    TEMPORARY POWER CONNECTION AGREEMENT

    I,________________________________________, with a residence address at


    _______________________________________________________________, referred to as the “Owner” hereby allows and
    authorizes LBS DIGITAL INFRASTRUCTURE CORP., a corporation organized and existing under Philippine laws,
    with office address at Unit 7-1, Three Neo Building, 3rd Avenue Corner 30th Street, Bonifacio Global City, Taguig, to
    connect to the power supply for its telecommunication facility located at Barangay Vista Alegre, Ilog, Negros
    Occidental to the source of Owner’s electric service being supplied by PALECO with meter number ____________ and
    sub-meter number _____________________.

    Both parties agreed on the following guidelines:

    ▪ LDIC shall ensure that the continuous and uninterrupted power supply from the common electrical
    connection to SMART facility until SMART or LDIC establishes its permanent power supply from
    utility Service Provider PALECO.

    ▪ LDIC may cut-off the power supply, and/or terminate this power supply agreement without good
    cause and notice to the First Party.

    ▪ LDIC shall be responsible to resume the power supply at its best efforts when the utility PALECO
    become unavailable for any reasons.

    ▪ LDIC shall pay its consumption based on sub-meter reading and prevailing per unit rate as stated
    on or calculated from the official recipients issued by BENECO to the ultimate power owner.

    ▪ Monthly power billing is immediately forwarded to the LDIC upon receipt from the utility Service
    Provider by email at powerteam@lbstowers.com thru given address below:

    Further, LDIC shall:

    ▪ Pay monthly billings for the power consumption of the above sub-meter starting from the time
    SMART connects to the power meter of the utility service provider.

    ▪ Cause the immediate processing of the monthly Statement of Account upon receipt thereof from the
    utility service provider, PALECO.

    No other charges or expenses shall be payable by SMART, except the power consumption recorded in the above sub-
    meter.
