Medical Records

You may access your healthcare records online through our patient portal, UK HealthCare MyChart, starting June 5, 2021.

After you register for the MyChart patient portal, you can:

  • Request prescription renewals when it’s convenient for you, not just when our offices are open.
  • Request, cancel or reschedule appointments.
  • View lab test results, radiology reports and office visit summaries.
  • Get health maintenance reminders.
  • View your immunization record and allergies.
  • View your hospital discharge instructions.

You can also use your MyChart account to share your records electronically with another provider. For more information, refer to the "Medical Record Access and Sharing" section of the MyChart Patient Quick Start Guide.

*Note that records available through the portal are not necessarily historically complete. Records included extend as far back in time as the electronic health record used by the particular clinic, usually 1-2 years.

How to get printed copies of your records

Your doctor may request your records. The doctor's office can call 859-323-5117 to request your medical records. We will fax the requested records directly to the doctor the day before or the day of the appointment. 

You may print the authorization form and mail or fax it. Complete the PDF form found at the link below: 

You may fax the completed form to 859-218-7658 or mail it to a Health Information Management office listed below.

You may request your records by filling out an authorization form in person at our new off-campus location, 2333 Alumni Park Plaza, Suite 110, Lexington, KY 40517, or our campus location on the 1st floor of Chandler (PAV H), next to the registration desk.

You may call for the form to get your records. Call us at 859-323-5117. An authorization form and a pre-addressed, stamped envelope will be sent to your address. Complete the authorization form, then either mail back using the addresses below or fax the form to the medical records department at 859-218-7658.

Attestations required for requests by the following groups (Effective on and after Dec. 23, 2024

Due to new federal regulations, if you are not the patient or the patient’s personal representative and you are:

  • Seeking records for use in administrative or judicial proceedings;
  • Seeking records for a law enforcement purpose;
  • Seeking records for a health oversight purpose; or
  • Seeking records to fulfill coroner or medical examiner duties,

then you will need to complete the attestation below in addition to presenting paperwork previously required.

The attestation on the Office for Civil Rights’ website is also acceptable.

Costs

The first copy of your records is free. A standard fee of $1.00 per page will be charged for additional copies. Please keep a personal copy of any information you request to avoid paying for your records.

At your request, your medical records will be released to any doctor free of charge.
