Regular health checkups are an important part of
individual, families and community health.
- Taking proper care of health at the right time can
prevent a lot of problems in the future.
- Appropriate screening should be done at the right time
by regular checkups.
- A check up every 6 months is appropriate.
- Regular health services, screenings, counselling and
treatments, give chances for living a longer, healthier
life.
FACTORS FOR SEEKING HEALTH SERVICES

RISK SCREENING: To identify high risk individuals,


assessing medical and social history of the family.

INTERVAL HISTORY: Changes in the medical, social


and emotional status are documented.

HEIGHT AND WEIGHT: Required for all preventive


health care visits.
VITAL SIGNS: Pulse and blood pressure required for all
preventive health care visits.

PHYSICAL EXAMINATION: Appropriate evaluation for


inclusion in the baseline physical examination.

VISUAL ACUITY TESTING: Document the patient’s


ability to see at twenty feet and referrals.

HEARING SCREENING: Test or inquire about hearing


periodically or once a year.
FAMILY PLANNING SERVICES: Planning, referral,
education, counselling, initial examination, diagnostic
procedures, routine laboratory services, contraceptive drugs
and supplies and follow up care.

MATERNITY CARE AND SERVICES: Pregnancy


testing and a nursing assessment with referrals to a
physician, physician’s assistant or nurse practitioner for
comprehensive evaluation, case management, referral and
follow up and visits as prescribed.
NUTRITION ASSESSMENT AND COUNSELLING:
Provision of safe and adequate nutrition for infants by the
promotion of breastfeeding and by proper use of breast
milk substitutes, individualized diet counselling and a
nutrition care plan.

EMERGENCY SERVICES: Emergency services like


heart attack, loss of breath, blood loss, loss of
consciousness, poisoning, etc., by 24*7 at First Referral
Unit/ Community Health Centre and district hospital. 
CHILD HEALTH CHECK UP: Assessment of past
medical history, developmental history, behavioural health
status, physical examination, immunization, dental and
vision screening, diagnosis and treatment, referral and
follow up.
IMMUNIZATION

-Vaccines are available for many of the communicable


diseases.

- It is essential for all children in a community to receive


full course of immunization as disease may cause an
outbreak in the community.
BARRIERS:

- Family may afraid that the baby may cry the whole day.
Painful, reactions may occur, suspicious on needles used
and no time to visit clinics.
WAY TO OVERCOME BARREIERS

- Conduct meeting with village leaders and mothers.


- Decide upon convenient time to conduct immunization
clinics.
- Provide effective health education.
- Provide information on clinic and outreach services.
- Follow principles of immunization.
- Community health nurse should have adequate training
to administer vaccine.
COUNSELLING

Counselling is an interactive process which helps to provide


a solution to an individual with health problems.
- Counselling can be provided on an one to one basis or in
groups.
- Community health counselling can assist with issues like
relationship breakdown, work related stress, coping with
chronic illness, depression, anxiety and related
conditions, to explore sexual identity, etc.,
-Community health nurse detect and address the health
needs of the community members.

COUNSELLING SERVICES THROUGH HEALTH CARE


ANTENATAL MOTHER: Counselling on diet, rest,
tobacco cessation, dangers of exposure to second hand
smoke, minor problems during pregnancy, advice on
institutional deliveries, pre birth preparedness, etc.
POSTNATAL MOTHER:

Counselling on diet, exclusive breastfeeding, hygiene, rest,


immunization, contraception, prevention of STI/RTI, family
planning methods, safe abortion services, etc.
OTHER SERVICES:

- Counselling for leprosy patients for regulatory and


completion of treatment and prevention of disability.
- Symptom based care and counselling with referral if
needed for RTI / STI/ HIV/ AIDS.
- Identify person with disabilities/
- Counselling on harmful effects of tobacco and alcohol.
DIAGNOSIS

Community diagnosis generally refers to the identification


and quantification of health problems in a community as a
whole in terms of mortality and morbidity rates, ratios and
identification of their correlates for the purpose of defining
those at risk or those in need of health care. Early detection
of health impairment is defined as “the detection of
disturbances of homeostatic and compensatory mechanism
while biochemical, morphological and functional changes
are still reversible”. - WHO Expert Committee
GOALS OF COMMUNITY DIAGNOSIS:

- To analyze the health status of the community.


- To evaluate the health resources, services and systems of care
within the community.
- To assess the attitudes toward community health services and
issues.
- To identify priorities, establish goals and determine courses of
action to improve the health status of the community.
- To establish an epidemiologic baseline for measuring
improvement overtime.
TREATMENT:

- Every CHC should have the standard operating procedures and


standard treatment protocols for common ailments and National
Health Programmes.
- CHCs are to provide diagnosis and treatment facilities for
routine and complicated cases of malaria, filaria, dengue, etc.,
- All the DOTS providers to deliver treatment as per guidelines.
- 24 hour services for normal and assisted deliveries, caesarean
section, newborn care, emergency care of sick children, blood
storage facilities, transport facilities, etc., should be available at
CHC / FRU.
FOLLOW UP:

Follow up is an act of renewing contact with sources of information


and data needed to reinforce or evaluate a previous action or report
such as re examination of an earlier diagnosis or prognosis.
FACTORS TO PLAN A FOLLOW UP VISIT:

- Current health status – signs of progression.


- Presence of any serious signs and symptoms.
- Home environment.
- Self care abilities.
- Level of independence.
- Level of nursing care needed.
- Mental status.
- Level of adherence
ROLE OF COMMUNITY HEALTH NURSE
;
-Community health nurses should actively engaged in early
diagnosis and treatment.
- Provide health information during home visits as well as mass
campaigns.
- Periodical mass education programmes.
- Holding meetings with community leaders, women self help
groups and mahila mandals. 
- Coordinate with NGOs activities that focus on secondary level
interventions.
- Health educating couples at the same time.
- Teaching on breast self examination and testicular self
examination.
- Motivating men and women to seek health care when they have
some problems. 
- Stress on the importance of compliance on treatment prescribed.
- Arrange specific health camps based on the need.
- Observing and health educating the patients in follow up
measures.
- For infected patients follow up is considered until they no
longer a threat to communicate disease to others.
