Dear Doctor,
Thank you for accepting Mrs Olsen, a 90-year-old woman, into your care for urgent admission and
observation (for undiagnosed symptoms of chest pain and nausea). She is set to be discharged today
(do you mean transferred by ambulance immediately)?
On 29/06/12, Mrs Olsen started complaining about indigestion pain every time she ate, so Mylanta
was added to her daily medication to calm her pain down. However, her symptoms worsened and
for the past few days, she has been suffering from severe pain in her shoulder, neck and chest
causing her insomnia. Therefore, painkillers like Normison and Panadol were commenced (were
given as needed) in order to ease her pain. Despite that, she is still unable to sleep causing her a lot
of distress.
Regarding Mrs Olsen’s medical history, she has been diagnosed with had hypothyroidism since 2000
and she has had/suffered from hypertension and glaucoma for several years. For this, 150mg of
Karvea, 0.1 mg of Oroxine and 10 mg of Normison per day were prescribed (are taken) as well as
one drop of Timoptol in each eye every morning, and before going to bed. Apart from this, she can
ambulate independently with a walking stick. OR: she was diagnosed with hypothyroidism in 2000.
In view of the above, it would be greatly appreciated if you could examine Mrs Olsen and get a
proper diagnosis of her symptoms. Note that, she is allergic to penicillin and that her son, Nicholas
Olsen, requires to be kept up to date about his mother’s condition. If you require any further
