I am a nurse writing to request home visit services for Ms Margaret, who will be discharged
tomorrow. She requires your esteemed care and management following angioplasty procedure.

On 15/07/2017, today, Ms Margaret underwent angioplasty owing to CAD with symptoms of


angina. Post-operatively, she is able to tolerate fluids and light diet. Area around the catheter
insertion has shown bruises. Ms Margaret is reluctant to return home, however, we are not
certain about her coping ability.

Regarding her medical information, she has been diagnosed with coeliac disease, hypertension,
hypercholesterolemia, and these have been managed with metoprolol, ramipril, atorvastatin and
diet. She belongs to the overweight category related to sedentary lifestyle.

While your visits, it's crucial to monitor post-operative medications, aspirin and clopidogrel along
with light exercises as recommended by physiotherapist and diet by dietitian provide gluten free
diet and plenty of fluids. Kindly note, observe for bruises or infection around the wound site.

We suppose, Ms Margaret has risk of social isolation so It would be greatly appreciated if you
could facilitate regular family visits to monitor her.
