Undertake physical activity and physical fitness assessments.
ACTIVITY 1: HR LOG: Below is an HR log, a template that allows you to self-assess and report your
heart rate before and after you perform a physical activity, the time you spent, and your Rate of Perceived
Exertion (RPE) of the physical activities you do in a daily basis. Reflect on the physical activities you did
today and fill in the needed data in the given template.

Note: Accomplish your HR Log before and after your scheduled daily activities with the assistance
of a family member.
Friendly Reminder: Make sure that you are physically healthy/fit before doing the activity.
Monitor the time spent during the duration of the activity.
Remember:
HR (pre) -your heart rate before doing the activity (count your heart rate in 10 seconds then
multiply the counted heart rate by 6.)
HR (post) -your heart rate after doing the activity (count your heart rate in 10 seconds then multiply
the counted heart rate by 6.
After the warm-up exercises, use the following rating scale to assess your performance:

Movements
A. Locomotor Movements – movements that allow you to move from one point to another.
• Walk – series of steps executed by both of your feet alternately in any direction.
• Run – series of walk executed quickly in any direction wherein only one-foot stays on the
ground while the other is off the ground.
• Jump – both feet lose its contact with the ground. The most common is a two-foot take- off
and two-foot landing.
• Hop – requires a push off from one foot and landing on the same foot.
B. Non-Locomotor Movement – movements that are performed in one point in space without
transferring to another point.
• Twist – a partial rotation of the body parts around an axis.
• Swing – a perpendicular motion of the body part that can move forward and backward or
side to side.
