TITLE:
1. SCOPE
Scope of this document to determine guide provide detail on how the as built fabrication
records and documentation are compiled to meet the FD3 Requirements of EMCL Project
Specification. It also provides guide lines for retrieval of all generated fabrication
documentation and certificate compiled as per the FD3 Fabrication Record Books Index
for EMCL EPC 3 Scope of Work by PT Bakrie Construction, Sumuranja Yard.

FRB FABRICATION RECORD BOOKS (FD3) – A Systematic
compilation of all “Fabrication Documentation” generated
during the execution of the contract. The documentation
shall reflect the final “As-Built” status and include all
documentation and certification required as evidence of the
integrity of the construction and suitability and traceability of
the materials used

4. FINAL REQUIREMENTS AND DELIVERABLES

4.1. Fabrication Records Books- FRB
Fabrication Record Books are defined as the collation of all documentation and
certification produced by all departments, disciplines, including sub-contractors
manufactured items, and fabrication and testing according to the Scope of Work.
The contents are detailed in Appendix – FD3 Fabrication Record Book Index. The
FRB are include sub-contractor’s documents and sub-contractor’s vendor
documents (SEAPI, KORINDO and MULTIFAB).
4.1.1. Content and Structure of FD3

The arrangement of the documents is by discipline code and section number
defined in MCL’s “Project Technical Documentation Requirements and
Deliverable (IDBU-ED-OSPDS-000001)” and the content of information
provided according to the Appendix H of this specification.
FD3 was arranged in alphabetical discipline code as follow:

4.1.2. Traceability of Record, Report and Documents

For traceability of documentation and certification records, Discipline Code is
the first digit as the main Section followed by Code of Information Type
(documents, report or record) to be referred and group into items.
For Example: Q 001

Q = Discipline Code (Q for Quality)
001 = Sequential numbering system within each Discipline Code which refer
to information type as per FD3 (001 refer to Quality Plan)

4.2. FRB Index

This index provides information regarding the total documentation package. This
provides a cross-reference to other sections and details the Volume numbering
indicated in Appendix- A
4.3. FRB Hand Over

4.3.1. Binder Format
The FRB shall be complied in 3-holed A-4 format ring binder with a
maximum spine width of 4 inches. Each binder shall not contain more than
one discipline. Tab divider shall be included to separate the sections and
colored sheets to separate the documents. All binders shall have outside
front cover and spine transparent plastic inserts.
4.3.2. Fabrication Record Books Copies

The number of FD3 hard copies to be prepared is 1 copies.
1 electronic copy with Microsoft word or PDF format files on CD.
Retention of project documents, the contractor shall be responsibility to
maintain the FRB for retention period 30
5 years. COMPANY shall have full
access to, and the use of this documentation during this period.

5. GUIDELINES
5.1. Material and Weld Traceability
The main reference of material and weld traceability is detail shop drawing
number and weld map number. In structure, shop drawings are grouped per
equipment detail and code number which is placed at the end of shop
drawing number.
Detail numbering system of shop drawing and weld map drawing:

Step by step structure material and weld traceability guidance:

Step 1: Identify what is structure equipment code as shown in the section
5.1.1 above and then select a shop drawing number as listed in the Section
N 020 Index of Fabrication and/or Shop Drawings (with reference to design
drawings)
Step 2: With refer to shop drawing number, select weld map number as listed
in N 020 No.2 Index of Weld Map Drawings, to identify weld joint number.
Step 3: When the weld joint number identified, piece mark number will be
captured in the both of weld map drawings and shop drawings.
Step 4: Item connected show in column item reference with shop drawing as
listed in N 195 select at detail material used at traceability to mill certificate
used refer to N 111 (e.g Drawing IDBM-BC-NDSHP-JA-1000 Item A (IT 1-1

inspection 02 01 2014.
Step 6: See of welder to conduct this joint and finish of welding refer to visual
report no 847 welder stamp ( 402/424/135/457/018) with finish weld date
12/01/2014.
Step 7: will be check be NDT UT at report no 561 and MT report 950 if

actual report status Repair be UT after repair to will see report UT no 561-R1
and MT report 950-R1 with status ACC.
