“AN ORDINANCE ENACTED THE ISSUANCE OF BARANGAY IDENTIFICATION


(ID) CARD TO BONAFIDE RESIDENTS OF BARANGAY ALAMINOS, MADALAG,
AKLAN.”

BE IT ORDAINED, by the Sangguniang Barangay of Barangay Alaminos, Municipality


of Madalag, Province of Aklan in session assembled that;

Section 1. Title – This ordinance shall be known as the “Barangay Identification (ID)
Card Ordinance.”

Section 2. Scope- this ordinance shall govern the use of the Barangay Identification
(ID) Card of bonafide residents of Barangay Alaminos, Municipality of Madalag,
Province of Aklan.

Section 3. Legal Basis- Section 384,385, 386, of Chapter 1 Title Book 111 if the
Republic Act 7160 also known as the Local Government Code of 1991.

Section 4. The Designed – A card of a rectangular in shape measuring fifty-two (52)


millimeters by eighty-five (85) millimeters with the following information in front or right
side of it; Heading and Barangay Telephone number with in scripted BARANGAY ID,
Picture of Holder, Name and Signature of Barangay Secretary and Name and Signature
Punong Barangay. At the back Card Owner’s Signature, Address, Birthdate, Sex,
Height Weight, Expiry Date, Name of Contact person and Contact Number in Case of
Emergency. The ID is sealed with Barangay Official Seal.
Section 5. It’s Uses- the Barangay ID shall be issued to the residents of the Barangay
who has resides in the barangay for at least six (6) months and authorizing the holder to
use it as him/her identification for whatever legal purpose it may serve best. The
Identification Card (ID) will have a one (1) years effectivity from the date of Issue.

Section 6. Charges/Fees – a Qualified resident who would like to avail for the Barangay
ID Card shall have fee the amount of Fifty Pesos (P 50.00) as payment for the card
reproduction cost.

Section 7. Effectivity- This Ordinance shall take effect immediately.
