21.1 DEFINITION

Is a term used by healthcare institutions referring patient leaving the hospital


before the physician recommends discharge.

21.2 OBJECTIVES
1. To protect the physician and members of the health care team for legal
issues.
2. To protect against liability.
3. To protect the third party( ex. Insurance company).

21.3 POLICY
1. Patient electing to discharge against medial advice shall sign a (DAMA)
form.
2. The attending physician must be notified.
3. No medications will be given unless allowed or prescribed by the physician.
4. If the patient is for payment, notify the reception for proper evaluation of
payment, documents and clearances. Secure all the necessary documents that
is found missing in the file prior to sending patient( ID, passport, police
clearance, contact number, consents). Provide a photocopy for each.
5. A guardian is needed for minor patients.
6. Inform all departments who is involved in the care of the patient(ex. Dietary,
physiotherapy).

21.4 PROCEDURE
1. Unit nurse notifies the Attending Physician and the Nursing Supervisor
of the decision for DAMA.
2. Nursing Supervisor and Physician explain through Social Worker Office
regarding patient safety and legal purpose and explain the possible
complications of decisions.
3. The nurse on duty will notify the in-charge reception for signing of
DAMA.
4. If the patient is unable to sign consent for some reason, a thumb mark
will be taken or a relative can sign in his behalf.
