    ISSUANCE OF DANGEROUS DRUGS LICENSE (S2-LICENSE MEDICAL PRACTITIONER)

    Schedule: Monday to Friday – 8:00AM to 5:00PM

    WHO IS QUALIFIED TO APPLY FOR S2-LICENSE? A PRC- registered Physician, Dentist and Veterinarian who will prescribe [a] Dangerous Drugs Preparations (DDPs) or [b] Drug
    Preparations containing Table 1 controlled chemical except drug preparation/s containing PPA at doses of 25 mg or below OTC drugs, per BFAD AO 163, S. 2000.

    WHO IS QUALIFIED TO APPLY FOR S2-LICENSE ONLINE? Renewal S2-licensed applicant, i.e.Physician, Dentist, and Veterinarian who initially applied in person at PDEA office and
    whose information were entered into the PDEA database.

    WHAT ARE THE REQUIREMENTS?

    1. DULY ACCOMPLISHED S2 LICENSE APPLICATION FORM (FM-CSVlrd-O1)


    2. ORIGINAL AND PHOTOCOPY OF VALID PRC ID CARD
    3. ORIGINAL AND PHOTOCOPY OF VALID PROFESSIONAL TAX RECEIPT (PTR).
    IN LIEU OF PTR, GOVERNMENT EMPLOYED PRACTITIONERS TO SUBMIT, CERTIFICATE OF EMPLOYMENT AND ORIGINAL COPY OF NOTARIZED AFFIDAVIT ATTESTING USE OF
    S2 LICENSE THAT WILL BE GRANTED SHALL BE FOR GOVERNMENT PRACTICE ONLY.
    4. ONE-TIME SUBMISSION OF TAX IDENTIFICATION NUMBER / BIR TIN VERIFICATION SLIP / E-TIN / LATEST INCOME TAX RETURN (ITR)
    5. VALID DRUG TEST RESULT (DOH-DDB IDTOMIS-GENERATED REPORT)
    6. ONE PIECE 2” X 2” ID PICTURE WITH WHITE BACKGROUND TAKEN AT LEAST 6 MONTHS FROM APPLICATION. NO EYEGLASSES.

    HOW TO APPLY FOR S-2 LICENSE?

    STEP APPLICANT / CLIENT ACTIVITY DURATION PERSON IN FEES FORM


    CHARGE
    1 Submit completely filled- out and signed Receive and check application form and requirements. Encode 10 minutes DDRO I - FM-CSVlrd-

    Verify printed S2 License Certificate. DDRO II
    4 Receive S-2 License Certificate. Release S2- License Certificate. Log issuance / release. File DDRO I - -
    application.
    (Due to centralized database, issuance

    NOTIFY PDEA IN WRITING AT LEAST 60 DAYS IN ADVANCE FOR AN INTENTION TO DISCONTINUE/RETIRE THE S2 LICENSE AUTHORITY GRANTED.
    WRITTEN NOTIFICATION ON LOSS OF LICENSE WITHIN 48 HOURS FROM OCCURRENCE TO PDEA COMPLIANCE SERVICE/REGIONAL UNIT.
    ADDITIONALLY SUBMIT NOTARIZED AFFIDAVIT OF LOSS AND POLICE BLOTTER.
    RE-APPLICATION FOR A NEW LICENSE AND PAYMENT OF CORRESPONDING FEES.
    A DANGEROUS DRUG PREPARATION IS PRESCRIBED IN A SPECIAL PRESCRIPTION FORM FOR DANGEROUS DRUGS WITH S2 LICENSE INDICATED THEREIN
    UNLESS OTHERWISE EXEMPTED BY A REGULATION.

    HOW TO APPLY S2-LICENSE RENEWAL?


    Same steps as above.
    Reminders:
    1. Renew license prior to expiration and immediately upon renewal of PRC License.
    2. Renewal through representative is allowed upon submission of an authorization letter, original and photocopy of representative’s valid ID.

    HOW TO APPLY ONLINE FOR S2-LICENSE?

    This 'S2 online' refers only to the basic online entry of information in the S2 application form. Such information is retrieved and processed only upon receipt of the client’s
    requirements (hardcopy) and Official Receipt or payment sent thru courier or authorized representative. Said S2 online is primarily designed for S2 offsite purposes and PDEA’s
    Regional Dangerous Drug Regulation Officers who transmit bulk S2 application documents to the PDEA National Office. With the online retrieval, the PDEA National Office need not
    enter the client's information and hence saves valuable time. The 'S2 online' however, is opened and made available to the public who may want to make use of the system despite
    its limitations.

    STEP APPLICANT / CLIENT ACTIVITY DURATION PERSON IN FEES FORM


    CHARGE
    1 Visit PDEA website: pdea.gov.ph. - -
    Click ‘S2 Application’.
    Click ‘Apply Now’ button.
    Login, i.e. enter username and password. If
    unable to recall, email cs@pdea.gov.ph.
    2 Enter complete data in the ‘Online -
    Application’.
    Upload picture. S2-Online
    Click ‘Print’ button at the bottom of the Application
    Application after completing all the required Form
    fields. A successful printing of the form bears with
    the Application Number at the upper right Assessment
    portion of the form.
    Applicant to sign on the space provided in
    the application form.
    3 Log-out of the system. - -
    4 Send the following to PDEA Compliance S2 License Fee: -
    Service, 2nd Floor, PDEA Building, NIA Php 500.00 / yr
    Northside Road, National Government Php 1500/3yrs
    Center, Brgy. Pinyahan, Quezon City through
    courier or authorized representative:
    (a) Duly-signed printed Application
    Form
    (b) Required documents
    (c) S2 Official Receipt or *S2 fee, in
    cash. For the amount, please refer
    to the printed form.
    (d) *Pre-paid self addressed return
    envelope

    If application sent thru authorized


    representative, please refer to steps above
    on “How to Apply for S2 license.”
    5 Email PDEA if no feedback received within Receive document from courier. Check contents of envelope. RO I - -
    15 working days Forward document to DDRO on duty.
    If application and requirements adequate, retrieve application DDRO I
    in the system and process application.
    If otherwise, inform DDRO or online applicant of the
    discrepancy. Application withheld / pending until complied.

    Issue S2 License Official Receipt (for S2 online clients). DDB Cashier


    Enter OR in the system. DDRO I
    Print S2 License Certificate of S2 online client and inform thru
    email/ SMS of respective S2 number with information that the
    S2 Certificate shall be sent through the return envelope
    provided.
    Prepare transmittal within the day (summary of S2 applications DDRO I
    processed) for D,CS and DG, PDEA appreciation.
    Re-check all processed regional applications. DDRO II
    D, CS recommends approval (system approval). C, LRD
    DG approval (system approval).
    6 Regional DDRO receives email notification Send email to Regional DDRO. DDRO II - -
    on bulk S2 Certificate for printing purposes
    at their respective Regional Offices.

    Client receives S2 Certificate and O.R. via Send S2 Certificate.
