This is to refer discharge Mrs Casey, who is recovering from hip replacement surgery followed by
following/after a fall, into your care for further management. She is set to be transferred today.
On 4.02.2014, Mrs Casey was admitted to the hospital with a fractured left neck femur and had an
immediate hemiarthroplasty intervention. After the surgery, she had IV therapy with three units of
packed cells with IV furosemide after each unit. Additionally, IV Cephazolin was commenced. Because
she used to take 10 mg of Ramipril Post-operatively, her blood pressure was significantly low, so her
regular dose of Ramipril was decreased to 5 mg. To ease her pain, Fentanyl was prescribed at first and
after that, Panadol or Panadeine has been prescribed with a maximum dose of four tablets per day. Her
wound was protected by dressings, draining tubes were inserted for two days, staples were used, and
anti-embolic stockings were given. Please note, she uses a pick-up frame to get out of bed and walk
short distances. Pillows and anti-embolic stockings were used.
In view of the above, it would be greatly appreciated if you could continue Mrs Casey’s care by making
sure she takes her medication and wears the stockings for another 14 days. Please re-check her Hb
levels and her BP twice a day for three days. Help Evaluation from a rehab therapist is also
recommended. Lastly, the remaining staples can be removed on day 10, and the wound can remain
exposed afterwards. If you require any information, please do not hesitate to contact me.
