RESOLUTION NO. 06 Series of 2023

AUTHORIZATION FOR SK CHAIRPERSON HON. SHAINE CARIZ M. SALAMAT


AND SK TREASURER MS. IRISH NICOLE A. PITARGUE TO OBTAIN OFFICIAL SK
TIN ID AND OPEN THEIR OWN SK BANK ACCOUNT.

WHEREAS, Acknowledging the importance of an organized process to enable the acquisition of


the Tax Identification Number (TIN) for the SANGGUNIANG KABATAAN OF BANGYAS;

WHEREAS, Recognizing that a designated person must supervise the gathering and submission
of the issuing paperwork in order to ensure the smooth operation of SK operation;

WHEREAS, All members of the Sangguniang Kabataan unanimously agree that SK Treasurer
Irish Nicole A. Pitargue and SK Chairperson Hon. Shaine Cariz M. Salamat are trustworthy and
able to take on the responsibility of obtaining a bank account and TIN number for the benefit of
the entire Sangguniang Kabataan in Barangay Bangyas.

WHEREAS, The Sangguniang Kabataan of Bangyas is responsible for the programs that they
plan the bank account and TIN number are used in transactions that include money transfers.

NOW THEREFORE, On motion of SK Councilor Katryn Ramos duly seconded by SK


Councilor Desiree Joy Requitud;

RESOLVED, As it herby approved the Authorization for SK Chairperson Hon. Shaine Cariz M.
Salamat and SK Treasurer Irish Nicole A. Pitargue;

RESOLVED FINALLY, The copy of this Resolution shall be given to Hon. Shaine Cariz M.
Salamat SK Chairperson and all Sk members for their file copy.

APPROVED, This 18th day of December 2023 at the Brgy. Bangyas Hall, Barangay Bangyas
Calauan, Laguna.
