    Dear Ms Storm,
    Re: Kate Wilson, aged 55

    This is to refer Miss Wilson into your care for further assistance and management due to relapse
    schizophrenia.

    On 12/02/2005, Miss Wilson was first admitted to the clinical centre where she was diagnosed with
    bipolar disorder, so she started treatment with olanzapine and sodium valproate; however, she was
    not compliant with the medication. Consequently, her symptoms got worse being developing
    schizophrenia the next year and for that, she was prescribed Risperidone injections every month.
    Additionally, she is a diabetic for 16 years and has a diverticulitis problem for 28 years.

    During a manic episode, Miss Wilson refused to eat anything for two weeks; therefore, she was
    readmitted to the clinic where she was given thiamine supplements to nourished her back to health.
    Moreover, her Risperidone injection was continued to be administrated, now daily, and corversyl, an
    anti-hypertensive, was added to her medication. Additionally, her medical compliance and blood
    glucose were carefully monitored.

    In view of the above, it would be greatly appreciated if you could help Miss Wilson with coordinate
    continued care. Please make sure that she is compliant with her medication and that she attends her
    appointment with her GP. Because of her isolating life, participation in a social skill training program
    is advisable. Lastly, it would be best for her to change accommodation. If you require further
    information, please do not hesitate to contact me.
