    Dear Nurse,
    Re: Ms. Ann Ballard

    I am writing to refer Ms. Ballard, a 22-months-old infant, who was accidentally scald of hot water that
    result of 2nd degree burn on her right trunk and arm. She is now ready for discharge today and she needs
    for follow-up.

    Ms. Ballard lives with her mother, together with her 4-year-old brother. Her mother, Mrs. Christine
    Ballard was feeling depressed due to financial problem and separation from her husband months ago.
    They spend with their father during weekends alternately.

    Prior to discharge, she was now managed by oral fluids for hydration and prophylactic antibiotic for
    wound cover. She requires daily Silvazine dressing on her affected site. Her mother needs to monitor her
    mental condition. Kindly introduce Mrs. Ballard to mothers’ support group and to financial counsellor
    for financial crisis.
