Weekly Menu with Serving Sizes for One Age Group
Background
The California Department of Education (CDE) developed the Weekly Menu with Serving Sizes for One Age Group template to assist Child and Adult Care Food Program
(CACFP) Operators to meet the documentation requirement for all centers (except At-Risk and Emergency Shelters) to maintain daily menus that are dated, posted, and
include serving sizes per age group. A completed sample and instructions are listed below. Fillable templates are on pages 2-3 and 4-5.
Instructions
1. CACFP Operators who serve Breakfast should use the template on pages 2-3. CACFP Operators who do not serve Breakfast should use the template on pages 4-5.
2. Enter the agency name, age group, and the first day of the week.
3. In the column for each day of the week, enter the name of the food served for each component provided and the serving size for each food served. Serving size can
be stated by weight (e.g., 2 oz), by volume (e.g., ½ cup), or by number (e.g., 2 slices or 2 pieces). Refer to the CACFP Meal Patterns Charts web page at
https://www.fns.usda.gov/cacfp/meals-and-snacks.
4. Complete the cells in the table(s) for each meal and snack claimed for reimbursement. Do not enter information into cells or tables when foods are not provided. For
example, if no extras are served, there is no need to delete the text in the fillable fields for that row. These cells will print blank.
5. Indicate grains that are whole grain-rich (WGR) by writing WGR after each grain that meets the WGR criteria.
