Re: Mr. Peter Dunbar, DOB : 18 March 1932

Dear Sir/Madam,

I am referring Mr. Dunbar who was diagnosed with type 2 diabetes mellitus. He requires monitoring of
medication compliance as well as reinforcement regarding importance of adhering to the recommended
diet.

Mr. Dunbar has a diabetes since August 2016 which was commenced with oral hypoglycemic along with
diet and exercises. At the same time, he was advised to limit alcohol intake. However, he was not
compliant to all of the advices. As a result he suffered from myocardial infarction last june 2018.

Since Mr. Dunbar lives alone, his daugther decided to transfer him in their house so that she can provide
her fathers’ needs. Kindly educate her that Mr. Dunbar needs to consume a diet that are low in calorie,
fat and sugar. Also, please encourage him to comply with the recommended diet.

Mr. Dunbar has a various maintenance medications which are Metformin 500mg three times daily,
Ramipril 5mg, Warfarin-variable 3-5mg and Sotalol 40mg once daily. Please make sure to monitor him in
taking these medications regularly as prescribed.

It would be greatly appreciated if you could visit Mr. Dunbar and provide the necessary health teachings
about the importance of taking the medications on time as well as following the recommended diet.
