EXECUTIVE SUMMARY

1. Introduction
 The purpose of performance indicator analysis is to assess and measure the effectiveness
of service processes. By evaluating key performance indicators, this analysis helps to:
1. Identify areas of improvement, track progress toward goals, and ensure alignment
with objectives.
2. Provides valuable insights that support decision-making, resources and
continuous improvement.
 National Indicator Approach is a tool use for monitoring the performance of transfusion
medicine service delivery in areas that is pertinent to safety of blood transfusion. The
quality indicators are as follows:
1. External Quality Assessment (EQA) in Pre-Transfusion Testing (≥ 90%)
2. Percentage of donation from regular blood donors ( ≥ 65%)
3. Percentage of febrile transfusion reaction investigation reported within 10
working days ( ≥ 80%)
2. Methodology
 Data were compiled every 6 months from all hospitals that offers clinical transfusion
service + blood collection via form NIA 1/2022 or form NIA 2/2022.
 All data were then tabulated in excel worksheets and analyze.

3. Key Findings
 The indicator ; “Percentage of donation from regular blood donors” shows the least
performance , this is mainly due to the covid and post covid era which sees many regular
organizers stop their annual campaigns and thus increase of lapsed donor.
 Lack of supervision leads to erroneous result entered in the EQA reporting, the need of
2nd verifier
 Management of transfusion reaction in blood bank unit without permanent Medical
Officers contributed to the longer reporting time.
 SIQ report not submitted in a timely manner and some SIQ report(s) have inadequate
information (no proper investigation, no root cause and remedial action given)

4. Recommendations

 Expansion of Blood Bank Information System (BBIS) to all collection centers, at the
moment only 22/100 centers are using BBIS. By implementing this system, donors able to
be tracked efficiently.
 In district hospital blood bank without a permanent Medical Officer, the nearest Hospital
with a specialist should conduct supervision especially in management of transfusion
reaction cases.

5. Conclusion

 Reinforce the importance of monitoring quality indicators.

1.0 External Quality Assessment (EQA) in Pre-Transfusion Testing

Standard: ≥ 90%
Rationale:
 External quality assessment (EQA) can identify pre-transfusion testing problems
within the laboratory and relate its performance relative to other laboratories. This
provides an opportunity for continuous quality improvement through the
identification of errors and the implementation of measures to prevent their
recurrence and ultimately make blood transfusion safer for the patient.

1.8 Reasons for shortfall in quality

 Testing of EQA sample was performed during on-call, while handling other routine
samples as well.
 Absence details about the sent samples leads to error during processing. Bottles are
not properly label.
 Unable to identify antibody (anti-C)
 Transcription error when submitting result from worksheet to NEQABB.
 The result for reverse action was by theory ( staff did not perform the actual reverse
action test)

1.9 Remedial action taken

 Verbal reminder given to specific staff, to be more vigilant when doing antibody
identification test. NEQAB result must be entered by officer in charge in the presence of a
2nd verifier.
 Update procedure of handling of NEQABB sample, to include 2nd verifier.

2.0 Percentage of Donation form Regular Blood Donor

Standard: ≥ 65%
Rationale:
 An adequate and reliable supply of safe blood can be assured by a stable base of
regular, voluntary, unpaid blood donors. Regular blood donors are the safest as the
prevalence of transfusion transmissible infections is lowest among this group of
donors.

2.8 Reasons for shortfall in quality

 Many are lapsed donors, post pandemic
 Wrong donor category entered in BBIS as the default is new donor
 Most of the donors did not bring the donation book. Difficult to trace.
 After pandemic campaigns are by new organizers – new donors

2.9 Remedial action taken

 Increase awareness among public on importance of regular blood donation
via social media. To dessiminate information on blood donation campaign
to public or blood donors via social media. To organize CME to educate
staff regarding the definition of donor category (new, lapsed, regular) and
to key in the donor catgeory nformation correctly in BBIS system esp for
blood donors with history of blood donation in non BBIS centre.)
 Staff involved in registration blood donors are informed regarding the new
definition of regular blood donor and the importance of correct donor
categorization.

 Blood donors are reminded to bring their blood donation booklet/other
relevant document during blood donation day.
 Strengthening the cooperation with organizers by frequently organize
blood donation drive and increase promotion to public.
 Encourage donor to become regular donors by conducting awareness and
CME to hospital staff. To encourage organizers to be regular organizers (to
increase pool of regular donors).
 Double effort in promoting as regular donor, MO: counsel/advise new
donor Nurses/bleeders: More engagement towards regular donor.

3.0 Percentage OF Febrile Transfusion Reaction Investigation Reported Within 10 Working Days.

Standard: ≥ 80%
Rationale:
 Transfusion of blood and blood components carries a small risk of acute or delayed
adverse reaction. Recognition and management of transfusion reactions is vital to
minimise the risk of harm to the patient. Therefore transfusion reaction investigation
and completed report by blood bank provided guidance regarding the diagnosis,
treatment, and prevention from subsequent adverse events.

 Unclear rules and responsibilities (Hosp Tg Karang)

 Incompetent in handling cases of transfusion reaction. Lack of supervision of
Lab Management. Newly appointed MO unfamiliar with the procedure of
reporting transfusion reaction. (Hosp Rompin)
 No permanent MO in Blood Bank.

3.9 Remedial action taken:

 All transfusion reaction shall be reported to officer in charge for fast action
and continuous monitoring. To allow dispatch of documents for Transfusion
Reaction Report to Hosp Sg Buloh on weekends to cut time and allow
acceptable time frame for HSB to finalized the report. (Hosp Tg Karang)
 Refresher training by senior MO. Lab supervisor to review the frequency of
supervision. (Hosp Rompin)
