Dear Ms. Hall,

RE: Ms. Emilia Johnson, DOB: 14 August 1947, a resident of 45 Bird Wood Road, Carina Height, Queensland

I am writing with regard to the above-mentioned patient who was admitted to our facility on 1 August
2016 and was scheduled for colon polypectomy on 13 August 2016. The patient is for discharge and she
needs continuity of care at home.

The patient underwent routing colonoscopy and multiple polyps were found. Thus, colon polypectomy
was performed. Post-operative complications where noted such as respiratory distress and Arterial Blood
Gas revealed metabolic acidosis which prompted us to move her to the Intensive Care Unit. She was
moved to a regular ward on 15 August 2016 and was hooked to oxygen support as needed. Pain
medications were given such as Paracetamol 1g IV and end one 2.5 g as needed for intolerable pain. A
laxative senna was prescribed once a day at bedtime and she is now on Foley catheter with adult diaper
due to incontinence. Stable vital was noted at regular ward with an oxygen saturation of 96-97% at 2-3
liters. She has moderate post-operative pain and her wound had no exudates.

She is a retired professor and a widow, lives with her 24-year-old son in a one-storey house. She is a known
smoker and can consume 10-15 sticks a day for 35 years. She is also diagnosed with COPD in 2006, with a
maintenance of Ipratropium Bromide inhaler, 1 puff, Budesonide + Formoterol, 2 puffs, SID and
Prednisone 40mg taken as a single daily dose for acute attacks. She has been a diabetic since 2002, for
were she takes Metformin 500mg twice a day, Glipizide 5mg once a day.

At present, the patient is for discharge. Home medications and instructions were given in the presence of
her son. Please continue changing her dressing at home. You are also advised to monitor her oxygen
consumptions. Furthermore, kindly follow up check-up schedule on 30 August 2016.
