count, and differential count of other body fluids.

Methods of training:
1. Orientation on the proper use of instrument and common procedures of urine
examinations.
2. Observation on the performance of special tests, sperm analysis, and cell count of
body fluids.
3. Application of the principles of the test procedures taught.

Skills to be learned:
1. Identification of urinary sediments and parasitic ova
2. Analysis of calculi
3. Qualitative and quantitative analysis of urine and stool
4. Proper waste disposal

MICROBIOLOGY

Period: 2 weeks

Objectives:

To be able to teach the Medical Technology interns the different methods and procedures of
identifying microorganisms through smears, cultures and microscopic reading. To allow
interns to perform sensitivity testing, media preparation and water analysis.

Brief description of the program:


1. Orientation on the different laboratory safety, universal precautions and specimen
handling.
2. Preparation of smears, staining procedure and microscopic reading for identification
of bacterial and fungal microorganisms.
3. Perform culture on different specimens and select the proper media to be used.
4. Isolate and identify the different predominant microorganisms through the different
media and biochemical reactions.
5. Perform antibiotic sensitivity testing.
6. Observe how to process specimens are processed for MTB culture and how
susceptibility testing is done.
7. Preparation of basic media that are routinely being used and how to operate the
autoclave machine.
8. Processing of water for bacteriological analysis.

Methods of training:
1. Orientation:
The interns are briefed on the proper use of the biosafety cabinet, laboratory safety,
and standard operating procedures being observed in the section.
