1.0 SCOPE

The scope of this document is limited to the procedure involved in the concrete foundation
works required for car parking structures in National Petroleum Construction Company
(NPCC).

The Other remaining works such as block, plastering, painting, JRC, glass curtain walls,
sliding doors, … etc. will be detailed in a separate subsequent method statement.

4.0 ROLES & RESPONSIBILITY

Project Manager: Overall execution and administration of the project as per Contract
Documents, Specifications and the PQP. Liaise with the Head (Quality & HSE) for
preparation and updating of PQP and PHSEP. Directly control the Site Engineers and
Supervisors. He is responsible to ensure that all the correct procedures are followed and all

necessary permits for the works are obtained in advance.

QA/QC Engineer: Responsible for the monitoring and implementing of quality related
matters and ensure the works are being executed with the approved PQP, Method
Statement and ITP.

Health and Safety Officer: Responsible for monitoring and implementation of safety
related matters such as work permits, safety induction, First Aid, PPE, approval of
diversions, follow the safety and traffic regulations by all the workmen during the
construction, according to the approved PHSEP.

Site Engineer: Responsible for managing and overseeing site activities involved in
construction. Ensures technical designs and drawings are being followed correctly. Provides
technical advice and solving problems on site, prepares site reports and other paperwork.
Liaises with Clients, Subcontractors and others involved in the project to ensure health and
safety, policies and construction requirements are adhered to.

Supervisor/Foreman: Plan and obtain required manpower and resources in coordination


with the Site Engineer. Carry out all activities as per the planned schedule to achieve target
dates. Coordinate with respective disciplines of work and liaise with the Site Engineer for
day to day activities. Ensure pre-task briefing is to be conducted prior to start of work at
each session.

Charge Hand: Works shall be executed as per direction of Supervisors/Foremen for all
activities as per the planned schedule to achieve target. Coordinate with respective
disciplines of work and liaise with the Supervisors/Foremen for day to day activities. Ensure
pre-task briefing is conducted prior to start of work at each session.

5.0 INSPECTION & TESTING

Site inspection/approval shall be in accordance with the approved ITP for all activities in
addition to the Quality Control Procedure and PQP.

All the required site test shall be conducted as per the applicable parts of ADM Standard
Specifications.

6.0 WORK PERMITS

All necessary work permits shall be obtained prior to the commencement of any activity at
site and shall remain valid throughout the entire duration of the operation.

Temporary fencing, safety barriers and sign boards will be installed prior to the work
commencement as per the approved logistic plan of project.

7.0 HEALTH, SAFETY AND ENVIRONMENTAL PROCEDURES

Requirements of Health, Safety and Environmental for the project shall be in accordance
with approved PHSEP, the Contract Documents and ADM Standard Specifications.
Before the commencement of any work, the required and applicable work permits shall be
checked and ensure all requirements of Work Procedures are complied with and they are
valid until completion of the activity and as per approved HSE plan.

All personnel accessing the site shall undergo safety induction and wear the mandatory
PPEs. All workmen and staff shall wear the mandatory and job specific PPE. Pre-task
briefing shall be conducted on every day before starting the work. Experienced and HSE
inducted workmen shall be deployed for the work. All personnel shall be cautioned while
working near any live lines such as power cable, water lines, drainage lines, telecom etc.

Prior to commencement of work all workers shall be given pre-task briefing. Hazards
identified for the activity shall be disseminated during pre-task briefing, especially for
underground services, slip & trip, improper access, excavation tools handling, no access
or aggress, vehicle collision, heat exhaustion, which shall be identified and documented
separately by permit to work system as per approved PHSEP.

First aid stations complete with all first aid equipment and trained first aiders shall be
maintained for the initial care.

Safety and security procedures shall be implemented as a minimal, warning signs and
lights, barricades, railing and other safeguards shall be provided as required by the nature
and location of the work. Also, permit-to-work will be obtained for high risk activities during
the project.

8.0 WASTE MANAGEMENT AND HOUSEKEEPING

Housekeeping measures shall be correctly implemented to prevent environment pollution


and hazards with respect to ISO 14001 and ISO 45001. The equipment/tools/materials
required for the work shall be stored /stacked in such a manner so as to give a safe working
atmosphere to the workforce at site.

All workplace areas shall be maintained clear of debris, waste and other rubbish, which
shall be disposed of in segregated containers for disposal. An adequate number of

containers marked appropriate labels for storage and disposal of waste materials shall be
strategically placed throughout the construction areas at all time.

Construction waste will be removed/ disposed from site at regular intervals using drum
truck as per local laws by licensed carrier to a licensed facility.

Any spillages, such as oil or grease shall be immediately cleaned up, by absorption in
inert sand or other suitable materials.

9.0 QUALITY ASSURANCE & CONTROL

The Quality Assurance and Control for the above-mentioned activities shall be exercised
and recorded in accordance with the Contract Documents, ADM Standard Specifications
for Quality Assurance Standards and the approved PQP.

The quality assurance shall be ensured for workmanship, equipment and materials
conformance to the applicable standards and requirements at every stage of the
construction. This shall be monitored by quality control personnel or designated substitute
on the site during the production / operation.
Quality assurance requirements shall also be imposed upon Sub-contractors, Suppliers
and any other parties associated and involved in the project. Non-conforming materials
shall be notified in writing to correct or remove the defective materials from the work site.

All inspections and tests shall be conducted in accordance with approved ITP and PQP.

Applicable documents with latest version such as Inspection & Test Plan, Method
Statements shall be readily available and used by inspection and test personnel at the time
of inspection as referring documents.

10.0 MATERIALS

 All materials to be used shall be of good quality and manufactured by a recognized and
approved international manufacturer and supplier. Certificates of origin and recent test
certificates shall be provided along with material submittals as part of the ITP.
 Material Inspection Request (MIR) will be submitted to Client upon material arrival to site.
 All materials must be stored properly as per the Manufacturer’s recommendations and
ADM Standard Specification. The material shelf life will be monitored as per
Manufacturer’s Data Sheets.

13.0 WORK PROCEDURE FOR CONSTRUCTION

13.1 Site Survey, Level & Setting Out Works

Upon receipt of Job Instruction / Drawing from Client’s Engineer, an initial site visit to be
made along with the Client representative to confirm the site and demarcations. The f
location of building shall be set out and elevations marked for levelling by the Surveyor

using steel pins and paint markings.

13.2 Excavation Works

 The Client shall provide the Contractor with the As-Built Services Layout Drawings to
highlight existence of any services that may be cautioned prior to start with any
excavation activity.
 Manual excavation up to a depth of 1.5 meter using hand shovels along the existing
services line shall be carried out first to locate the mentioned services as per the
provided As-Built Services Drawings.
 If more services other than those mentioned in the provided As-Built Services Layout
Drawings are present, the work area shall be excavated manually in a grid pattern.
 The located services shall be relocated and any existing adjacent services shall be
protected.
 Mechanical excavation using backhoe loader/ excavator shall be carried out up to the
required depth.
 The Contractor shall notify the Client of any other obstruction encountered and shall
provide support, protection or removal of such obstruction as directed by the Company.
 All excavation will conform to the dimensions and elevations indicated on the IFC Shop
Drawings.
 Excavation will extend a sufficient distance from edge of concrete walls to allow for the
placement and removal of concrete formwork.
 The excavated grade shall be properly shaped to the required elevations and parallel to
the required surface.
 All excavated materials shall be stockpiled at convenient location at site and pile shall be
maintained and kept trimmed and so as to cause as little inconvenience as reasonably
possible.
 Excavated materials shall not be left, even temporarily, where it will interfere with other
construction operation.
 Excavation material type shall include:
i. Common Excavation Materials
Any excavated materials including soil and existing concrete, interlock tiles, etc.

ii. Suitable Materials


Any materials excavated from the site or imported from outside that are
conformance with the specifications and approved by the Client for their intended
use.

iii. Unsuitable Materials


Materials other than suitable materials and shall among others include:
o Particles in excess of 75mm in size
o Peat, Logs, stumps and perishable materials
o Materials which are contaminated or susceptible to spontaneous combustion
o Material with undefined properties
o Material with high salt content >1%
o Material with high moisture content greater than with maximum specified

o Building rubber and domestic and industrial wastes
o Soil and rock susceptible to deterioration/change of their properties
o Gypsum and dredged materials

 If any, for deep excavations more than 1.50m or average man height depth with loose
soil, sides shall be protected by means of temporary shoring and strutting.
 On reaching the required level of excavation, the bottom of the excavation shall be
cleared out, trimmed, watered & compacted to 95% modified proctor dry density as per
ASTM D-1557 using the plate/double drum compactor.
 All excavated areas shall be properly barricaded and appropriate signs shall be provided
as required. Unauthorized entry in excavation areas will not be permitted.
 Excavation will involve removal of the necessary external items such as street lighting,
bollard, boundary walls, concrete footing, precast concrete base, precast concrete post,
horizontal and vertical bars, pavers in walkways, grass, shrubs and trees, etc. and these
will be stored in the allocated storage space with the necessary protection.

13.3 Dewatering Works

 If the site conditions require the use of dewatering, then this shall be further discussed
and agreed.
 Based on the above condition, the procedure for dewatering will be mentioned in detail
accordingly.

13.4 Pervious Backfill

 Ensure the Pervious Backfill is wrapped on all sides with geotextiles.


 The Pervious Backfill shall be 300mm as per project specification.
 Place Pervious Backfill on prepared subgrade without wrinkles or folds; and overlap adjacent
rolls using manufacturers overlap requirements.
 Check for damaged geotextile or geogrid before placing Pervious Backfill. Repair
following manufacturer’s recommendations

13.5 Backfilling and Compaction Works

 Backfilling material shall be as per ADM Standard Specification.


 Areas underneath where the RCC foundation are to be casted may be backfilled, as
required, with specific layer using suitable imported material, such as sub-base.
 After the construction of the RCC foundations, the backfilling shall be using the suitable
excavated material around foundation in layers not exceeding 200mm and compacted to
95% modified proctor dry density as per ASTM D-1557 using the plate/ double drum
compactor.

13.6 External Hard and Soft Landscaping Works

13.6.1 Kerbstone Installation Works

 Demarcation, colors, finishing, shape & sizes of the kerbstone shall be carried out as
per approved shop drawings, material submittal approval and in line with existing
kerbstone designs.
 Prior to start of kerbstone installation, concrete blinding shall be constructed with laid
polythene sheets as per approved shop drawing.
 The kerbstone shall be fixed to the blinding using mortar.
 Finally, Concrete hunching shall be constructed to secure the kerbstones in place as
per approved Shop drawings.

13.6.2 Footpath Interlock Installation Works

 Colors, finishing, shape & sizes of tiles shall be as per approved shop drawings,
material submittal approval and in line with existing interlock tiles designs
 Prior to installation of Interlock tiles, laying and leveling of black sand with polythene
sheets shall be carried out as per approved shop drawing.
 Interlock cut pieces shall be cut using tiles cutting machine.
 Finally, fine sand shall be laid on the interlocks to fill the gaps and joints & compacted
using rubber-plate compactor.

13.6.3 Softscape Works

 Existing softscapes comprising of trees, grass and shrubs on the construction area will
be removed and properly disposed of in assigned storage location.
 The current landscape beside the construction area shall be leveled to construction
formation level. Existing trees in this area shall be protected from damages during
construction.
 Tree pits shall be constructed in designated areas as per approved Shop drawings.
 New trees and grass in line with Contract Documents shall be planted as per approved
Shop drawings.

13.7 Steel Reinforcement and Concrete Works

The bottom of excavation shall be compacted and offered for inspection and testing by
the Client’s Engineer.

Upon approval of above activity, a blinding concrete with certain type and thickness shall be
laid to the required levels and size, supplied from approved ready-mix plant manufacturer as
per approved Shop drawings.

13.7.1 Steel Reinforcement Work

 Bar bends and tolerances shall be in accordance with the specifications and fabricated
to the dimensions and configuration shown on the IFC shop drawings, using the steel
bars cutting and bending machine.
 Reinforcement shall be accurately placed in accordance with the IFC Shop drawings
and shall be adequately secured, wired and held in position. The tie wire ends shall be
bent so that they do not protrude into the cover zone.
 Reinforcement shall be supported on a concrete spacer block of a size to give the
correct cover to the reinforcement. Concrete spacer block shall be made of the same
materials, to the same specification.
 Chairs made of reinforcement shall be used to support the top mats of reinforcement
and they shall be so dimensioned as to be stable during concreting operations. The
chairs shall be wired to the bottom mats of reinforcement supported on concrete
spacer block as described above.
 Bar lap splices shall be accurately placed in accordance with the IFC Shop drawings
and shall be adequately secured, wired and held in position.
 All reinforcement shall be placed on timber and covered with tarpaulin sheet. Different
types & sizes will be kept separately. All reinforcement will be kept to protect from
damage, free from dirt, loose mill scale, rust scale paint, oil, or other foreign
substances.
 Steel Reinforcement Bar Bending Schedule & Shop drawings for each individual
structure will be prepared and submitted by the Contractor for Client approval.

13.7.2 Form work


 The design and construction of the formwork shall be in accordance with the IFC Shop
Drawings.
 Formwork shall be constructed of plywood.
 Internal form ties shall be of such a nature that no permanent metallic part has less
cover than the reinforcement and shall be constructed so that the removal of the ends
or end fasteners can be accomplished effortlessly.
 Formwork shall be coated with a release agent before the reinforcement is placed.

13.7.3 Placing of Concrete

 The method for placing of concrete will be worked out to prevent segregation of the
material & Client Engineer’s approval will be taken before concreting begins.
 Before concreting commences, the forms shall be thoroughly cleaned and freed from
all debris using the air compressor.
 In continuing concrete, fresh concrete will be placed before the already placed
concrete is less than 20 minutes.
 Concrete of specified grade will be placed & compacted in horizontal layers normally
not exceeding 300mm in depth.
 Concrete will not be dropped from a height greater than 1.5m to prevent
segregation. Concrete will be free of all rock pockets, honey combs & voids.
 Complete Records showing the details of placing concrete in each part of the work

will be maintained and will be available for inspection at the site.

13.7.4 Compaction of Concrete

Mechanical vibrators will be used for compaction. For this, stand-by units will available in
the site during the period of compacting. All operators handling vibrators will be trained in
their operation properly.

13.7.5 Curing
Continuous curing will be carried out in a moist atmosphere for a minimum period of 10
days.

Concrete area will be kept covered with hessian clothes or polyethylene sheets, which will
be kept moist throughout the curing period.

All formwork shall be removed after concrete has attained sufficient strength and without
damage to concrete.

13.7.6 Concrete Tests

 All necessary arrangements for the sampling & testing of fresh & harden concrete in
accordance with the BS 1881 will be taken.
 Frequent slump test will be carried out in accordance with BS 1881 on samples of
concrete taken immediately before placing to determine the consistency of concrete.
 Compressive strength test will be carried out on concrete cubes formed in 150mm
molds. One set of cubes (3 cubes crushed in 7 days & other 3 in 28 days) will be done
to maintain the quality of work.
 Particular care will be taken to ensure that the test cubes are stored under uniform
conditions in the site & during any transit between site & laboratory.
 All the test reports will be maintained in the site office & will be available for the
inspection whenever required.

13.8 Waterproofing Works

 Ensure that all preceding activities or activities completed which cannot be accessed
after waterproofing works are inspected, released and approved.
 If any, obtain clearance to proceed in areas which may not be accessible after
waterproofing, prior to commencing waterproofing works.
 Rectify non-conforming conditions prior to commencing the waterproofing works.
 Ensure only appropriate waterproofing materials are used.
 Ensure all surfaces to receive waterproofing fluid Application (two coats of rubberized
bitumen) firmly to achieve good adhesion and to be in one continuous operation.
 All application shall be according to manufacturer’s instructions and approved Shop
drawings.

14.0 HEALTH & SAFETY

Refer to the site safety policy for general safety controls. One of the main aspects of this
procedure is Safety and Health Control. This is to control occurrence of injuries as well as
damage to equipment and properties during the entire construction period.

Induction trainings will be conducted for every worker and visitor before they are allowed into
the work area.

Tool Box meetings shall be conducted every day before the start of the work.

Experienced workmen only shall be deployed for the work

Hazard identification shall be done during toolbox meetings especially for heat stress.

Strictly adhere to the control measures for the hazards identified in the approved risk
assessment to reduce risks as low as reasonably possible.

In order to ensure a safe working environment during the entire construction period, the
following safety measures will be implemented:

14.0 Machinery and Equipment:

Prior to use any machinery/equipment at site, it shall be inspected and certified by


qualified person.

Periodic Preventive Maintenance shall be made on all equipment and machineries.

All operators and driver are to be equipped with valid driver’s/operator’s license and
competency certificate from authorized third agency.

Only authorized drivers or operators will be allowed to operate machineries or


equipment. Only authorized signalman shall be assigned together with the operator or
driver.

All lifting equipment as well as its accessories such as sling wire, nylon sling,
shackles, chain blocks, crane hook and latch shall be inspected periodically.

14.1 Personal Protective Equipment:

PPE shall be issued to all workers such as safety shoes, hard hat, working gloves, hi-vis
vests, ear plugs, safety goggles and safety belt and/or safety harness for workers who will
be assigned in elevated places.
Wearing of the basic PPE shall be enforced fully.
