Jake Peterson, who has been suffering from intermittent cycles of alopecia, requests your
assistance and support as he transitions from high school to your university.
He first presented in the clinic 5 years ago with complaints of intermittent cycles of hair loss
in patches and regrowth following a bullying incident. A referral to a trichologist was made
and he was prescribed steroid creams for three months for which he discontinued later on.
He has shown personality changes, aggression, and sleep deprivation since then.
He returned a year ago with a recurrence of his anxiety and worsening hair loss. He was
recommended to see a doctor but preferred homeopathic treatment for which was
ineffective and had worsening physical and mental health. He also had issues with
maintaining a relationship and participation in sports activities. He also consumes at least 20
units of alcohol per week. During the exam, he was sent to a separate room due to extreme
anxiety.
With this, it would be greatly appreciated if you could assist him in transitioning to your
university and independent living. He also requires monitoring of his condition and please
organize special exam arrangements as necessary.
Should you have any concerns, please do not hesitate to contact me.
Nurse.
